- ควบคม pH ของเลอด (buffer): ความสามารถในการให/รบ

Principle: โปรตนตางชนดใน buffer เดยวกน จะม

net charge แตกตางกน

จงเคลอนทในสนามไฟฟาไดระยะทางตางกน

pH 8.6: plasma protein สวนใหญ จะม net

charge เปนลบ แตจะม net charge ตางกน

: เคลอนทไปทางขวบวก

: แยก plasma protein เปนแถบๆ ตามการเคลอนท

ไดเปน 5 กลม (แถบ) ใหญๆ

albumin ในเลอดต า ภาวะบวมน า (edema)

ระดบ plasma protein ทต าทสด ทยงไมท าใหเกดอาการบวมน า

เรยกวา edema level

- solubility of hydrophobic substances: LCFA

- detoxify toxic substances: unconjugated bilirubin

- transport: hormones, drugs

การรกษา pH ของเลอด

- สามารถจบกบ H+ ทเกดขนในรางกาย

- ชวยลดความเปนกรดในเลอดได

ม half life ประมาณ 20 วน

ระดบ albumin จงบงชถงภาวะทางโภชนาการของรางกายในระยะยาว

การเปลยนแปลงระดบ plasma albumin ในระยะสน อาจเกดจากการ

เปลยนแปลงปรมาตรของเลอด ซงสงผลใหความเขมขนของโปรตนเปลยนแปลง ในขณะ

ทปรมาณสทธของโปรตนไมเปลยนแปลง

คาเฉลยของ plasma albumin ในผใหญปกต: 35-55 g/dL

ความผดปกตของ albumin ในเลอด

1. analbuminemia: absence (or very low level)

2. hypoalbuminemia: low level

1. hyperalbuminemia: high level

- autosomal recessive, พบไดนอย - เกดความบกพรองในการสงเคราะห albumin - ไมสามารถตรวจพบ albumin ในเลอดไดหรอพบในระดบทต ามาก

- dehydration - ความเขมขนของ albumin สงขน

- ปรมาณสทธของ albumin ไมเปลยนแปลง

Hypoalbuminemia: ระดบ albumin ในเลอดต ากวาปกต

- การสงเคราะหทตบลดลง (ขาด protein, liver failure)

- สญเสย albumin ออกนอกรางกาย (ปสสาวะ, ผวหนง, ล าไส)

- การสลาย albumin ทเพมมากขน (thyrotoxicosis)

โดยเฉพาะใชเปน tumour marker ในตรวจกรองมะเรง

ระดบสงกวาปกต: มะเรงตบชนด hepatocellular carcinoma,

germ cell tumours หรอ มะเรงทกระจายไปทตบ

• ธาตเหลกสะสมในเนอเยอตางๆ

• copper deficiency, malnutrition

• severe liver diseases

ปองกนพษทเกดจาก free iron

ความผดปกตของ transferrin ในเลอด

1. ระดบ transferrin ในเลอดต า

- congenital atransferrinaemia: พบนอยมาก

- ภาวะทรางกายมการสงเคราะหโปรตนโดยรวมต า

(malnutrition, liver diseases)

- ภาวะทรางกายมการสญเสยโปรตน (โรคไตบางชนด)

- การอกเสบ: negative acute phase reactant

2. ระดบ transferrin ในเลอดสง

- iron deficiency, pregnancy

สงเคราะหโดยตบ

หนาทหลก: จบกบหมฮมอสระในเลอดเพอน าไปก าจดทตบ

ฮมอสระในเลอด: การสลายของ hemoglobin, myoglobin หรอ

heme proteins อนๆ เชน ในภาวะ intravascular haemolysis

สามารถจบกบ porphyrins ไดดวย

hemopexin ในเลอดต ากวาปกต: intravascular haemolysis

C-reactive protein (CRP)

glycoprotein, สงเคราะหโดยตบ

จบกบ phosphatidylcholine บนผวเซลลทเกดพยาธสภาพ, จลชพหลายชนด

น าไปสการก าจดเซลลหรอจลชพนน โดย immune system

ถาตมตอท 85-100๐C จะสามารถละลายได
