MINOR AUTHORIZATION AND CONSENT FORM

AudioMicro.com is recommended for adults only. This website generally does not provide

services or sell products to children under the age of eighteen (18). If you are under eighteen

(18), you may only use this website with the involvement of a parent or guardian.

Any use of AudioMicro.com by person(s) under 18 years of age (a "Minor" or "Minors")

will bind the parents or legal guardians of the Minor to the terms and conditions of

AudioMicro.com, including but not limited to our Terms of Service and, as applicable,

other policies and contracts.

To specifically authorize a Minor to use AudioMicro.com, please have a parent or guardian read

the following provisions, execute the remainder of this form and return it by fax to AudioMicro at

(320) 451-3838.

1. I hereby declare that I have legal custody of the Minor child named ________________ (the

"Child").

2. I hereby grant my full authorization and consent to AudioMicro for the Child to access

AudioMicro.com and use the AudioMicro Service. I further warrant and represent that the termsand conditions of AudioMicro.com are fair, just, equitable and freely entered into hereunder.

3. This authorization and consent is effective commencing on the ______day of

4. I understand that as parent or legal guardian of the Child, I am solely responsible for any

liability incurred by the Child during the Child's use of AudioMicro.com or the AudioMicro

Service. I agree to execute any such further documents as may be required by AudioMicro.com

in connection with the use and services of AudioMicro.com. By executing this form below I

acknowledge that I have read and that I understand and further that I agree to be bound by

AudioMicro's Privacy Policy, Terms of Service and as applicable, Content Provider Agreement

and Content License Agreement.
