Women’s Health and Cancer Rights Act Notice

We would like to remind you about the Member Assistance Program that was introduced in 2017. It is available to all non-Medicare PPO Plan enrollees eligible in the Health and

Welfare Fund, and their household members (not restricted only to eligible dependents). The MAP is a free, confidential, professional consultation and referral program. This program is designed to help you and your household members address and resolve personal problems that may be interfering with work or home life. There are no costs to you for these services and counseling is provided for up to eight (8) sessions per issue/incident per eligible person, per year.

The Health & Welfare Fund has contracted with MHN to provide these professional services for you 24 hours a day, 7 days a week, 365 days a year. Information, guidance, or

assistance can be obtained by calling: (800-327-0577). These no-cost counseling sessions are only available through MHN.

When you call the Member Assistance Program, a trained intake specialist will assist you with obtaining services. They will provide you with names and telephone numbers for providers best suited to meet your needs (In-network providers where available). Out-of-Network Docs at In-Network HospitalsI stayed in my Network….Why did I get a bill?You’re scheduled for surgery, and you’ve done your homework. You know that your doctor is admitting you to a hospital that participates in the Anthem Blue Cross network – our PPO network. You’ve checked that your surgeon participates as well. So, there shouldn’t be any surprises, right?

Even if your hospital participates in our PPO network, that doesn’t mean that all the providers working there do as well. If you need surgery, or have a serious illness, there may be several providers involved in your treatment. And each of them may or may not contract with Anthem Blue Cross.

When you receive treatment in a hospital, be aware that you may get a bill from providers who don’t participate in the PPO network, such as:• ER doctors • Radiologists • Surgeons assisting your• Pathologists • Anesthesiologist in-network surgeon

What Can I Do?Although the Health Plan does cover non-network care, it is at a lower percentage (70% of usual and reasonable charges versus 90% of the contracted amount for in-network) and after a $500 deductible versus $250 in-network deductible. But the non-network provider may also bill you for the difference between their “billed charges” and Anthem’s determination of “usual and reasonable charges.” While you can try to negotiate after you’ve already received the bill, out-of-network providers are under no obligation to accept a lower payment.

The best way to try to avoid this situation is to ask questions. Talk to your doctor and the hospital admitting office to see if all of the providers involved in your care are contract providers with Anthem Blue Cross.

What about Emergencies?If you’re in a car accident, suffer a heart attack, or have another emergency, you typically don’t have a choice about where to go for care. You’ll usually be taken to the nearest hospital, which may or may not participate in our Anthem Blue Cross PPO network. Although the Health & Welfare Plan will cover emergency care at the in-network rate of 90% whether the hospital is in-network or not, you may be billed by the non-network hospital and/or the non-network ER doctors for the difference between their “billed charges” and Anthem’s determination of “usual and reasonable charges”.

But remember, this only applies to real emergencies. If it’s not a true emergency, the hospital costs for care received in the ER of a non-network hospital will be covered at the lower non-network rate (70% of usual and reasonable charges), the higher $500 deductible will apply and you would be subject to “balance billing” as described above.

Medicare Eligible Retirees Have Choices in Health Care Retirees enrolled in Medicare and eligible in the Health & Welfare Fund have three options for their health care. Pick the option that best fits your needs. If for any reason you decide you don’t like the option you are in, you’re not locked in. You can change the beginning of the next month.

The three available options are:

1. Kaiser Senior Advantage HMO. This plan is not available in all areas. • Generally, only $5 copays for medical care and $5 copays

for prescription drugs. • This option is an HMO where all medical care and

prescription drugs must be obtained through Kaiser facilities, providers and pharmacies.

• Monthly premium of $100 per retiree, $200 retiree and spouse.

2. UnitedHealthcare (UHC) Medicare Advantage PPO. This plan has been significantly improved from when it was first rolled out in 2016.• No copays for medical care and $5 to $30 copays for

prescription drugs.• This is a PPO plan and does not have restrictions on in

and out-of-network coverage. You have access to UHC’s national network or can see any other provider as long as the provider participates in the Medicare program and will accept assignment of benefits from UHC. The provider does not have to be in the UHC network. Although 96% of the providers used by our members will accept UHC assignment, some do not. So, if you are considering this option, we suggest checking with providers you use on a recurring basis if they will accept UHC assigned claims.

• Monthly premium of $160 per retiree, $320 retiree and spouse.

3. Operating Engineers PPO Plan. This is our traditional PPO plan where we coordinate benefits in a secondary position after Medicare pays.• Generally, no copays for medical care after

coordination with Medicare and $10 to $40 copays for prescription drugs.

• Monthly premium of $218 per retiree, $435 retiree and spouse.

The Health & Welfare Fund also has two other options where members would receive their medical benefits elsewhere on their own and only receive supplemental benefits, like vision, dental, hearing aids, etc. from the Fund for a lower monthly premium. If you would like to learn more about these Plans, referred to as Plan M and Plan L, please call the Member Services Department in the Fund Office.

encourage your kids to be active from an early age• Join a local team - there’s teams for all ages and skill levels• Don’t like the great outdoors? Take up bowling or take a

This gives you immediate access to an Anthem Blue Cross network, board-certified doctor via webcam, chat, or voice at no cost to you without having to wait for an appointment or going to an urgent care center or hospital emergency room.

LiveHealth Online is convenient, easy to use, and secure. Simply log onto www.livehealthonline.com and follow the registration instructions. To register, you will need your OEID number. Once registered, you can then choose a doctor and begin your consultation.
