The officer had just stepped back out of the car, from running some information over the car radio, only moments before the truck crashed. Both the Officer and other people associated with the crash were spared any serious injury, but they were “run up the embankment.”

• Unfortunately, Key Performance Indicators in this case are measured by collisions & highway closures that make the nightly news.

• OW/OD is rarely notified of collisions or traffic stoppage due to OW/OD loads “hung up” at construction sites.

• Ironically there was a physical “GAP” between the central office and the districts, causing miscommunication.

• No defined protocol has been established for reporting construction notifications within each district.

• No standardized notification form.• District engineers were not notified that specific

S5 Actions Taken Members of the KYTC OW/OD team met with the

Division of Construction, Transportation Engineer Director & Transportation Engineer Specialist.

The Construction Notification form was presented with a Q & A session.

A “trial run” of the newly implemented Construction Notification report was sent to District 7

S6 Results• All district engineers will be required to complete the Construction

Notification form as part of the pre-construction checklist and email them directly to the OW/OD group inbox 10 days prior as standard protocol.

• A Dept of Highways Transportation Engineering Specialist has agreed to be the single point of contact to coordinate accurate reporting from district engineers.

OW/OD now has a process and information that is:Specific – no more guessing who sends what info

Measurable – standardized reporting allows for compliance and KPI review

Achievable - All 12 districts reporting on same form to one location

Relevant &; - Only necessary info will be reported. No more Bid proposals or unwanted info

Timely - All notifications must be submitted at least 10 days prior

S7 Standardization• The Construction Notification log standardizes the data required

for all 12 districts with a single point of contact.

• The OW/OD section may now begin filing these notifications in an easily accessible area for each member to reference and used to update the OW/OD restriction web page.

• District engineers now have defined reporting protocol in all 12

districts.

• The new Reporting form provides necessary information for Bentley implementation.

and consistent manner. In order to better accomplish this, a new policy and form have been developed.• • The “KYTC – Division of Motor Carriers Highway Construction Notification Form” has been developed and will be the method that notification is given to communicate

any restrictions that our construction activities will place on motor vehicles. This form should be completed and e-mailed to [email protected] at least 10 business days prior to the beginning of any restrictions. This form should be discussed at the preconstruction meeting and should be included in the Pre-Construction Conference Checklist. Questions about completing the form can be answered by calling the Overweight/ Over-dimensional Section at 502-564-1257.
