1 Medical PlansOur two medical plan options provide comprehensive health and prescription drug coverage. One plan is also coupled with a health savings account (HSA).

2 HSAThe HSA allows you to set aside pre-tax funds to help cover eligible qualified health care expenses. It offers many valuable advantages.

3 Dental PlansOur two dental plans offer 100% coverage for in-network preventive services, such as annual cleanings, as well as partial coverage for basic and major services.

4 Vision PlanOur vision plan offers coverage for annual exams, basic lenses, frames and contact lenses.

5 Voluntary TeleHealth ProgramThis program allows you and your covered dependents to consult with a doctor by phone or online for common non-emergency medical issues and receive a prescription, if needed.

6 EAPOur company-paid employee assistance program (EAP) provides all employees and their family members with 24/7 confidential assistance on issues related to your family, career, finances and emotional well-being.

7 Life and AD&D InsuranceWe offer company-paid basic term life and accidental death and dismemberment (AD&D) insurance equal to one-half of your base annual earnings and voluntary

life insurance options that help you maintain financial security.

8 Disability InsuranceOur voluntary short- and long-term disability (STD and LTD) coverage pays you a percentage of your income should you need to take time off due to a serious illness or non-work-related injury.

9 Universal Life, Critical Illness and Accident InsuranceThese voluntary coverage options give you the opportunity to augment your existing coverage for even greater financial security.

10 401(k)Our 401(k) allows you to contribute to the plan effective the first of the month following six months of continuous employment. AH4R will

match $1.00 for every dollar you contribute, up to 3% of your pay and $0.50 for every dollar you contribute between 3% and 5% of your pay. To receive the full match of 4% of pay, you must contribute 5% or more of your pay.

11 HolidaysYou are eligible for 9 paid holidays per calendar year.

12 Paid Time Off (PTO)You may accrue up to 120 hours of PTO each year for the first three years of continuous service. You may accrue up to 160 hours per year thereafter.

13 Leaves of Absence (LOA)An LOA may be granted to employees under the following circumstances: medical, family and military leave.

14 Office HoursOur typical hours are 8:30 a.m.–5:30 p.m.

Medical Plans: UnitedHealthcare (UHC)AH4R offers two medical plan options through UHC that not only provide coverage for illness and injury, but also enable you and your family to focus on staying well. Both plans: • Offer comprehensive healthcare and prescription drug coverage • Cover qualified, in-network preventive care at 100%

Following are some key points to consider when choosing your medical plan:

UHC EPO Select Plan • The plan provides coverage for in-network services only,* meaning

you may only seek care from a contracted provider participating in UHC’s “Select Network” (an in-network provider) in order to utilize healthcare services. Note that referrals are not required for specialty care. • Qualified, in-network preventive care is covered at 100%. For non-

preventive services, you will either pay a copayment or you will pay coinsurance (a percentage) of the costs after your calendar- year deductible is satisfied.** UHC High-Deductible Health Plan (HDHP) PPO Select Plus Plan With Health Savings Account (HSA) • This is a “consumer-driven” plan. It offers lower premiums

in exchange for a higher deductible. Qualified, in-network care is covered at 100%. For non-preventive services, you are responsible for paying the contracted allowable amount of healthcare expenses until your calendar-year deductible is satisfied.** • However, the plan is coupled with a powerful savings vehicle,

called an HSA, that enables you to pay for current, qualified healthcare expenses and save for future expenses on a tax-free basis.*** • You have the freedom to seek care from both in-network and

out-of-network providers. However, you will likely pay less when you use in-network providers, as they have agreed to reduced, negotiated rates for their services.

*Out-of-network benefits are available to employees in Arizona.**Once the calendar-year deductible is satisfied, all remaining out-of-pocket costs will be applied toward the calendar-year out-of-pocket maximum. Deductibles, copayments and coinsurance amounts are all applied toward the calendar-year out-of-pocket maximum. ***State taxation rules vary.

Health Savings Accounts (HSAs): Sterling Health Services AdministrationBasically, the HDHP PPO Plan, along with your HSA, puts health care spending in your hands. With lower premiums to pay for coverage, you choose how to spend your healthcare dollars. You can either pay for eligible services by using funds in your HSA, or you can pay for them out of your own pocket.

HSA AdministrationFor your convenience, your HSA is held with Sterling and offers payroll deductions into the HSA on a pre-tax basis. Sterling also offers two account options: • The Value Plan has a monthly management fee of $2.50, which

is paid in full by AH4R. However, you are responsible for any additional fees associated with transactions for the account. As an example, there is a $1.50 usage fee deducted from the account for each debit card swipe.

• The Standard Plan has a monthly management fee of $8.75. On a monthly basis, AH4R will pay $2.50 and you will be responsible for the difference of $6.25. With this plan, there are no additional fees for transactions within the account.

NOTE: Under both plans, monthly maintenance fees are payroll deducted. Additional contributions used to cover the cost of the transactional fees do not apply to the IRS maximum.

How Your HSA Is FundedThere are several ways to contribute money to your HSA: • Pre-tax contributions through payroll deductions • After-tax cash contributions that are deductible when you file

your taxes • Catch-up contributions up to $1,000 per year if you are over age 55

(until you enroll in Medicare)

Total Annual Contribution LimitIt is important to note that your contributions may not exceed the IRS maximum (refer to the table below). Note: Individuals 55 and older may make additional “catch-up” contributions up to $1,000 each year until they enroll in Medicare.

Qualified Health Care ExpensesHSAs enable you to pay for the following qualified healthcare expenses on a tax-free basis: • Qualified medical, dental and vision expenses not covered by the

plans, as defined by the IRS in Publication 502, available online at http://www.irs.gov/pub/irs-pdf/p502.pdf • COBRA premiums • Qualified long-term care insurance and expenses • Health insurance premiums when receiving unemployment

compensation • Medicare and retiree health insurance premiums (not Medicare

Supplement premiums) • Medigap insurance premiums

Advantages of an HSAThere are numerous benefits to owning an HSA: • Triple-tax advantage:*

1. You contribute pre-tax funds through payroll deductions, meaning the money comes out of your paycheck before federal income tax is calculated. This, in turn, reduces the amount of taxable income, so less tax is withheld from your paycheck. 2. Funds grow tax-free, and unused funds roll over year to year. 3. You can withdraw funds tax-free to pay for qualified health care expenses now and in the future—even in retirement. • Control: You own and control the money in your HSA. You decide

how or if you want to spend it. You can use it to pay for doctor’s visits, prescriptions, braces, glasses—even laser vision correction surgery. • Investment opportunities: Once you reach and maintain a

minimum account balance, you can make investments to help your money grow tax-free. • Savings potential: There is no “use it or lose it” rule. Your account

grows over time as you continue to roll over unused dollars from year to year. • Portability: Your HSA is yours for life. The money is yours to spend

or save, regardless of whether you change health plans,* retire or leave the company.

*State taxation rules vary.**You must be enrolled in a qualified high-deductible health plan to contribute to an HSA.

Voluntary TeleHealth Program: TeladocDid you know that 72% of doctor visits are for common conditions, which can easily be treated without an in-person visit to a healthcare provider? Whether you are at home, work or on the road, TeleHealth gives you access to U.S. board-certified doctors and pediatricians 24/7/365 via your mobile device or computer. You can get the care you need when and where it’s convenient—at a fraction of the cost of a visit to the ER or urgent care center. Even better: doctors can write a prescription,* if needed, that you can pick up at your local pharmacy.

With TeleHealth, you can get advice within minutes for a variety of non-emergency medical issues, including (but not limited to) allergies, anxiety, back problems, bronchitis, cold/flu symptoms, ear infections, diarrhea/constipation, headaches, rash/skin problems, sore throat/stuffy nose, sprains/strains and more.

*Prescription services may not be available in all states.

Employee Assistance Program (EAP): UHCAt no cost to you, AH4R provides you with access to an EAP. The EAP provides confidential, 24/7 coaching to help you manage the stressors and challenges of work, life, personal and family-related issues. Both you and your eligible household members have unlimited telephone access, online tools and resources and up to five free face-to-face counseling sessions per issue, per year. Face-to-face and telephonic sessions are 100% confidential and not reported back to AH4R.

Dental Plans: UHCAH4R offers two dental plan options through UHC:

DHMO Plan (California Only) • This plan provides coverage for in-network services only and

requires you to select a contracted dental provider. • Most preventive services are covered at 100%.

Note: You can only use HSA funds as they are deposited in your account. You can always reimburse yourself later once you have accumulated funds in your account.

DPPO Plan • This plan offers you the freedom and flexibility to seek care from

both in-network and out-of-network providers. However, you will likely pay less when you use in-network providers, as they have agreed to reduced, negotiated rates for their services.

• The plan covers diagnostic and preventive care in-network at 100%. Other benefits are payable after you meet the deductible. Orthodontia benefits are also covered for children and adults with a $1,500 lifetime benefit.

Vision Plan: UHCEligible employees have an opportunity to enroll in the PPO Plan offered through UHC. Members have the freedom and flexibility to use both in-network and out-of-network providers. However, you will likely pay less when you use in-network providers, as they have agreed to reduced, negotiated rates for their services.

Term Life and Accidental Death and Dismemberment (AD&D) Insurance: Liberty MutualAt no cost to you, basic term life and AD&D insurance is provided to all eligible employees. You also have the opportunity to purchase additional, voluntary term life and AD&D insurance at affordable group rates. NOTE: If you want to elect coverage for your spouse/domestic partner or child(ren), you must first elect voluntary term life and AD&D insurance for yourself.

Basic Term Life and AD&D Insurance

Disability Insurance: Liberty MutualAH4R offers voluntary short-term and/or voluntary long-term disability (STD or LTD) insurance through Liberty Mutual. Disability insurance provides benefits that can replace a portion of your lost income if you become unable to work due to a covered injury or illness.

Additional Voluntary Benefits: TrustmarkAH4R offers the following voluntary coverage options to give you the opportunity to augment your existing coverage for even greater financial security: • Universal Life Insurance: In addition to the Liberty Mutual

voluntary term life insurance, you also have access to a universal life insurance plan through Trustmark. This plan offers permanent life coverage for you and your entire family while building cash value on a tax-deferred basis. Employees up to age 70 are eligible to apply for universal life coverage.

• Critical Illness Insurance: In the event of a serious health problem such as a stroke or heart attack, this coverage complements your benefits package by paying for unforeseen additional expenses not covered under your health plan. This financial benefit can help offset potential expenses. Examples of additional expenses are lost wages, experimental treatments, travel and lodging expenses en route to treatment facilities, in-home care and more.

• Accident Insurance: Having an accident can affect your finances. Trustmark’s accident insurance gives you something to fall back on. Accident insurance can pay benefits based on the injury you have and the treatment you need, including emergency room care, doctor visits and related surgery. This benefit can help offset the out-of-pocket expenses that medical insurance does not cover, including deductibles and copays.

single point of contact for all of the benefit programs

listed in this brochure. The BSC Benefit Specialists can

answer questions you may have about how your plans

work or how to access healthcare providers. Additionally,

the BSC Benefit Specialists will guide you through

making changes if you experience a qualifying life status

event (e.g., marriage, divorce, birth, adoption, etc.), as

well as supply the appropriate forms you may need and

assist you with resolving claim issues.*

The BSC Benefit Specialists are available at [email protected] or (800) 842-6635, Monday through Friday, from 8:30 a.m. to 5 p.m. (PT).
