Faculty of ScienceGuidelines for Submitt ing an Application for an Appeal against an Academic Decision

This application form provides students of the University of Sydney with a mechanism to appeal against academicdecisions which have been considered at a School level. Students are able to appeal on the grounds that dueacademic process has not been observed by a School in relation to issues of assessment, coursework, SpecialConsideration, marks and final grades. Students must demonstrate that the School or Faculty has not acted inaccordance with theAssessment Policy 2011 and associatedAssessment Procedures 2011, available 24 hoursfrom the University website:

The information that you provide in this application, including any associated documentation, is required to processyour appeal. The information that you supply will be retained by the Faculty in a confidential file that will only beavailable to relevant staff. All records will be destroyed lawfully and securely within an appropriate timeframe.

How to submit an appeal:

1. Your appeal must be lodged within 15 working daysof the academic decision which you are appealing.

2. Fill in the application form and attach any relevant supporting documentation you wish to be considered, e.g.personal letter, medical documentation, etc.
