Mr. Gentile moved the Board of Commissioners, County of Jefferson, State
of Ohio, approve and sign the following:
CONTRACT
THIS CONTRACT made this 12th day of April, 2018 by and between The Lash
Paving company, 67925 Bayberry Drive, Suite B, St. Clairsville, Ohio
43950, (a corporation and existing under the laws of the State of Ohio),
hereinafter referred to as the CONTRACTOR, and the Jefferson County Board
of Commissioners.
WITNESSETH, the CONTRACTOR and the COUNTY for the consideration stated
herein agree as follows:
ARTICLE 1 - STATEMENT OF WORK - The CONTRACTOR shall furnish all
supervision, technical personnel, labor, materials, machinery, tools,
equipment and services, including transportation services, and complete
all work required for the PURCHASE OF VARIOUS ASPHALT CONCRETE PRODUCTS
FOR 2018, for the Jefferson County Engineering Department, submitted with
the Bid or Proposal dated February 22, 2018.
ARTICLE 2 - CONTRACT PRICE - The COUNTY will pay the CONTRACTOR for
performance of the CONTRACT, in current funds, subject to any additions or
deductions agreed between the COUNTY and CONTRACTOR in an amount not to
exceed (BID SHEET).
ARTICLE 3 - CONTRACT - Now therefore in consideration of the payments and
covenants to be made and performed by the COUNTY as set forth in the
conditions of the CONTRACT attached hereto, the CONTRACTOR covenants and
agrees to do and complete, within the prescribed time, all of the work in
conformity with and subject to all of the terms of the CONTRACT of which
this CONTRACT is a part, and further agrees that in addition to this
CONTRACT, the CONTRACT will include the following documents bound
herewith:
Advertisement for BID or PROPOSAL, INSTRUCTIONS TO BIDDERS, COVENANTS AND
CONDITIONS OF CONTRACT, SPECIFICATIONS, BID BOND, PERFORMANCE BOND.
This CONTRACT together with any supplementary drawings, agreements or

CONTRACTS required to properly complete said work, which said other
documents are as fully a part of the CONTRACT as if hereto attached or
herein repeated, forms the CONTRACT between the parties hereto. In the
event that any provision in any component part of this CONTRACT conflicts
with any provision of any other component part, the provision of the
component part first enumerated in this Article 3 shall govern, except as
otherwise specifically stated.
IN WITNESS WHEREOF, the parties hereto have caused this CONTRACT to be
executed in three (3) original copies on the day and year first above
mentioned.

Mr. Gentile moved the Board of Commissioners, County of Jefferson, State
of Ohio, approve and sign the following:
CONTRACT
THIS CONTRACT made this 12th day of April, 2018 by and between Valley Tree
Services, LLC, 206 Deepwoods Lane, Weirton West Virginia 26062,(a
corporation and existing under the laws of the State of West Virginia),
hereinafter referred to as the CONTRACTOR, and the Jefferson County Board
of Commissioners.
WITNESSETH, the CONTRACTOR and the COUNTY for the consideration stated
herein agree as follows:
ARTICLE 1 - STATEMENT OF WORK - The CONTRACTOR shall furnish all
supervision, technical personnel, labor, materials, machinery, tools,
equipment and services, including transportation services, and complete
all work required for the YEAR 2018 TREE REMOVAL PROJECT, for the
Jefferson County Engineering Department, submitted with the Bid or
Proposal dated March 22, 2018.
ARTICLE 2 - CONTRACT PRICE - The COUNTY will pay the CONTRACTOR for
performance of the CONTRACT, in current funds, subject to any additions or
deductions agreed between the COUNTY and CONTRACTOR in an amount not to
exceed One Hundred Eleven Thousand Three Hundred Dollars and No Cents
($111,300.00).
ARTICLE 3 - CONTRACT - Now therefore in consideration of the payments and
covenants to be made and performed by the COUNTY as set forth in the
conditions of the CONTRACT attached hereto, the CONTRACTOR covenants and

The Jefferson County Drug Task Force is pleased to inform you that we were
awarded the Edward Byrne Memorial Justice Assistance Grant (JAG) 2017 in
the amount of $20,324.96. This funding will be used for task force
operations during the grant period of January-December 2018.
The funding is contingent on funding approval by the federal government,
but unfortunately, there is no word yet as to if and when that will occur.
In the meantime, all grant recipients were advised to prepare all pre-
award conditions for approval so once the funds are available, there won’t
be any further delays.
We are grateful that the commissioners agreed to be the subgrantee for
this grant, and as such, we respectfully request that the board chairman
sign the pre-award conditions-the Equal Employment Opportunity Policy
form, and the form acknowledging that the task force will be compliant
with any requirements necessary to receive this federal funding - so that

Agency (Ohio EPA) Division of Surface Water (DSW) has received an
application for and has begun to consider whether to issue or deny a Clean
Water Act Section 401 water quality certification for a project to
construct a 11-mile 10-inch ethane supply pipeline from existing MarkWest
facility in Cadiz to a junction point located two miles southeast of Scio,
and a 12-inch diameter ethan supply pipeline from the Utica East Ohio
plant in Scio to a junction site in Pennsylvania, 32.6 miles of which will
be located in Ohio. The application was submitted by Shell Pipeline
Company LP. The linear project is located in Harrison, Carroll, and
Jefferson Counties. The Huntington District Corps of Engineers Public
Notice number for this project is LRH-2015-00931-TUS Clearkfork. The
Pittsburgh District Corps of Engineers Public Notice number for this
project is LRP-2015-1191. The Ohio EPA ID number for this project is
185627A.
Discharges from the activity, if approved, would result in degradation to,
or lowering of, the water quality within the Upper Ohio watershed (HUC
05030101), the Upper Ohio Wheeling watershed (HUC 05030106), and the
Tuscarawas watershed (HUC 05040001). Ohio EPA will review the
application, and decide whether to grant or deny the certification, in
accordance with OAC Chapters 3745-1 and 3745-32. In accordance with OAC
rule 3745-1-05, an antidegradation review of the application will be
conducted before deciding whether to allow a lowering of water quality.
No exclusions or waivers, as outlined by OAC rule 3745-1-05, apply or may
be granted.
Starting April 12, 2018, copies of the application and technical support
information may be inspected at Ohio EPA-DSW, Lazarus Government Center,

The Board of Commissioners, County of Jefferson, State of Ohio, received
the following:
Please accept the following quote for additional survey work at the towers
building parking lot.
Limits of Work. The limits of work are defines by the red outline on the
attached drawing.
Scope of Work: The scope of work includes the following exclusively;
Elevations to be determined by OPUS Solution of local control point, or by
utilization of the ODOT VRS. Work to be on same datum and coordinate
system as the work performed on the Towers Parking Lot in 2016.
1. Property lines: No property line work to be performed in this phase.
2. Locate and map all manmade visible improvements, and include trees
over 6" diameter. Survey work for topo and improvements to be performed
by use of High Definition Scanning, and conventional survvy techniques.
Images and point clouds to be collected with Leica P20 HDS scanner and
process in Leica Cyclone Software and Leica Cloudworx for AutoCAD.
3. Locate and map existing utilities from visible evidence, and markings
provided by owners of utilities that respond to an O.U.P.S. request.
Note: It is hereby understood, and agreed, that the amount of utilities
located, and the degree of completeness, and correctness of the survey of
the existing utilities is dependant upon the response by the Owners of
each utility to an O.U.P.S. request, or phone call. Vince Dowdle &
Associates LLC, it’s owners, agents, and employees, take no responsibility
for, the mapping, or surveying, of existing utilities which are not
visibly and noticeably marked on the surface of the ground during the
field work for the survey here-in outlined. Vince Dowdle & Associates
LLC, its owners, agents, and employees, take no responsibility for, the
mapping, or surveying, of existing utilities that may be shown on plans in
the owner’s, or client’s possession, but which were not delivered to 157

for the survey here-in outlined. Vince Dowdle & Associates LLC, it’s
owners, agents, and employees, make no warrantee as to the completeness or
correctness of said existing utility markings made by their respective

utilities, and accept no responsibility for, or liability from, any and
all damages, either directly or indirectly, from or due to the
incompleteness, and/or incorrectness of said plans and markings provided

by others. Note: Utility companies have two weeks to response to an
O.U.P.S. call for mapping purposes, and this response, which is totally
beyond the control of Vince Dowdle & Associates LLC, could affect the time
line of the completion of the survey, and the amount of utilities shown on
the drawings.
4. Collect existing surface elevations and generate 1 ft contours.
Deliverables.
A. Three hard copies of survey, and provide an electronic file for use in
AutoCAD format as a compliment of the hard copy. Survey will be developed
in Auto CAD Civil 3D and exported to AutoCAD 2013 version.
B. A ReCap file of the high definition scan that cen be opened in AutoCAD
recap, or attached to the dwg in AutoCAD Civil 3d.
The estimated cost for work outlined above....................$3,175.00
This quote was submitted without knowledge of any special requirements,
conditions, or wage rate requirements and we reserve the right to re-
submit this quote, if we feel it is necessary, after any such
requirements, conditions, or wage rate requirements are made known to us.
-Survey work outside of the limits of work, or beyond the scope of work is
not included, and if requested, will be performed at an additional cost.
-The work is to be performed during a regular 8 hour workday, on Monday
thru Friday.
-This estimate is good for 30 days, after which the price is subject to
change.
-Professional Liability limited to cost of survey work here-in quoted.
-All work will be done in an expeditious manner, however, weather will be
an acceptable reason for delay.
-All work to be performed with the use of temporary signs, any other
traffic control deemed necessary shall be the responsibility of others and
in no way is part of this estimate or scope of work.
-This is not intended to be a commitment to your firm, or to the project,
merely a quote.
-If at any time after this authorization is signed and returned, the work
is cancelled or delayed by you, the client, it is here-by understood and
agreed, that Vince Dowdle & Associates is entitled to, and you the client
shall pay, for all work performed up until that cancellation is received
in written form.
-Permissions to access of neighboring properties is client’s
responsibility.
Please indicate your approval and agreement with the terms and conditions
of this proposal by completing the “Statement of Authorization” below and
returning it to our office.
By Authorizing this work you are also giving us permission to, enter upon
by foot and/or vehicle, and to cut brush and/or trees to enable work, said
brush and/or trees to be left on site as it falls, and to do all manner of

access to property is client’s responsibility. You are agreeing to pay
for this surveyor’s professional opinion of where the property lines (deed
lines), structure locations, and other features are located, relative to
each other, and on the surface of the earth, if the same are included in
the scope of work. These Provisions and the accompanying Proposal
constitute the full and complete Agreement between the parties and may be
changed, amended, added to, superseded, or waived only if both parties
specifically agree in writing to such amendment of the Agreement.

DS Architecture Architect, Mr. Paul Romanic addressed the Board of
Commissioners this date of Thursday, April 12, 2018 regarding the
Electrical Boards Project at the Jefferson County Justice Facility. The
Commissioners stated that they would like Mr. Romanic to contact Sheriff
Fred J. Abdalla, and ask his recommendation concerning the project. Mr.
Bill Ward, Juvenile Detention Director, was also present and spoke to the
Commissioners with his concerns regarding the Juvenile portion of the
project.
