Application Evaluation Report

• Your application evaluation report is an evaluation of transfer credit from outside institutions into your degree program at Columbia Southern University. This report will designate any transfer credit and what classes you have left to complete your degree. You will also receive a Degree Action Plan (DAP) along with your application evaluation report.

• Here are a few points to remember when reviewing your evaluation:

1. You must enroll in SLS 1000-Student Learning Success as your first course

2. Each section will tell you how many hours that you transferred in & how many hours remain to finish that section (see highlighted areas). You will then need to choose your classes from the corresponding section according to how many hours you still need.

3. If you see a class in a section where “Required” is listed next to it then that is a required class and must be completed for that area of your evaluation

4. Please read the ‘advising note’ located at the beginning of your application evaluation report. This will review the upper level class requirement for your degree.

• During Unit III you will contact your Academic Advisor and plan your Degree Action Plan. This degree action plan is a recommendation for your first few enrollments of classes that you are required to take. These classes are taken directly from your evaluation report.

• The purpose of the degree action plan is to help you get enrolled into your first classes while assisting you in getting oriented to taking online courses at CSU.

• There are two types of Degree Action Plans (DAPs): Per Course DAPs and Term DAPs – Per Course DAPs are for students in the open enrollment system

(10 week course).

– Term DAPs are for students in the term system (8 week course).

• FYI – Academic Advisor information will be listed at the bottom of your degree action plan

• Your application evaluation report will be personalized based upon any transfer credit you may bring to CSU. The following application evaluation report is an example of the format.

- The next few slides contain an example of an Application Evaluation Report

• Your application evaluation report is subject to change depending upon which degree program you request to be evaluated for; therefore, your requirements will be different based upon your transfer credit.

In this example the Application Evaluation Report provides the number of hours you transferred in, the number of hours required, and the number of hours needed to fulfill the requirement (e.g. Humanities/Fine Arts/Communications).

Your Application Evaluation Report will provide a detailed listing of the various transfer hours (if applicable) that you bring in to fulfill each requirement. For example, if you brought in 3 hours of history transfer credit, you would still need 3 to complete the history requirement (6).

The Application Evaluation Report will also list the various schools for which transfer credit applies. This will allow you to get a comprehensive view of the courses you brought in and the amount (if applicable) you need to take.

Finally, the Application Evaluation Report provides terminology for conditional and unconditional admission status based on copies of the transcripts, enrollment system and academic requirements.

Transfer Credit

• Associate’s degree students must complete a total of 60 semester hours of the requirements of your degree program

– Maximum amount of transfer credit allowed for this degree: 45 semester hours*

• Bachelor’s degree students must complete a total of 120 semester hours of the requirements of your degree program

– Maximum amount of transfer credit allowed for this degree: 90 semester hours*

• Master’s degree students must complete a total of 37 semester hours of the requirements of your degree program

– Maximum amount of transfer credit allowed for this degree: 18 semester hours*

*This amount is not guaranteed but based upon your transfer credit applicable to your degree program at Columbia Southern University

Admissions Status • When you enter CSU, you are placed into one of

two categories:

• Conditional – Six semester-hours have not yet been successfully

completed.

– All official documents have not yet been received.

– Student is required to meet qualitative/quantitative measures during the first 12 semester hours approved by the Academic Advisor.

– Students may complete up to 13 undergraduate credit hours of 7 graduate credit hours before submitting official transcripts to CSU

Note: any one or more of the above will place the student at Conditional status until all requirements are met.

• Unconditional – Application approved, transfer credit evaluated, all

required official transcripts on file, and academic requirements of Conditional status have been fulfilled.
