Our Achievements:Implementation of quarterly learning institutes where

our leaders learn:

• evidence-based leadership, specific tactics and skills that connect us to purpose, align us with goals and successes of the organization and recognize excellent performance and contributions;

• about accountability, leader evaluation, critical conversations and goal alignment;

• skills to connect and communicate more effectively with their staff using Huddle Boards, rounding and stop-light reports for internal communication.

DRHC Standards Team has translated our organizational values into real actions and behaviours; our “Standards of Behaviour”. Modeling these standards enable us to function as a high performing healthcare team and provide consistent, high quality service to our patients and their families.

• We have had success in recruiting new physicians: Dr. Ian Ayers, Dr. Izabela Apriasz, and Dr. Riaan Bester.

• We are very honoured to have a large group of dedicated volunteers that do many different things to support our patients and staff.

• We were very proud to receive a Gold Level Quality Healthcare Workplace Award for our focus and strategic approach to creating a culture and workplace that supports high quality services.

Quality care and patient safety is our number one priority. Over the past year, staff and medical staff have worked very hard on a number of projects and initiatives to advance

quality and safety.

Our Achievements:• We have made significant improvements related to

medication safety through our medication reconciliation process.

• We have also embarked on a new project to modernize how we manage and process medications to assist us in ensuring that the right patient receives the right medication, the right dose, through the right route, at the right time with the right documentation.

• We will be using new computer assisted technology and have been working to create computerized standard order sets to reflect evidenced-based best practices for many common disease presentations.

• We have made significant improvements in raising awareness among staff, medical staff and volunteers with respect to hand hygiene compliance. Our excellent infection control performance continues to be supported by thorough housekeeping, cleaning, and hand hygiene practices.

• Our Obstetrics Team has been certified under the MoreOB (Managing Obstetrical Risks Efficiently) program. This program promotes high standards of patient safety and skills in obstetrics.

Under the leadership of Dr. Kerri Wilson, Chief of Inpatient Services, and Joan Mallyon, Director of Professional Practice, the team completed the first phase of the program with distinction.

2013/14 Quality Improvement HighlightsThrough excellence in safety, quality and service delivery, the Dryden Regional Health Centre continues to strive to ensure our patients and their families receive the health care services, information, access, collaboration and support they need. Our 2013/14 quality improvement accomplishments reflect our commitment to be the best we can in health care delivery at a local, regional and provincial level.

Access to Care• Early morning access to patient laboratory results ensures providers can make timely treatment decisions

Effectiveness in Care• Communication huddle boards have been displayed supporting daily/weekly sharing of information with all staff. • The DRHC continues to manage resources effectively with a positive year-end financial report • Staff & physician engagement continue to focus on strategies aimed at making the hospital the best place to work

Patient Satisfaction• All patients leaving the DRHC in-patient unit receive a post discharge phone call to answer any questions regarding their health and

hospital care• Positive improvement in patient satisfaction scores reflected from survey questions, “Would you recommend this hospital” and

“Overall quality of care”

Safe Delivery of Care• All admitted patients & those discharged home have a complete list of medications with a follow-up report to their family doctor &

local pharmacist• The DRHC continues to report zero hospital acquired infections (CDI, MRSA and VRE) • Providing quality surgical care, the DRHC continues to utilize the Safe Surgical Checklist on all patients • Our hand hygiene patient & provider awareness campaign continues to be a success in our facility with positive reports across the

organization

Integrated Care• Reducing hospital readmission, the Hospital and Family Health Team collaborated to provide patients that have experienced a heart

attack an opportunity for education and follow up with a team of health care providers

2014/15 Quality Improvement PlanQuality Improvement is a systematic approach to making changes that lead to better patient outcomes (health), stronger system performance (care) and enhanced professional development. (Health Quality Ontario)

Each year, the Dryden Regional Health Centre designs and implements projects specifically targeted at improving quality of services and moving the organization towards its goal to be the BEST; employ the best people, be the best place to work; have the best facilities and equipment and provide the best patient care for the best patient experience.

In 2014-15, the DRHC will be focused on improving:

Access to Care by:• Reducing wait times in the Emergency Department and improving patient flow.

Effectiveness in Care by:• Improving organizational financial health; • Improving Physician engagement; • Increase Employee engagement; and • Reducing unnecessary short term sick leave.

Integrated Care by:• Improving patient satisfaction; and • Improving the delivery of culturally safe care.

Patient-centred Care by:• Improving patient satisfaction; and • Improving the delivery of culturally safe care.

Integrated Care• Ensuring medications are reconciled on admission and before discharge; • Reduce hospital-acquired infection rates; • Reduce rates of deaths and complications associated with surgical care; and • Ensure all Accreditation Canada standards and Required Organizational Procedures are met.

Our “Quest for the Best” strategy recognizes that:

• working with patients is critical to understanding the experience of illness and care

• it’s no longer what we do to patients, or for patients but what we do with patients.

Our goal is to involve patients as active participants in their own care and in helping us improve our care processes. By embedding the patient voice in our services, we hope to:

• improve quality and safety,• reduce waste and inefficiencies, and• improve overall outcomes.

Our Achievements:• We continued to work collaboratively with our First Nations and Aboriginal partners in meeting the

needs of people throughout our region.

• We recognized that delivering safe care means culturally safe care.

• We provided approximately one-half of our hospital staff and medical staff day-long training in helping them to appreciate that recognition of culture is critical in providing effective and safe care.

• It is our goal to have mandatory diversity and culturally safe care training available for all health providers over the next year.

• This training was delivered by experts in cultural diversity from Sioux Lookout Meno Ya Win Health Centre.

Our Aim:To advance and maintain affordable,

sustainable health services

Our Achievements:• Through the efforts of our entire

organization, we were able to come out of the year with a $30,000 surplus from operations.

• Through the advocacy efforts of the Ontario Hospital Association’s Small Rural and Northern Leadership Council, we received the support of the provincial government of an additional 1% on-going base budget adjustment (approx. $170,000/year for the DRHC) and a multi-year commitment for an annual $20 million transformation fund for small, rural and northern hospitals. The Northwest’s share of this fund was approximately $3.5 million dollars, which helped to fund over $0.5 million in capital equipment, training and technology for the DRHC in the past year.

We cannot provide quality services and excellent patient experience without modern facilities and equipment.

The Dryden Regional Health Centre is fortunate to have the tremendous support of communities and businesses in the area.

Through the leadership of the Dryden Regional Health Services Foundation and the generosity of service groups, businesses, and individuals, we were able to purchase new equipment to support our oncology program, surgical services and diagnostic imaging departments.

Our inpatient room renewal project is also on track for completion over the next few months.

Our Aim:To advance and build a high performing, locally integrated health system

Our Achievements:• This past year saw the formation of the Dryden Area Local Health Hub Steering

Committee. The Health Hub is an important component of the North West Local Health Integration Network’s (LHIN) Health Services Blueprint. The Hub brings health service providers together to work in creating a more integrated, seamless and coordinated local health system.

• We’re collaborating on the development of a Regional Orthopedics Model and on-going enhancements to emergency and critical care support for Northwestern Ontario residents.

• We are working with other Northwestern Ontario Hospitals as part of the North West Health Alliance in creating capacity to manage information and technology throughout the region.

• We are proud to partner with health service providers throughout the area to advance the quality and effectiveness of mental health and addictions services for our First Nations communities.

• This past year, we signed collaboration agreements with Grand Council Treaty # 3 to support the Minobimaadiziwin and Paawidigong First Nation strategies.
