Medical Insurance

Two health plans are offered and you have the option to elect the plan that best suits your needs. The PPO Open Access HSA plan provides both in-network and out-of-network coverage and the HMO Open Access HSA plan provides in-network coverage only. Singer Nelson Charlmers offers a Health Reimbursement Arrangement (HRA) with a choice of set-up options.

Dental Insurance

Dental insurance is available to all employees and dependents even if you are waiving medical insurance. Contributions are required to enroll in the dental plan, but once an employee has completed five years of employment, the company pays the full cost for single coverage. We offer one dental plan option. You can visit any dentist but you pay less out-of-pocket when you choose a participating dentist.

The Met Life plan offers: MetLife Dental PPO Network. Family coverage for spouse and children. No charge for preventive care. You have the flexibility to visit your dentist or any licensed dentist, in or out of the network. But

with one of the largest dental networks in the industry, there's a good chance your dentist is in the network.

No referrals required and members can opt in and out of network. Find out if your dentist is in the MetLife network at www.MetLife.com or contact a

knowledgeable Dental Benefits Representative at 1-800-942-0854. Customer Service can be reached at 800-988-8333, following the prompts (Press 1, enter

Vision

Vision insurance is available to all employees even if you are waiving medical and/or dental insurance. The VSP plan provides both in-network and out-of-network coverage and the company pays the full cost for both you and your dependents.

Your eyes say a lot about you and can even tell your VSP doctor about you. During your WellVision Exam®, your VSP doctor will look for vision problems and signs of health conditions too. VSP alsooffers discounts on additional pairs of glasses and sunglasses and Laser Vision Correction.

Health Reimbursement Arrangement (HRA)

Administered through Flex Facts, Singer Nelson Charlmers has designed an HRA coupled with a High Deductible Health Plan that can save you money. Both of the company’s medical plans have an upfront $2,500 combined Medical/Rx deductible for single coverage ($5,000 if you are covering other family members). The company supplements these medical plan deductibles through a Health Reimbursement Arrangement (HRA) that reimburses you for half of the deductible applied expenses. You also have the choice of whether it’s the first half or the second half. In general, electing the second half reimbursement allows you to have and fund a Health Savings Account (HSA) and electing the first half gives you a more immediate reimbursement of expenses applied to your deductible. Your specific circumstances will dictate which half is more advantageous to you.

Through the HRA, you receive:

Web access to transaction history, available benefits, and claims history Single debit card to be used at the point of service Fast and accurate daily claims reimbursements Real-time access to account information via either phone or the Internet Convenient enrollment process

Hospital Deductible Insurance

Benefits You Receive:

Voluntary Hospital Deductible Insurance (HDI) is available to purchase as a supplement to your group medical insurance. This is available to you and your dependents at the monthly rates below.

The HDI plan provides a lump sum payment to help offset a High Deductible Health Plan deductible gap. Benefits are payable when a covered person is confined as an inpatient for at least one night. Benefits are payable once per covered person per plan year. This plan can be an advantageous supplement to our health insurance plans which have deductibles, copays and coinsurance for inpatient hospital stays.

Flexible Spending Accounts (FSA)

Benefits You Receive:

FSA’s provide you with an important tax advantage that can help you pay health care and dependent care expenses on a pretax basis. By anticipating your family’s health care and dependent care costs for the next year, you can actually lower your taxable income.

Health Care Reimbursement FSA

This program allows Singer Nelson Charlmers’ employees to pay for IRS-approved medical care expenses, not covered by their insurance plan, with pretax dollars when a prescription is provided. Some examples include:

● Deductibles, Coinsurance and Copays for qualified Medical and Prescription Drug Expenses

Contact lenses, contact lens solution, eye examinations and eyeglasses Dental services and orthodontia Chiropractic services Acupuncture Over the counter drugs (doctor’s written prescription required as of January 1, 2011)

You should carefully consider your needs when determining your FSA election amount as it operates on a “use it or lose it” basis. The annual maximum that you can contribute to the FSA for whether you are single or have a family is $2,500.

Dependent Care FSA

The Dependent Care FSA allows Singer Nelson Charlmers’ employees to use pretax dollars toward qualified dependent care such as caring for children under the age 13 or caring for elders. The annual maximum amount you may contribute to the Dependent Care FSA is $5,000 (or $2,500 if married and filing separately) per calendar year. Examples include:

The cost of child or adult dependent care The cost for an individual to provide care either in or out of your house Nursery schools and preschools (excluding kindergarten)

Long Term Disability BenefitsTemporary Disability Benefits

Benefits You Receive:Singer Nelson Charlmers provides full-time employees with short term disability income benefits under NJ’s statutory short term disability insurance through Sun Life. In the event you become disabled from injury or sickness, disability income benefits are provided as a source of income. Contributions are shared between the company and the employee as determined by the State.

Long-term disability insurance is available to all employees on a voluntary basis. The plan covers both total disabilities (i.e. not able to work at all) and partial disabilities (i.e. unable to work full time or unable to perform all your job duties).

The chart below describes the benefits in more detail for both short and long-term disability.

Life & AD&D Insurance

Benefits You Receive: Basic Life & AD&D InsuranceSinger Nelson Charlmers provides full-time employees with $25,000 of group life and accidental death and dismemberment (AD&D) insurance. Contact Human Resources to update your beneficiary information.

Voluntary Life & AD&D InsuranceEmployees who want to supplement their group life & AD&D insurance benefits may purchase additional coverage. When you enroll yourself in this benefit, you pay the full cost through payroll deductions. You can purchase coverage on yourself in $10,000 increments up to five times salary. Minimum coverage is $10,000 and maximum coverage is $500,000. You can also purchase coverage for your spouse in $5,000 increments and children in $2,000 increments. Maximum benefit for spouse is $250,000 and maximum benefit for children is $10,000. The first $50,000 of your additional coverage and the first $25,000 of spousal coverage can be purchased without regard to your or your spouse’s health condition but coverage in excess of these Guaranteed Issue amounts require evidence of good health and is medically underwritten.

Long Term CareLong Term Care Plan

Long-term care insurance generally covers custodial care including home care, assisted living, adult daycare, respite care, hospice care, nursing homes, Alzheimer's facilities, and more. You must be unable to perform at least 2 out of 6 Activities of Daily Living (ADL) as follows: Bathing, Dressing, Toileting, Transferring, Continence, Feeding.

Premiums paid on a long-term care insurance product may be eligible for an income tax deduction.

Benefits Provided:

Your plan’s Elimination Period of 60 consecutive days is the amount of time you must wait before benefits become payable. You can purchase $3,000-$8,000 of monthly coverage in $1,000 increments (with or without inflation) with a 3 year, 6 year or lifetime duration. After two years of employment the company provides employees working at least 25 hours a week with a Basic $3,000 coverage plan (no inflation) with a 3 year payment duration.

Guaranteed Issue Period:

The first opportunity for an employee to enroll in the Guaranteed Issue Period is as a new hire after working for 90 days. The second opportunity to enroll in the Guaranteed Issue Period is when you have been employed for two years at which time company’s Basic plan coverage begins. Coverage amounts up to $6,000 (with or without inflation) with either a 3 or 6 year duration can be purchased during these Guaranteed Issue Periods with no medical underwriting. Medical underwriting is necessary if you choose to buy $7,000 or $8,000 of coverage, or the unlimited lifetime payment duration. Medical Underwriting is required for ALL other family members regardless of their coverage election.

Business Travel Accident

Benefits You Receive:

Global ProtectionSinger Nelson Charlmers provides you with Group Travel Accident coverage. This benefit offers you 24-hour global protection while traveling on company business. The Accidental Death & Dismemberment coverage provides a schedule of benefits covering loss of life, limb, sight, hearing and speech. Each employee is covered for a Principal Sum of $250,000.

The beneficiary will be the same as named under our group life plan unless you make a separate designation. An option is included for a seatbelt benefit which provides an additional 10% of the Principal Sum up to $2,000.

In the event of an emergency, the policy covers Emergency Medical Evacuation up to $50,000, Repatriation of Remains up to $10,000 and Emergency Prescription Medication and Legal Referral including bail bonds.

Travel AssistanceThrough Cigna Secure Travel you also have access to pre-trip planning services, including foreign travel assistance, assistance with lost or stolen items, translation and interpretation services and 24-hour multilingual assistance.

Offered in your EAP, or Employee Assistance Program, is a confidential hotline to discuss some of the problems you face. It is open 24 hours a day, 7 days a week. Call any time with personal concerns, including:

401(k) Plan

401(k) Plan

Benefits You Receive:Singer Nelson Charlmers offers employees the option to participate in a 401(k) Plan as long as you have attained the age of 21 and completed 90 days of service.

The Plan allows for Singer Nelson Charlmers to make contributions to an employee’s plan at 25% of the first 5% contributed by the employee.

If you have an existing qualified retirement plan (pre-tax) with a prior employer, you may transfer or roll over that account into the plan anytime.

The plan is administered by a company called Klerer Financial.You can choose Investment options that are provided through Nationwide.

Identity Theft

ICR Bridge – Identity Theft

Benefits You Receive:Singer Nelson Charlmers offers employees the option to participate in an Identity Theft program through ICR Bridge.

ICR Bridge is a reputable company that provides you and your family with various credit monitoring services that will track all changes to your financial or public records. You also have access to an on-call fraud specialist who will offer you unlimited assistance to restore your identify. Additionally, a personal insurance policy and reimbursements are available.

Because ICR Bridge is concerned with your personal and private information, they monitor over 1,500 governmental data bases such as all 50 states DMV’s, USPS address changes, police records, etc. They will notify you within 24 hours if any of these data bases register a change. You will be alerted of any suspicious activity through secure e-mail.
