CSI-KINDUZ Lean Six Sigma International Awards The CSI-KINDUZ Lean Six Sigma International Awards are given to the most outstanding Organizational, Team and Individual achievements through the deployment of improvement programs. The Awards present an opportunity for Organizations, Teams and Individuals to win recognition for the outcomes delivered to Customers, Shareholders and other Key Stakeholders.

Who can apply? Organizations, Teams and Individuals around the world can apply for the awards.

Award Categories Nominations for the Awards are being accepted for the following three categories:

Awards for Sustained Excellence – Individual Category

Individuals can apply or be nominated for these areas:

Six Sigma Hall of Fame

Lean Hall of Fame

Note: Depending on the number of entries, Winner/runner-up may be chosen in each category.

The committee may take a special note of effort taken by an organization, in addition to the

above listed awards. The decision of the Awards Committee will be final.

Conditions for Entry

Entry for sending nominations is open to everyone around the world including

organizations, teams and individuals from the manufacturing, educational, defense,

educational and other industries.

Awards for this year will consider the work done after April 1st 2005 to November

31st 2010.

Judging Process

Nomination to each category will be judged by the Awards Committee, consisting of

members from the Industry and Academia.

The judging process shall be based on:

On-line nomination submission

Discussions over various media between the Awards Committee and

Nominated Members

Field Visits if required by a few members of the Awards Committee

Presentations by Short Listed Entries to the Awards Committee

Final Award list is generated based on the combined evaluation at different

stages as above

The Awards Committee reserves the right to seek additional information from the

nominated entries during the evaluation process

The final recommendation of awards will be made by the Awards Committee on the

basis of the information available to them in the nomination form, discussions, field

visits (if required) and presentations made by the applicants.

Evaluation Criteria for the Awards

The evaluation criterion is based on two indicators i.e.:

Results, and

Enablers

The attributes considered for Result indicator are mainly in terms of outcomes

The attributes considered for Enabler indicator are in terms of the processes in place so as to

achieve the desired results.

The quality of content submitted by the applicants for the nominations will be of most

important part of the evaluation process. The information should be as per format and

restricted to the prescribed page length defined in the respective “Nomination Submission

Template”. The information provided in the nomination form should contain the crucial

and important aspects only.

The quantity of content submitted will play NO ROLE in the evaluation process.

The key indicators and attributes being used for evaluation under each award category are

given in subsequent pages. A brief explanation of each of them is given in the respective

“Nomination Submission Template”.

Each nomination entry should accompany a “Statement of Intent” stating the reasons for

nominating the entry to CSI-KINDUZ Lean Six Sigma International Awards 2010. This

should not be of more than half page.

Nomination process opens on 30th April 2010 and closes on 31st December 2010.

Judging process will take place from 1st January 2011 to 15th February 2011

Award Winners will be felicitated during that the 2011 Lean Six Sigma International

Convention to be held in Hyderabad, India.

How to Apply

Visit the Lean Six Sigma International Convention website at

http://www.leansixsigmaconvention.org/ for nominating your entries

Entry nomination should be done through the following link:

http://leansixsigmaconvention.org/global-html/register-for-awards.html

One the nomination has been sent, download respective award template, enter the required

details and send an email to [email protected]

Entries for Organization and Team Category should be supported by an “Authorization

Certificate” as per prescribed format, duly signed by the respective head of the

Organization or Team. A scanned copy of the same should be e-mailed along with the

online nomination process

Entries will be accepted, strictly as per the respective category “Nomination Submission

Template” and general guidelines, and entries with deviations will not be accepted.

It should be ensured that the entries are submitted as MS-Word or Open Office Word

file of not more than 15 A4 pages, as per prescribed format given in the respective

category nomination template.

Anytime before the close date of nomination 31st December 2010, the entry information can

be modified by the authorized official. After the said date provision for editing will not be

available to the users. The Awards Committee if desires necessary may make changes in the

dates and inform on the website.

Please ensure complete contact details for all further communication related to awards

As part of the awards, the selected entries are published online to be released during the

Awards function; therefore it will be desirable that the correctness of language in the entries

is checked by the nominees before the close date.
