    Concern: Fatality incident of a Hydra Crane helper who slip and came under the left front wheel of Hydra and sustained seriousinjury.

    Concern: Many incidences noticed due to failure of Brake caused by problem in Air lines

    Action Taken: Fail Safe Brake in all heavy vehicles made mandatory to avoid any hazard due to main brake failure

    Concern: Many incidences occurred due to toppling of three wheeler Auto.

    Action taken: It was decided to replace all three wheeler with four wheeler vehicle.

    Concern: Many cases of foreign body in eye and head injury of cyclists were noticed due to non use of Safety helmet & SafetyGoggles

    Action Taken: It was made mandatory to use Safety helmet & Safety Goggles during cycle riding.

    Concern: Vehicles coming inside works used to park here & there creating congestion on road.

    Action taken: A designated parking space named as Transport Park for 900 vehicles parking capacity is provided to reducecongestion on road.

    Title: Eliminate Fatality Potential due to unwanted movement of helpers

    Concern: Helpers coming inside works with vehicles used to sit, sleep, and cook under the vehicle, caused several fatality of

    helpers.

    Action Taken: It was decided to stop helpers coming inside works with heavy vehicles and to promote helper less drivingwith the help of Three piece mirror and reversing audible buzzer. For loading / unloading & signaling to place vehicles at loading /

    unloading points trained crew members are provided at loading /unloading points.

    Title: Eliminate Fatality Potential by reducing movement of Cyclists & Pedestrians on

    main Road

    Concern: Thousands of cyclists and pedestrians were wandering on road to move one site to another, by the same timenumbers of heavy vehicles plying on same road. It creates fatality potential on road.

    Action Taken: To reduce movement of cyclists & pedestrians on main road, Free bus services are provided to facilitate

    employee to go one site to other.
