    Introduction The purpose of this questionnaire is to establish the contractor scope of business and evaluate the level of necessary resources, ability and expertise to perform any work that may be tendered by BG prior to any site visit and assessment which may be required. It is recommended that prior to completing the full Questionnaire, the Contractor answers the question(s) posed in the following Section titled "Contractor Capability Statement - MANDATORY REQUIREMENTS".

    Contractor Capability Statement - MANDATORY REQUIREMENTS 1 Start-up costs are considered to be in the region of $ 000.000,00 and are likely to be expanded by the successful Contractor prior to settlement of Contractor's first invoice.     Section 1 Company Profile

    1.1 Name and address of company, including telephone and facsimile numbers and electronic mail address.

    1.2 Place of Registration and Trade Register Number

    1.6 Company's principal business, giving the full range of services you can provide or

    goods you can supply, for which you would be interested in submitting tenders.

    1.7 Details of Ownership/Corporate Organisation:

    1.7.1 Name of Parent, Associated and Subsidiary companies; details of membership of any Group of companies; Organisation chart depicting relationships within group.

    1.7.2 Name of major shareholders in company and their percentage of equity

    interest.

    1.7.3 Name of Managing Director or Owner

    1.8 Provide Tax Registration details and Numbers

    1.9 Provide Fiscal Attestation proving compliance with Directorate of Taxes

    1.10 Provide recent Non-bankruptcy/legal prevention certificate or sworn statement that your company is not in a situation of bankruptancy/legal prevention.

    1.11 Address to which Tenders and pre-qualification correspondence should be sent,

    Section 2 Commercial

    2.1 Provide copies of audited accounts for previous three (3) years

    2.2 Current annual turnover for company

    2.3 Copies of Annual Reports for last three (3) years

    2.4 Financial limits for any one Contract/Order

    2.5 Maximum value of work per annum you would be capable of undertaking

    2.6 External Auditors details (who may be asked to provide a reference)

    2.7 Insurance Details: Please provide details including: name(s) of Underwriter; level of cover; limits;

    deductibles/excess and expiry dates for the following: Public Liability Insurance cover Contractor's All Risk cover Professional Indemnity cover

    2.8 Please provide details of any contractual Claims disputes you been involved with in the previous five years.

    Section 3 Experience

    3.1 Provide details of relevant and recent experience of any major contracts which you wish to undertake for BG and which you have executed within the last three years.

    The details to be provided should include: Date; Value; Name & Address of Client

    including name of referee and phone number; Title of Contract/Order; Description of Work/Goods/Services provided.

    3.2 Please demonstrate that you have the experience and expertise, current and past, in

    the execution of the type of work envisaged.

    3.3 Can you demonstrate that you have the experience and competence to carry out the envisaged work.

    Section 4 Resources

    4.1 Personnel

    4.1.1 Please provide a copy of your company Organisation Chart

    4.1.2 Provide breakdown of the management, technical, engineering, administrative, manual and specialist manpower resources in your organisation:

    4.1.3 Detail the manpower resources available to work on this project/contract

    4.1.4 Give details if there have been any significant changes in the number of people employed by your company during the last 12 months

    4.1.5 Will your Onshore/Offshore supervisory personnel, in daily contact with BG Libya Staff,

    be able to speak and write English fluently.

    4.1.6 Do you have personnel at the office from which this contract would be managed, who can fluently speak and write English sufficiently well to understand technical, operational and commercial enquiries.

    4.1.7 Provide details of how you supplement your current permanent workforce (methods of

    recruitment) and: - How do you select personnel for onshore/offshore work - At what stage do you check references - At what stage do you verify qualifications - At what stage do you require a medical examination - At what stage do you verify that personnel have valid certificates for

    offshore safety training.

    4.1.8 State type of work that is typically subcontracted and advise under what circumstances you subcontract work or use subcontract labour i.e:

    4.2 Training - Craft + Professional

    4.2.1 Give details of any in-house training facilities and/or Training Department

    4.2.2 Provide the following details of your company's Training Policy, including that of Management:

    a) Training Bodies which the company utilises b) Number of Apprentices, Trainee Engineers, Graduate Trainees and other

    Trainees. c) Relationships with any educational establishments d) Training centres used and courses attended by personnel in order to

    support/carry out Type of work envisaged under this contract/project.

    4.2.3 Provide a summary of the training courses undertaken by your employees within the last 2 years.

    Section 5 Quality Assurance and Quality Control

    5.1 Provide details of your company's Quality Assurance Policy

    5.2 Provide details of any accreditation of your quality management system by a recognised authority (ISO 9001-2000 or EN 29001), and submit copies of the certificates.

    Please note that your company may be subject to a Quality Audit to verify that system is in place.

    5.3 Provide details of any standard procedures used within your organisation for managing

    and ensuring quality.

    5.4 Provide details of how you ensure the quality and adequacy of materials, plant and equipment that may be supplied under the contract.

    Section 6 Health, Safety and Environmental

    6.1 Provide details of your Safety Management structure and reporting system

    6.2 Provide details of method of accident and incident reporting

    6.3 Provide Safety statistics for last three (3) years

    6.4 Details of how you communicate any general and specific health, safety and environmental rules and procedures to your employees and subcontractors.

    6.5 Details of your emergency response procedure - for both onshore and offshore.

    6.6 Explain briefly how Safety issues are resolved in your company.

    6.7 Give the name and title of your Safety Officer.

    6.8 What arrangements exist in your company for First Aid training and will any of the staff

    employed under the contract envisaged possess a formal First Aid qualification.

    6.9 How often is your workforce examined by a Medical Physician.

    6.10 Explain how staff are made aware of their duties and responsibilities under current legislative requirements, and how will any future changes in legislative requirements be communicated to staff.
