STEP 3. ANNUAL SALES / PRODUCTION (All figures are subject to verification by the Indiana Department of Revenue.) 

Manufacturers shall report the following:  1. For a small brewery licensed under IC 7.1‐3‐2‐2(b), report the number of barrels sold in Indiana for the calendar year immediately preceding the date of this renewal 

application.  Reported number should include:  

Beer sold in a tasting room or retail restaurant permit premises;  

Beer self‐distributed under IC 7.1‐3‐2‐7(5)(A); and 

Beer sold and distributed through a beer wholesaler in Indiana.  2. For a farm winery licensed under IC 7.1‐3‐12‐3, report the number of gallons of wine sold in Indiana for the calendar year immediately preceding the date of this 

renewal application.  Reported number should include:  

Wine sold in a tasting room or retail restaurant permit premises;  

Wine sold and distributed through a wine or mircorwine wholesaler in Indiana; and  

Wine sold through direct shipment under the authority of a direct wine seller’s permit to an address in Indiana.   3. For an artisan distiller licensed under IC 7.1‐3‐27‐2, report the number of gallons sold in Indiana for the calendar year immediately preceding the date of this renewal 

application.  Reported number does not include any amount distributed through a liquor wholesaler in Indiana.  Reported number should include liquor sold in a tasting room or retail restaurant permit premises.

STEP 4. ANNUAL DISTRIBUTION (All figures are subject to verification by the Indiana Department of Revenue.) 

Small breweries/microwine wholesaler shall report the following:  1. For a small brewery licensed under IC 7.1‐3‐2‐2(b), report the number of barrels sold and delivered by the brewery to retailer and dealer permits under IC 7.1‐3‐2‐

7(5)(A) for the calendar year immediately preceding the date of this renewal application. If no beer was sold and delivered by the brewery directly to retailer and dealer permits under IC 7.1‐3‐2‐7(5)(A), permittee should put 0. 

2. For a microwine wholesaler described in IC 7.1‐4‐4.1‐13(c), report the number of gallons of wine and brandy sold to retailer and dealer permits in Indiana for the calendar year immediately preceding the date of this renewal application.

If there have not been any changes to the permit premises floor plan since the initial application or the last renewal, skip this step.  

INSTRUCTIONS: If there have been any changes to the floor plan since the initial application or the last renewal that have not been approved by the ATC, applicant must submit a floor plan drawing on letter size paper (8 ½” x 11”) and attach the drawing to this renewal application.  The drawing must show dimensions and identification of any existing family room(s), seating arrangement(s), ballroom(s), bar(s), dance floor area(s), kitchen area(s), restrooms, storage areas, entrances/exits, patios, beer gardens, service windows, and alcoholic beverage display areas for all types of permits.  Sign and date the drawing.   

NOTE: All floor plan changes must be approved by the Commission.  If you amended your floor plan without prior approval, submit the floor plan drawings with this renewal application and contact your local excise district office immediately.  Contact information for local excise district offices can be found at https://www.in.gov/atc/isep/2379.htm.   

STEP 7. CARRYOUT FROM RETAIL PERMIT PREMISES Dealer permits may skip this step.   

For retail permittees, complete State Form 56515, Renewal of Retail Carry‐Out and Six (6) Month Report, and attach to this renewal application.   

STEP 8. AFFIDAVIT OF APPLICANT
