Terms & Conditions條例及規則: 1. The service provider of this course is Tinytots Coaching Limited. Discovery Bay Recreation Club and Club

Siena are not a service provider of this course. Discovery Bay Recreation Club and Club Siena are only agent to the service provider for administrative work such as handling application and collecting payment from the participant.

2. Completed application forms and payment (Cash, EPS or Cheque) should be submitted to the Reception of the Discovery Bay Recreation Club (DBRC) or Club Siena prior to taking the first class.

3. Pro-rata payment will be subject to 10% admin charge of full original amount due. 4. Member guests will be subject to 25% guest surcharge of the lesson fee that based on total no. of lessons

going to be applied. 5. Any late payment (pay after the first day of class) is subject to a DBRC 20% admin charge of the total course

fee. 6. All fees are non-refundable unless your application is unsuccessful; no call means it is successful. 7. Cheque with correct amount of the course fee must be attached with this application form when submitting

the form to the Reception. Otherwise, no enrollment will be accepted. The cheque should be made payable to Discovery Bay Recreation Club Ltd.

8. After confirmation of place, failure to attend lessons in no way absolves the participant from the financial obligation. NO refunds or credit will be provided for students withdrawing after commencement of the course. In case of any cancellations form must be submitted to the reception desk prior to the course commencement date.

9. Lessons will be cancelled if Red and Black rainstorm warnings or Typhoon 8 Signal is hoisted or any adverse weather conditions that warrant the cancellation of the lesson. If lessons are being cancelled due to the above reasons, please call DBRC Reception at 2987 7381 or Club Siena Reception 2987 7382 for more information.

10. Lessons cancelled due to the adverse weather will not be refunded or rescheduled and a credit note will not be provided.

11. Classes will be automatically cancelled if no students show up 15 minutes after scheduled commencement of class.

12. Discovery Bay Recreation Club Limited and the Services Provider – Tinytots Coaching Limited reserve the right to change the venue and time of the course without prior notice.

13. Tinytots Coaching Limited reserves the right to use photos and videos of the participants and their artwork for training and promotional purposes, unless otherwise informed by you in writing.

14. Helper or lesson companion may accompany children at the Management’s discretion but must leave the venue as soon as the lesson is finished.

15. The appointed instructor will take every precaution to prevent accidents. 16. If the parent or guardian would like to stay in the venue of the course without a valid membership card or a

guardian pass, $60 entrance fee will be charged on every visit. 17. If your child suffers from any illness that may require attention during the course, please make a note on the

application form. 18. The club reserves the right to charge Members the outstanding incidental charges due to Tinytots Coaching

Limited. 19. Photos or video clips taken during class, events or shows may be used for promotional purposes. 20. Please wear proper sports shoes to attend the lesson. No spiked shoes and marking shoes is allowed. 21. Our Club deserve the right to claim for compensation for damages or loss. Description: Futsal is a fun and fast-paced 5-a-side football game played on a Basketball-sized court. Originating in South America, it is still the most widely played type of football played by children in Brazil today. The weighted-ball and tighter spaces in Futsal means players get many touches of the ball and really helps improve technical skills, creativity and decision-making. Brazilian players grew up playing Futsal and many of the greats like Pele, Ronaldinho, Neymar and Coutinho say it played a big part in their success. As one of the top football training providers in Hong Kong, Brazilian Football Academy deliver diverse football programs and clinics, which are fun, inspirational and educational.
