3. ALS Blue Card

For the incoming second to fourth year students who wishto have their Blue Card replaced, kindly fill out the

Note that this is subject to a P50 fee, to be paid at the startof the school year.

As for the remaining students who will opt to renew theirBlue Card, the Blue Card Team will be setting up a booth

also at the start of the school year to issue the stickers forthe A.Y. 2015-2016.

DEADLINE FOR FILLING OUT THE REPLACEMENTFORM: July 4, 2015
