What you need when opening new accounts:
• Application: Ordering from this document will fulfill this requirement.
• New Account Opening Table (Disclosure): If FIS produces your applications, they already contain the New Account Opening Table and will fulfill this requirement unless your institution has more than one offer on the application. If you have 2 or more offers on the application, you will need to do a new account opening disclosure. Simply contact FIS and request a copy of the New Account Opening Disclosure order form.
• New Account Agreement (Credit Card Agreement): This required document discloses and further defines all of the terms of your institutions credit cards to the new cardholder. Be advised in addition to all the definitions in this document you must also include an explanation of the balance computation method as well the inclusion of billing error rights notice (the long form). FIS does not support nor help produce this, please work with your compliance or legal advisor to create this necessary document. Once you have it completed, FIS can provide you a quote on the print production of the piece. Just request a quote from your Client Services Representative.
Ordering your Applications and Marketing Materials is Easy…
Here’s What You Need to Do.
Step 1 Familiarize yourself with the program.
All benefits, descriptions, requirements, and cost for Applications and Marketing Materials are outlined in this “Ordering Guide.”
Step 2 Read and complete Applications and Marketing Materials Order Form.
Review and select the campaign theme that best fits your financial institution’s needs. Complete Order Form and make sure all required information and signatures are provided.
Step 3 Read and complete The Disclosure Form.
Provide disclosure language that describes your institution’s terms. Complete Disclosure Form(s) and make sure all required information and signatures are provided.
Step 4 Read and complete The Enhancement Form.
Provide information on all enhancement(s)/feature(s) that are offered with your card application. Complete Enhancement Form and make sure all required information and signatures are provided.
Step 5 Provide the Necessary Art Work.
Provide Logo and Return Mailing Address artwork as specified in the Art Specification Form.
Step 6 ONCE RECEIVED, REVIEW PROOF DOCUMENT THOROUGHLY. FIS WILL MAKE EVERY
EFFORT TO MAKE SURE FEDERAL REGULATIONS ARE FOLLOWED BUT THE ULTIMATE RESPONSIBILITY FOR ANY AND ALL INFORMATION CONTAINED IN THE APPLICATION IS THE SOLE RESPONSIBILITY OF YOU, THE FINANCIAL INSTITUTION.

PROGRAM BENEFITS & DESCRIPTIONS
Applications are more than just a form used to get information about a prospective cardholder. If properly designed, applications are a very effective brochure that highlights the special features and benefits of your credit card.
FIS produces applications with professionally designed marketing themes that match up with other marketing tools such as lobby posters, take-one stands w/inserts, table tent displays, buttons and statement inserts. No matter what your marketing strategy is or the size of your budget, we can help.
Production time defines the estimated time it will take for your financial institution to receive your applications and/or marketing materials.
• Order Confirmation: A confirmation of receipt of your order will be faxed or emailed to you within 2 business days.
• Proofing and Approval: FIS will provide you with a proof within (5)-business days of completing your order (assuming all information and requirements are provided on time). Your order includes one round of proofs (1 round = 2 proofs), one proof for editing, and a second proof for your final approval. Additional proofs will incur an additional fee for each proof thereafter.1 FIS will automatically cancel your order if your proof is not received back within ninety days and your financial institution will be invoiced via the Integrated Billing Statement for work incurred to date.
• Order Shipment: Your order will be shipped via UPS Ground Service2 within 30 business days (6 weeks) of receipt. This timeframe is based on the assumption that all proofing and approval is completed within seven (7) business days. Therefore, each day of delay experienced in the proofing and approval process will delay the ship date one day. Once your proof has been “approved to print,” final product will be shipped within two (2) weeks thereafter (This does not apply to custom requests).
The following section addresses basic product information and requirements that FIS will use when producing your Applications and Marketing Materials.
• Color Selection: There is a universally accepted system of identifying specific colors known as Pantone Matching System (PMS)3. This system associates a number with each of the 700 plus color shades in the PMS system. Please provide the color and PMS number to be used when processing your order. If you do not know your PMS color(s), please contact your marketing department/designer. Otherwise, send us a sample of the color you want us to use and we will match the color as closely as possible.
All standard pricing as dictated by “Exhibit A” includes one PMS color per application order. This color will be used on the logo,headings and text. The application side of the document will be printed in black ink. The use of extra PMS colors and/or any other ink (i.e. Metallic, Fluorescent, Pastel, etc.) on side A or B of the application will result in additional charges. Please refer to “Exhibit A” for more information. If you plan to provide more than one PMS color, specific instructions must be provided to FIS in writing directing where the various PMS colors are to appear on the applications.
• Paper Selection:
Applications: All theme applications are printed on 80 lb coated paper3, and all standard applications are printed on 60 lb commodity (uncoated) paper. Refer to “Exhibit B” to view layout and dimensions information.
Marketing Materials: All large posters and table tents are printed on 100 lb coated paper.3
Statement Inserts: All Statement Inserts are printed on 80 lb coated paper.3

IMPORTANT INFORMATION It is not FIS’s intent to provide regulatory advice relevant to applications or disclosure requirements. Your institution is responsible for complying with all state and federal laws, rules, and regulations that pertain to credit card applications and disclosures. We highly recommend that your legal counsel and/or compliance
officer review your completed application order/disclosure(s) prior to submitting to FIS.
We do not offer a website ready product. Our applications are printed copies only and not for duplication. The Financial Institution has purchased the applications, not the rights to the proof or creative image.
FIS owns the copyright to the applications and we currently do not sell this copyright to Financial Institutions, only the printed applications.

Travel Accident InsuranceYou, your spouse and dependent children up to age 19 (age 25 if a full-time student at any institute of higher learning) are automatically covered with common carrier travel accident insurance every time you travel by air, bus, train, ship, taxi or any other common carrier anywhere in the world when you charge your entire fare to our card. This coverage is provided to you at NO EXTRA COST.
Credit InsuranceOur Credit Insurance program can protect your card, your family, and your credit rating when you may need it most due to unexpected events. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Note for Issuer: If you utilize a vendor other than FIS for credit insurance, please provide insurance disclosure language to your relations rep.
Price GuaranteeBy simply using your (Institution’s Name) Credit Card for the purchase of a new item, if a price reduction is then advertised on the identical product within 60 days from the date of purchase, you are eligible to receive reimbursement. The program will reimburse the difference between the purchase price and the new reduced price.
Extended WarrantyOn purchases made in full using our credit card, the extended warranty program doubles the U.S. manufacturer’s warranty period and extends the coverage up to one additional year for warranties of one to five years (some exclusions apply).
90-Day Product ProtectionCovers new consumer products when purchased in full with our credit card against direct physical loss or damage for 90-days from date of purchase (some exclusions apply). Coverage is in excess of other collectables insurance.

B. Logo should be “camera-ready” art. Emailed Logos are also acceptable provided the file is “TIF” or “EPS” 600 dpi. xerox copies and faxes cannot be used for artwork and they are not considered to be camera-ready. Poor quality logo art will result in a poor quality logo printed on the applications and marketing materials.

Frequently Asked Questions:
1. What is Pantone Matching System (PMS)? The Pantone Matching System is a popular method used by the printing industry to print spot colors. It allows us to match the specific color of your artwork for the printing process. You should be able to obtain your PMS color from your Marketing/Graphic Design Department. If you do not know your PMS color, please attach a color sample and we will match it as close as possible to the PMS Chart. Your application order includes one (1) PMS color. Refer to Exhibit “A” for additional PMS color pricing. Please note PMS is a trademark of Pantone, Inc.
2. What do you mean when you refer to Paper Weight? Paper Weight indicates the thickness of the paper that is being used to print the applications, statement inserts, etc.
3. Do I need more than one disclosure form if I am promoting more than one program in the same set of applications? Yes, if you are ordering one set of applications and promoting more than one credit card program, and those programs have separate disclosure information (i.e. APR, Fees, Enhancements, etc), you must complete a separate disclosure form (simply make blank copies of appropriate pages within this ordering document) for each program. Please see “Exhibit D” for extra forms.
4. Can I order more marketing materials even if I place a marketing kit or a tier package order? Yes, you can elect to purchase other a la carte items such as posters and take-one stands. However, you must only choose one theme per order.
5. How can I request a color copy of the themes? You can request an electronic color copy of the themes by emailing us at [email protected].
6. Can I place a custom order if none of the themes fit with our marketing needs? Yes, you can customize your applications and statement inserts. Please make sure to complete the Application and Marketing Material Order Form and indicate your request on the “Order Type:” portion of the form. Also include written specifications for the customization of your order. After you place your order, a written quote will be provided to you for approval within five (5) business days. Note: Federal Regulations restrict us from promoting more than three (3) variable rates card programs per applications when rates are not similar. In this case, you may want to request a custom application.
7. What is a “Camera-ready” logo? The term “Camera-ready” is used to describe an extremely crisp and clear copy. Usually you can obtain your logo and other camera-ready artwork from your marketing department/design agent that created your logo.
8. Do I need to complete the entire A&MM Ordering Package every time I place an order? No, if you are placing a re-order, and there are no changes to your artwork, disclosure, and/or your enhancements, you only need to submit a completed order form. However, if there are any changes to your original order, you need to submit all the forms that apply to those changes.
9. What do I need to do if I find an error on the applications once they have been printed and shipped? If the error was caused by FIS, we will reprint your order at no cost to you. If the error was an oversight by your institution and was approved for printing, we will reprint them for you at a discounted rate. Please contact your Client Services Representative for more information.
