The purpose of this portfolio is to demonstrate the progress in meeting individual, course, and program goals from my university’s nursing program. This program is a six semester, evidence-based program that seeks to improve the health and wellness of diverse populations through the advancement of nursing science, promotion of excellent clinical scholarship and practice, and the

development of leadership in the healthcare environment. This portfolio contains examples of my academic accomplishments and assignments that I have collected from all six semesters.

Program Objectives

1. Integrate the knowledge, theories, and methods of a variety of disciplines to inform decision

making.

2. Participate in the process of inquiry, analysis, and information literacy to promote quality care

and patient safety.

3. Apply leadership concepts, abilities, and decision making in the provision of high quality

nursing care, healthcare team coordination, and the oversight and accountability for care de-

livery in a variety of settings.

4. Integrate evidence, clinical judgement, inter-professional perspectives, and patient prefer-

ences in planning, implementing, and evaluating outcomes of care.

5. Demonstrate skills in using patient care technologies, information systems, and communica-

tion devices that support safe nursing practice.

6. Demonstrate basic knowledge of healthcare policy, finance, and regulatory environments, in-

cluding local, state, and global healthcare trends.

7. Use inter- and intra- professional communication and collaborative skills to deliver evidence-

based patient centered care.

8. Assess health/illness beliefs, values, attitudes, and practices of individuals, families, groups,

communities, and populations in order to provide comprehensive care to diverse populations.

9. Demonstrate the professional standards of moral, ethical, and legal conduct.

I have completed this objective by utilizing evidence-based practice through academic article litera-ture to develop a plan of care for selected patients at the hospital setting. Using data from classroom

settings and scholarly articles, I make appropriate decisions for interventions to provide the most competent and genuine patient care. This process was taught since my first semester and proceeded through all my clinical rotations which included adult health, pediatrics, obstetrics, mental health,

and advanced adult health.

I completed this objective by participating in a group discussion and research on a particular topic that concerns patient safety. Throughout the program, we are continually taught to question and analyze cur-rent practice in order to provide consistent and up to date patient care. We are encouraged to analyze spe-cial situations such as sentinel events and case studies, and to create possible solutions for these particular cases. In doing so, I have learned the research process, the inquiry and analysis of problems, and explor-

ing possible solutions to provide quality patient care and safety.

I have completed this objective by demonstrating certain leadership skills in classroom and clinical settings. I have emphasized through written journals concerning the important skill of

delegation. I have also analyzed myself during moments of confrontation, and how I dealt with that particular confrontation in order to better my skills as a leader. This prepared me to

interact with a multidisciplinary healthcare team during my internship at the hospital, and provided the skills I needed to delegate tasks, as well as accept delegated tasks.

I have completed this objective through use of evidence-based practice and adjusting this information to effectively educate a targeted audience. This skill helped me during my internship, in which I interacted with multiple patients of differing educational back-

grounds and differing developmental stages. I learned to communicate complex informa-tion into comprehensive teachings for patients and families.

I have completed this objective by successfully passing all nursing skills exams required by CSU Stanislaus before being allowed to participate in clinical rotations. I also demonstrate competency in using technology systems as evidenced by a letter of recommendation by my current preceptor stating my skills with following hospital policy and procedure, which includes electronic medica-tion administrations, electronic charting, and use of a variety of technologies surrounding patient

care.

I completed this objective by analyzing health related issues within a community in the county of Merced. This entailed working with state officials at the public health office to gather relative data pertaining to issues of health in the community. This process resulted in the knowledge of regulatory laws that affect federal and state policy, as well as how programs for local communi-

ties are funded, whether from state or federal budgets.

During the nursing program at CSU Stanislaus, there have been multiple opportunities for collabo-ration with fellow classmates concerning patient care. These opportunities helped in improving my

skill concerning interaction with multidisciplinary team members during my clinical rotations to provide evidence-based care of patients and their families. These include bedside interactions con-cerning treatment options, explanations of certain processes and procedures, and advocating for

patients concerning wishes and their decisions to their care.

Having a nursing program in the central valley of California provides a diverse patient population with a multitude of beliefs and customs that are important when caring for these populations. There are also multiple healthcare providers within a small area including Doctors Medical Center, Memo-

rial Medical Center, Emanuel Medical Center, St. Joseph’s Hospital, Madera Children’s Hospital, Doctor’s Behavioral Health Center; all of which I have had clinical hours at some point during the

three years in this program.

Confidentiality is a staple lecture in our nursing program. I was taught to follow strict guidelines as written by the Health Insurance and Portability Act as well as have a firm grasp on established ethical theories. I have maintained this degree of

confidentiality when interacting with patients throughout my nursing school career and have been consistent at each facility that I have had clinical rotations.

I have participated in numerous classroom lectures, simulations, field trips, and clini-cal rotation during my nursing school career. Each semester has expanded my knowl-edge base and will continue to grow exponentially when working as a licensed RN. In order to be an effective nurse, one must continue to learn new information for current

practice. Healthcare is ever changing, and continual education outside of nursing school is important.

Through each semester I have learned the alternative methods of interventions to provide care for specified patients. These interventions are meant for different ages, diseases, and

some are invasive as well as non-invasive. I have expanded my knowledge by researching and implementing alternative therapies in order to manage illnesses that broaden across

the lifespan
