I. DOCUMENTARY REQUIREMENTS:

1. For the first time applicants; Local Chapter Constitution and By-laws.

2. For renewing members; Amendments or Revisions of the Local Chapter Constitution and By-Laws.

3. *Annual Report of previous federation.

4. *Directory of Local Chapter Officers. (see ES.pub)

Note: The filename will be Local Chapter-ES.Example for Angeles University Foundation:

5. *Official list of Local Chapter Officers, adviser/s and members. (see LCMembers.xls).

Note to LC heads: We will still make use of the JPIA codes for the member Local Chapter.

The JPIA Number Code will be: Federation Year- Region/Local Chapter Number- Member #

Example for Region 3, Angeles University Foundation:

7. Calendar of Activities. 8. *Resume of all graduating students.

9. A compact disc containing the following files:1. Soft copy of a formal picture with white background of each

member. The file name of each picture should be changed into the name of

the respective member. (e.g. Julius S. Corpuz.jpg) The picture will be seen in your membership card.

2. A .txt file containing the members’ information. (See MI.txt for a sample) The format for every member will be

NAME, PHOTO FILENAME, POSITION, ID NUMBER, SCHOOL

The file will be used for the printing of membership cards.
