The regular meeting of the Arizona State Board of Nursing convened at 8:09am, Thursday, September 24, 2020 in

I. Call to Order
The meeting was called to order at 8:09am, Thursday, September 23, 2020.
The following Board members were physically present: Andersen, Berrigan, and McCormies.
The following Board members were virtually present: Boyer, Gutierrez, and Hill-Mekoba.
Hill-Mekoba presided.
Hill-Mekoba welcomed members of the audience, explained the procedure for addressing the Board, and stated the
mission of the Board.

The proceeding began at 8:22am with the following Board members present: Andersen, Berrigan, Boyer, Gutierrez,
Hill-Mekoba, and McCormies.
Vincent was telephonically present and addressed the Board.
Bushong confirmed the Board received the updates that were uploaded earlier in the week and stated the Respondent
has agreed to but has not signed the proposed Consent Agreement.
Board members discussed that the evaluation was completed and the appropriate action.
McCormies moved, Hill-Mekoba seconded, based upon the information presented at this meeting and contained in
the investigative report, accept the recommendation of Board staff, if signed issue licensure, and meeting all licensure
requirements. Agreement would need to be signed by the end of the Board meeting. (No vote was taken on this
motion.)
Vincent stated that he is no longer seeking Arizona licensure because he is moving back to Delaware.

Board members discussed options to proceed, including how discipline will affect him in other states. Campbell stated
that the application and privilege to practice needs to be addressed and gave examples of how to proceed.
Berrigan moved, McCormies seconded, and after discussion it was unanimously carried, to table the case.

The proceeding began at 8:36am with the following Board members present: Andersen, Berrigan, Boyer, Gutierrez,
Hill-Mekoba, and McCormies.
Regis was telephonically present and addressed the Board.
Barajas did not have any updates.
Board members discussed whether a showing of extraordinary circumstances or the possibility of irreparable harm
has been made supporting the need for a Summary Suspension.
Board members discussed whether the foregoing conduct poses an immediate threat to the public health, safety, or
welfare and requires emergency action pursuant to A.R.S. § 41-1092.11(B).
McCormies moved, Andersen seconded, and it was unanimously carried, based on the information presented at this
meeting and the findings of fact and statute/rule violations identified in the investigative report, make a finding that
public health, safety and welfare imperatively requires emergency action and issue the attached Order summarily
suspending the license of Paul Frederick Regis RN206636; UCNA1000001135 pending further hearing. The hearing
shall be promptly instituted and determined.

Wiedmann was present.
Bushong confirmed the Board received the update that was uploaded earlier in the week which included an interview
the Respondent had with Board staff.
Based upon the information presented at this meeting, Board members discussed whether a showing of extraordinary
circumstances or the possibility of irreparable harm has been made supporting the need for a Summary Suspension.
Board members discussed whether to summarily suspend the licensee or refer the matter through standard Board
procedures. Board members discussed whether the Respondent was currently working.
Hill-Mekoba moved, McCormies seconded, and it was unanimously carried, based upon the information presented at
this meeting and contained in the investigative report, offer a Consent Agreement for Stayed Revocation Suspension,
not to exceed 12 months, pending the completion of a comprehensive psychological evaluation by a Board approved
licensed psychologist with expertise in substance abuse, and the successful completion of any acute treatment
recommendations resulting from the evaluation and confirmation from either the evaluator or treatment providers that
Respondent is safe to return to practice, followed by a minimum 36-month Stayed Revocation Probation, and to
include the attached stipulations. If not signed within 30 days issue a Notice of Charges. Prior to a second motioner,
Hill-Mekoba amended the motion to add if the Consent Agreement is not signed by the end of the day, will review
tomorrow. Hill-Mekoba also added Wiedmann’s undeclared nursing assistance license to the motion.
McCormies stated concern with the motion.

Courtney was telephonically present and addressed the Board and requested a dismissal. The Respondent was
telephonically represented by legal counsel, Scott King, who addressed the Board.
Fotinos provided an update to the Board, that Board staff spoke with Respondent's current supervisor who had very
positive reviews of Respondent's clinical practice/judgement. Board staff was also provided additional historical
medical records for Patient SG indicating that her baseline heart rate was in the 60s.

Boyer moved, Hill-Mekoba seconded, and it was unanimously carried, based upon information presented at this
meeting and contained in the investigative report, to dismiss.

VI.R.2. Bonnie Jean Rouzaud

DaPena, who also addressed the Board.
Complainant, JO, was present and addressed the Board.

Hill-Mekoba moved, Gutierrez seconded, and it was unanimously carried, based upon information presented at this
meeting and contained in the investigative report, issue a Letter of Concern as proposed by Board staff and as
contained in the investigative report.
VI. RN/LPN/LNA/CNA Investigative Reports
VI.E.6. Julie Marie Johnston
AKA: Julie Marie Iwinski

Johnston was telephonically present and was telephonically represented by legal counsel Charles Hover, who
addressed the Board and requested this case be continued to the next meeting to allow further examination of video
in evidence.
Chambers confirmed the Board received the update that was uploaded earlier in the week.
McCormies moved, Berrigan seconded, and it was unanimously carried, based upon the findings of fact and
statute/rule violations identified in the investigative report, and information presented at this Board meeting, upon
meeting all licensure requirements and signing a Consent Agreement for a Stayed Revocation Suspension, not to
exceed 12 months, followed by a minimum 36 month Stayed Suspension Probation, and a $200 administrative
penalty, grant licensure. The license will be immediately suspended pending the completion of acute treatment
recommendations for advanced ethics training and substance abuse treatment and written authorizations to the
Board from all treatment providers that can attest to improvements in psychological and psychiatric functioning
to safely return to the practice of nursing. Upon successful completion of the acute treatment recommendations and
written notification from the facilitators, the minimum 36 month Stayed Suspension Probation will begin under
applicable terms contained in the attached stipulation worksheet. If the acute treatment recommendations are not
successfully completed within 12 months, or other violations of probation occur, the stay shall be lifted and the
license revoked, for a minimum of 3 years. If violations occur during the 36 month Stayed Suspension Probation, the
stay will be lifted and Respondent’s license will be suspended pending further review by the Board. If not signed
within 30 days, deny licensure and issue a Notice of Charges against Applicant’s privilege to practice as a RN in
Arizona.

the Board.

Board members discussed Applicant’s positive employer reviews and concerns with Applicant’s current use of
alcohol.
Boyer moved, Hill-Mekoba seconded, and after discussion it was unanimously carried, upon meeting all licensure
requirements, grant licensure. Applicant has submitted substantial evidence that the basis for the previous surrender
has been removed and that the reissuance of Applicant’s license will not be a threat to public health or safety.

and then return to the Board. If the Interim Order is not completed, based upon the information in the investigative
