Cancellations & “No Show” PolicyIf you are unable to keep your scheduled appointment, please call the office and we will be more than happy to reschedule for you. Failure to notify us at least 48 hours prior to your appointment may result in a cancellation fee.

Prescription RefillsRefill requests are handled during normal office hours when our staff has full access to medical records. Refills cannot be called in on holidays, weekends or more than twelve months after your last exam. Please have your pharmacy contact our office directly.

Test ResultsYou will be notified when we receive your pathology or other test results, usually within two weeks from the date of your procedure. If you do not hear from us within three weeks of your procedure, please call and notify our staff.

Fees, Payment & InsuranceWe participate in many insurance plans. Please be certain to provide our office with your insurance card or other billing information, completed and signed insurance forms, and all required referral and authorization forms prior to or at the time of each visit. If you do not have the necessary referral information at the time of your visit, you may be asked to reschedule your appointment.

For those patients not carrying medical insurance, we require full payment at the time services are rendered, unless prior arrangements have been made. All elective cosmetic procedures must be paid for in advance.

Mohs SurgeryDr. Cowan is a board-certified dermatologist with advanced training in Mohs Micrographic Surgery and reconstructive surgery. Patient Visit InformationTo ensure your visit goes smoothly, please note:

• Bring your insurance card, picture ID and referral from your Primary Care Provider if it is required by your insurance plan.

• Co-pay is due at the time of registration. We accept MasterCard, Visa, checks and cash.

• If you don’t have insurance or are otherwise planning to pay on your own, you must arrange payment with our billing department. Call 1-877-661-3376 for assistance.

• Children under the age of 18 must be accompanied by a parent or legal guardian. A temporary guardian is permitted with written authorization from a parent or legal guardian.
