PERSONAL MEDICAL HISTORY
ENGLISH COURSE
Objective of the English Course
To equip students with the language skills to participate better in the IFFAsia program
To enable students to gain confidence to communicate in English, to appreciate the
various learning experiences and make connections for deeper understanding.
To practice analytical skills, inference abilities and improve organization of ideas and
thoughts in English
To grow in leadership ability and team work through the various class activities
The English Course is offered in collaboration with the Assumption Language Centre
(ALC), Quezon City, Philippines. IFFAsia will help the students to enrol with ALC for the
intensive English Course
In ALC the English course is well organized and adapted to the students’ different
language levels; the methodology used is creative and enables systematic learning
Students are exposed to diverse cultures and they have good opportunity to practice using
English.
We choose to collaborate with ALC to tap on their core expertise in teaching language
while we offer the learning experiences to develop personhood and values, and equip
students with professional experience and skills needed to create positive change and
influence in societies.
We believe in partnership and collaboration to build synergy and communion in our
efforts.

How to apply for the English Course
1. Fill-up the attached ALC Application Form for the English Course
2. ALC Application Form must be submitted with the IFF Asia Application Form by
31st August 2013.
3. Students will be given a diagnostic test to determine his/her aptitude level in English before
the commencement of the class.
APPENDIX A
APPLICATION PROCEDURE
Candidates are advised to submit the application as early as possible. The application form can
also be downloaded from www.fondacio-asia.org
Accomplish the application form, include your letter of motivation and respond to the 2 essay
questions. You may email the soft copies of these, along with scanned copies of your other
official documents to facilitate application, to [email protected]. However, make sure to
prepare the originals and certified true copies of these documents for submission once your
application has been accepted.
Ensure you have the following documents scanned to email with your application.
1. Letter of Motivation
2. Essay - to answer two (2) questions
3. Academic Documents
a. High School - Certified true copy of High School certificate and its English translation
and the transcript of records and its English translation
b. College/University - Certified true copy of the bachelor’s degree certificate and its
English translation and the transcript of records and its English translation
Applicant for:
- Certificate in Pastoral Leadership must submit item a.

4. Birth Certificate - For foreigners, a certified true copy of birth certificate and its English
translation; for Filipino applicants, this should be the NSO original copy;
5. ID page of passport. - Passport pages showing the following details - Picture, Name,
Passport number, and Passport validity
6. Recommendation (using the enclosed Recommendation Form) from an immediate
supervisor of the association or community where the applicant is involved in/ Parish Priest
or local Bishop, to be endorsed and sent directly to the Institute.

The Institute will then assess the applicant’s educational background and other experiences.
After which, the applicant will be interviewed by a screening committee and subsequently sit for
an entrance exam conducted by Xavier University.
The Institute will inform the applicant of its decision within 1 month of receiving the application.
Successful candidates will be given a provisional acceptance on condition that they can attain the
required English proficiency, meet the admission requirements and succesfully complete the
entrance exam conducted by Xavier University.
In addition to the provisional acceptance letter, a preparation kit will be given to advise students
on the preparations required before the program.
(Note: Applications with incomplete documents will not be processed)
APPENDIX B
GUIDE ON LETTER OF MOTIVATION
Please attach a letter, which contains the following information:
Who are you? A brief description of your personality and background.
What is your reason for applying? What is your motivation for participating in this
formation?
What are your plans after you finish the formation and studies?
How will this formation respond to your needs? What are some of the aspects in this
formation which correspond well to what you are looking for personally and for your
local community?
APPENDIX C
GUIDE ON WRITTEN ESSAY
Please read the two (2) sets of questions below. Type your answers (1/2 to 1 page per question)
as best as you can. Please write these on your own without asking help from others.
1. Describe the situation of a group of people in your local community that affects you or for
whom you are concerned. What would you like to do for them?
2. From your own experience, how do persons from other religions see the Catholic Church in
your country? Should you try to evangelise them - why? For you, what is a Christian
community?
APPENDIX D
FINANCIAL ASSISTANCE
The Institute of Formation, Fondacio Asia (IFFAsia) subsidizes, to a large extent, the cost of
formation to enable young adults from diverse backgrounds and countries to participate in this
formation program.
It is the policy of IFFAsia to engage the participation of the student and the sending organization
to raise the needed funds for the subsidized formation cost, visa fees, pocket money and cost of
air fare to and from the Philippines.
Thus, before requesting the Institute for financial assistance, the applicant and the sending
organization are encouraged to apply the following means:
a. Applicant need to first make one’s own maximum personal contribution for his/her formation
program
b. Seek the support of family and friends for sponsorship
c. Obtain the support of the sending organization to finance his/her formation program
d. To also approach the local community, through BECs or other network together with the
sending organization for financial support and prayers too.
Before coming to IFFAsia, applicants are required to do some sponsorship. The effort of their
sponsorship is of great value as:
a. It helps them to have a better hold of their formation year with greater focus and clear
motivations.
b. The sponsorship received can supplement their personal contributions and share the
formation cost with a network of family and friends
c. It is part of their formation and mission training where they learn to trust in God’s providence,
and how to fundraise for mission projects at the Institute and later in ministry.
The sending organization is also encouraged and required to support their students in this effort.
How to start on your sponsorship
a. Prepare a personal presentation to share your background briefly about your family, studies,
work and your faith life. Share your motivation on why you choose to enter this formation
program. Include your target amount to raise for the formation and indicate your personal
contribution and what you need.
b. Go and meet family, friends, and parishioners to invite them to be your support, not only for
sponsorship but also for prayer. Often, the time when you establish new relationships with
your sponsors and prayer partners, it is also the time of your spiritual conversion and
salvation.
c. In receiving any sponsorship, it is strongly encouraged that all sponsorship are channelled
through the sending organization or sent directly to the Institute to ensure accountability and
integrity.
How to apply for financial assistance
IFFAsia can offer scholarship or study loan to cover the partial cost of formation program and
accommodation only. The cost of airfare, visa and pocket money will NOT BE covered.
Preference will be given to those who have been able to secure maximum amount from other
sources.
Forms for scholarship grant or study loan will be emailed upon request. Application for financial
assistance must be submitted together with the program application. After deliberation by the
Panel, scholarship or study loan may be granted.
APPENDIX E
ADMISSION REQUIREMENTS
Master in Development Leadership
Post Graduate Diploma in Development Leadership
• Bachelor Degree
• Aged 20 to mid-30s (candidates above 25 years old are preferred)
• At least two years of work experience and ministry/service involvement
Certificate in Pastoral Leadership
• High School graduate
• Aged 20 to mid-30s
• At least two years of work experience and ministry/service involvement
* Candidates above the age limit can apply and will be considered on a case to case basis
Admission Criteria
• Committed to fulfil the required two (2)-year internship, part-time or full-time, with the
sending organisation, local Church or community after their formation
• Has a * sending organization, e.g. an association, NGO, institute, local community, parish,
diocese, Caritas, etc.
• Proficient in oral and written English. There are English classes from mid-September to
mid-December the preceding year, every year for those who need it.
• Complete the entrance exam conducted by Xavier University successfully.
• Willing to participate fully in all activities and projects proposed during the formation year.
• Has the necessary physical and mental health to follow the demands of the program.
Note: The Institute reserves the right to refuse any student who does not meet the above criteria, even
during the course of the year.
* Role of sending organization This program requires the active participation of three parties - the student, the sending
organization and IFFAsia. A tri-partite agreement is done with the parties concerned to
promote a mutual understanding in their roles and responsibilities.
The following are the key responsibilities of the sending organization:
Academic criteria - Prepare the candidates, ensure sufficient maturity and capacity in
English language
2 Year Internship requirements - Provide the students support for project development
and implementation in one’s local context to fulfil the academic requirements.
Pastoral care - Collaborate with IFFAsia to enable students to sustain in the ministry
Financial contribution – Commit to assist students to get a network of sponsors in the
local community
(Exceptions to the above agreement may be considered on a case to case basis)
APPENDIX F
DOCUMENTS FOR UNIVERSITY ADMISSION
(to be submitted before Program commencement)
A. One (1) photocopy of passport pages where name, photo, birth date, birthplace, valid visa and
latest entry/arrival dates appear
B. One (1) authenticated* copy of birth certificate and its English translation
C. One(1) authenticated* copy of marriage certificate (for married women)
D. Academic Credentials
1. If freshman undergraduate, he must have completed at least Grade 11. He must also p resent
the following requirements: a) authenticated* high school report card and its English translation,
b) One (1) photocopy of authenticated* certificate of graduation or certified true copy of the
high school diploma and its English translation.
2. If transferee, he must also present the authenticated* copies of the official Transcript of
Records (TOR) with grading system and its English translation bearing the original signature
of the registrar and seal of the school.
3. For MA student, he must also present the following requirements:
a) authenticated* official Transcript of Records (TOR) for the bachelor’s degree with the
official grading system and its English translation, bearing the original signature of the
registrar and seal of the school
b) photocopy of authenticated* certificate of graduation in college or certified true copy of
the college diploma and its English translation.
*IMPORTANT: The authentication (with Red Ribbon) must be done by the Philippine
Foreign Service Post (PFSP) or embassy located in the student-applicant’s country of origin
or legal residence. The English translation needs to be authenticated as well.
APPENDIX G
HOUSING/RESIDENTIAL SERVICES
Housing facilities are provided for students for the duration of their formation in IFF Asia.
Male and female students will be housed in separate residential lodgings. Details on the
student activities and conduct, housing facilities and requirements are included in the student
handbook.
