Food intolerance is defined as adverse reac-tions to foods, beverages, food ingredients or food additives, which are not due to clas-sical IgE-mediated food allergies.

Symptoms of food intolerance reactions cover a wide spectrum and are not limited to the digestive tract. While food allergy reac-tions typically occur within minutes, food intolerance reactions are predominantly delayed type reactions. In addition, food intolerance reactions are rather unspecific, as intestinal cramps, flatulence, diarrhea or constipation, urticaria, hives, headaches or fatigue.

This makes it often very difficult to identify relevant food components provoking the symptoms as well as the underlying patho-genic mechanisms, as they can be of very different nature.

In many cases, food intolerance is due to a deficit in enzymes required to digest certain food components like sugars or histamine.

There can be underlying medical conditions resulting in adverse reactions to food, e.g. by delaying food digestion or limiting absorp-tion of nutrients.

In other cases a patient may react very spe-cific to components of a certain food, like special proteins, by immunological mecha-nisms.

In the first case, appropriate diagnostic pro-cedures can be used to detect if a patient is lacking enzymes required to digest e.g. fruc-tose or lactose, for example by a simple breath test.

FOOD INTOLERANCE

In case of immunogenic reactions, identify-ing the critical foods can be very difficult. The most accepted procedure is a mixture of elimination diet and stepwise provocation with individual food components.

Unfortunately it is quite tedious and lengthy, usually performed in special clinics and can be inconclusive. So far, the mechanisms behind these adverse reactions are not well understood. Nevertheless, the measure-ment of IgG and IgG4 antibodies against food components can support and accelerate the identification of the foods that should be omitted by the patients.

Our tests are very precise tools to detect IgG respectively IgG4 antibodies in patients with suspected immunogenic food intoler-ance. They allow to test against a wide spec-trum of foods and food components at once. As we offer tests for several food panels, focused as well as wide spectrum screen-ings can be performed. In addition the prac-titioner may choose a panel that covers the patient’s dietary habits and preferences.

AESKULISA® and AESKUBLOTS® tests can be performed manually or automated, do just require only common laboratory devices and procedures. The AESKUCARE® test, a POCT device, allows to test the blood of an individual patient during patient’s visit at the practitioner’s office, requiring just a few drops of blood from a finger prick.

All our tests are CE-marked and are follow-ing the high diagnostic standards used in IgE allergy testing, but are very specific for IgG and IgG4 antibodies.

The food extracts we use in our ELISAs have been developed and optimized in-house, using state of the art tech-

nologies. The allergens are prepared and purified from native sources as fresh pro-duce.

There are different kit configurations avail-able, addressing your specific diagnostic needs, allowing verifiability, flexibility and save costs: screening, profile and single assays.

The screening kits contain such foods that quite often evoke food intolerance reactions. Other kits allow to test against a wider spectrum of foods, covering also foods that are less common.

In profile kits each well of a row is coated with different food extracts and allow you to choose between quantitative or qualita-tive analysis.

There is also the possibility to measure only one parameter with our single kits, which are suitable for quantitative deter-mination or for confirmation of qualitative results, due to its high specificity.

ELISA TESTING

Test principle

• AESKULISA® for food specific IgG/IgG4 (slgG/sIgG4) is a conventional ELISA system (Enzyme Linked Immu-nosorbent Assay).

• On the surface of each cavity of the microtiter plate extracts of foods or food mixes are immobilized.

• For the test, a serum or plasma sample is diluted and transferred to the microtiter well.

• slgG/sIgG4 antibodies will bind to the corresponding antigens of the food extract.

• Bound human sIgG/sIgG4 is detected by a specific horseradish peroxidase (HRP) conjugated anti-human IgG/IgG4 antibody. After addition of the substrate TMB (3,3‘,5,5‘-Tetramethylbenzidine) bound sIgG/sIgG4 antibodies are visualized by a blue coloration of the solution.

• Addition of stop solution interrupts the reaction and the blue color turns into yellow.

• The color intensity correlates with the amount of bound antibody and is quantified via photometric measurement.

AESKULISA® Food Intolerance LineFood intolerance reactions usually are less dangerous to patients than the classic IgE-driven food allergies, but can also significantly reduce their quality of life. Usually food intolerance reactions are delayed type reactions and can occur up to 3 days after food ingestion. This makes it very difficult to identify the relevant food com-ponents and ingredients. To rule out enzyme deficien-cies, breath tests can be used, and supervised elimina-tion diets can verify the intolerance to a specific food. IgG/IgG4 assays can be used as tools to support the practitioner in the identification process.

Food intolerance reactions have been described in chil-dren and in adults of any age, and were reported for up to a fifth of the population.

BLOT TESTING

Kit Components

Test principle

• Controls and up to 40 different antigens are bound in equispaced lines on the test strips.

• Serum sample from patient is added, during incubation specific antibodies of positive patients bind to the antigen. Afterwards, unbound substances are washed away.

• In a second incubation step, the specific antibodies are detected with an anti- human IgG4 antibody conjugated with the enzyme horseradish peroxidase (HRP). Afterwards, unbound antibody conjugates are washed away.

• The strips are incubated with a HRP substrate solution. This will stain the lines that had been recognized by the patient’s antibodies with a blue color. Densi-tometric quantification of the color intensity allows to calculate the patient’s sIgG4 level of each of the tested foods. The observed intensity of blue color is proportional to the antibody concentration in patients sample.
