Facial oedema and exanthematic pustulosis: casereport
An adolescent girl experienced facial oedema andexanthematic pustulosis after receiving an unspecifiedantibacterial, which recurred during treatment withamoxicillin/clavulanic acid and cefuroxime.
The girl presented for allergy tests at age 17 years. Shereported receiving an antibiotic (that she described like apenicillin) for a cold 1 year previously [route and dosage notstated]. She had developed facial oedema and exanthematicpustulosis in the peribuccal region and forehead after 3 days oftreatment.
The girl’s reaction resolved after 3 days of systemiccorticosteroids. The same reaction occurred when shereceived amoxicillin/clavulanic [indication, route, dosage andduration of therapy before reaction onset not stated] acid3 months prior to presentation. This also resolved withcorticosteroid treatment. Skin prick and intradermal tests werenegative for benzylpenicilloyl polylysine, benzylpenicillin, amoxicillin, ampicillin, cefuroxime and minor determinantmixture. Oral challenge to penicillin V, ampicillin andamoxicillin were also negative after 2 hours. She was givenamoxicillin/clavulanic acid every 8 hours for 5 days with noreaction. However, when given cefuroxime every 8 hours for5 days [route and dose not stated] she developed exanthematicpustulosis, similar to her previous reaction, on the fifth day.
Author comment: Drug allergy is not always diagnosedwith tests performed inside the Allergy Department of ahospital. Sometimes, we have to repeat the conditions underwhich the reaction occurred.Burgos Montero A, et al. Exanthematic pustulosis by cefuroxime: A case report.Allergy 67: 432, Nov 2012.
