About the scheme The aim of the programme is to provide an optimal environment for personal and professional growth for the trainee and enable them to:

Be eligible to enrol on the Specialist Division of the Register in General Practice. Practice life-long learning to maintain them in their career as general practitioners. Promote self-awareness and how it affects doctor-patient, personal and professional

relationships. Practice critical evaluation, auditing and carrying out research.

The basic aim is to enable doctors, at the end of their training, to function independently as competent family doctors within the health care system and to be aware of their own changing personal, educational and developmental needs in order to be able to meet them.

In order to promote life-long learning, an interactive teaching model, with a strong emphasis on self-study in order to stimulate the trainee's creativity, is used. Within the small group work, the trainee learns teamwork, how to keep secrets and how to accept changing truths. Through the 'human doctor' programme, he/she develops counselling skills and achieves human growth. During the four-year programme, which combines high challenge with high support, the trainee hopefully develops competence in dealing with disease, develops a professional identity and learns to heal.

The scheme is based on the first floor in the Graduate Entry Medical School (GEMS) Building in the University of Limerick.

The ethos of the programme is experiential, that is, learning by doing. The content of the programme is dictated by the trainees' needs, with topic menus provided by the scheme. Local general practitioners through the CME scheme influence the curriculum for these menus.

The methods used in the day-release small group are varied but are usually problem-based with time given at each session for process analysis. This keeps the session real and allows trainees to assess their successes and failures and tease out why. As with all problem-based programmes, there is a critical evaluation and personal development section to enhance the trainees' learning experience. A very successful trainee-led journal club is also run every week where self-chosen pre-read papers are evaluated.

Dermatologist Dr Bart Ramsey is an excellent asset to the scheme. Being a graduate of the Dublin GP scheme before becoming a consultant dermatologist has both helped and sparked his keen interest in GP training. His dermatology forum, which is run over four years, offers a comprehensive course in dermatology to the trainees, and has proven to be invaluable to the Limerick graduates

The Limerick GP Scheme is meeting the challenge of helping trainees to maximise their individual strengths by providing a realistic timetable with a self-selected relevant syllabus. Add in a sprinkling of enthusiastic general practice teachers and a vibrant University Campus, and you have an environment of personal and professional growth.

GOVERNANCE

Under the governance of the Irish Medical Council and the Irish College of General Practice the Mid-

West Specialist Training Programme recruits, trains and certifies General Practice Trainees for the

Specialist General Practice division of the Medical Council. Mid-West Specialist Training Programmes

one of 14 schemes in the country and is locally regulated by the Mid-West Specialist Training Steering

Committee.

Criteria Document

The Mid-West Specialist Training Programme Committee report to the National Coordinating Training

Committee (N.C.C.T.), which reports to the Post Graduate Training Committee of the ICGP (P.G.T.C.).

Recruitment Recruitment is by open completion, advertised and administered nationally by the Irish College of

General Practice. In 2017 there were 5 interview centres, 3 based in Dublin, 1 for the South in Cork, 1

in the West Galway. These centres are agreed in the autumn for the upcoming interviews the following

spring interviews. Candidates are ranked nationally through STT based interviews. They are allocated

places based on ranking and preference. Dr Griffin will attend the Career’s day in UHL to promote

General Practice as a speciality. The Mid West Scheme also attends the National Careers day in

November, which is held by the ICGP in Dublin. Candidates are also encouraged to sit in on our

Wednesday teaching session and meet with the PD team and the current trainees. This year on the

request of the NDDP we were to recruit 13 Trainees unfortunately we only secured 10 trainees.

Early Exposure to General Practice In the first 2 years of General Practice Training the Trainees in the Mid-West Specialist Training Scheme will

spend 4 weeks attachment to our Trainers Practices’. These attachments ideally should take place 1 every

six months and are arranged early by the Trainee. Arrangements should be confirmed in the first 6 weeks of

each 6 months. Every effort should be made by the Trainees to spend 1 attachment in the planned placement

that they will be assigned to in their Registrars years. The Trainees supervised by the Trainers complete a

self-assessment record in each attachment, which is signed by both the Trainee and the Trainer. These

records are monitored by the Programme directing team and are presented at all one to one assessment or

when required.

The 4th Year Morning Programme

Dr Johnny Loughnane facilitates the morning sessions and covers “special skills” with an emphasis on

General Practice. Where possible skills are taught in a General Practice setting and include the

This module is based on the ICGP Core Curriculum. The Trainees select the topics and choose the

learning outcomes. The sessions can be problem based; “show and tell” or case scenarios and they

are resourced by the trainees. These sessions are focused on the common health issues seen in

practice and in particular the methods and mechanisms of dealing with problems that are not “clear

cut”. The sessions also include a dilemma topic that is chosen by the trainees. Evidenced based

solutions source by the trainees are explored where possible. This helps the trainees to make

informed judgements on issues using guidelines but also taking into account all other issues

influencing the particular dilemma.

Management of Musculoskeletal Problems in GP – Facilitated by Dr Michael

Griffin

The emphasis here is on MSK assessment. It was based on joint examinations with plenty of

hands on experience.

At the completion of each learning module, which involved a separate joint the Trainee, should

be able to:

The 3rd Year Programme

The 3rd year trainees engage with and complete a summer curriculum schedule each year. Dr Blaithin

Tuohy facilitates this

The aims are:

Introduction to the 3rd and 4th year curriculum for General Practice

Introduction to the community services with which they will liaise with during their

General Practice work i.e. Community Dietician, Speech Therapist, Community Welfare

Initiate their Video Analysis

Continue with their personal and professional development with the scheme’s Human

Doctor run by Dr Griffin and facilitated by Mr Michael O’Mahony, Psychologist.

The 3rd programme continues with morning teaching sessions from September to July facilitated by Dr

Tuohy. The curriculum is chosen and resourced by the Registrars with learning needs selected to assist

their learning needs for their 1st year in General Practice. Dr Tuohy also arranges for the Registrars

video assessments to be completed and evaluated during this year and supports the Registrars in

achieving completion of their Audit. The 3rd year registrars engage with the 1st and 2nd year trainees in

the afternoon sessions bringing their experience to the mixed group. Appendix 2 Schedule and

Teaching and Day Release

The 1st, 2nd, and 3rd Year Programme

The 1st, 2nd and 3rd Year Programme is facilitated by the Programme Directing Team. The 3 years are

integrated and these small groups alternate over the year. Curriculum planning is carefully prepared

for the afternoon sessions using the website forum all through the summer; all trainees are asked to

submit topics for inclusion. Dr O’Connor collates these and a topic summary is created. On the planning

day the schedule is completed with the chosen topics and the Trainees are assigned to resource each

topic.

All trainees are also assigned a day that they present a problem orientated journal of their choice. The

programme starts with 20 minute problem orientated Journal session, and then a 90 minute general

practice topic session and then 20 minute problem based discussion. There is a review of the day’s

sessions, feedback is given by all and collated at the end of each teaching day, and this review is filed.

Appendix 3 Schedule and Outcomes

Dermatology Forum Dr Bart Ramsay a GP trained consultant dermatologist runs a three-hour dermatology forum per term

on the half-day release. Trainees are encouraged to post their digital dermatology photos on the Web

for discussion.

The Human Doctor Module: The Human Doctor module is taught during all 4 years of Training. It begins with a mindfulness day

held in the beautiful serene surroundings of Glenstal Abbey in Murroe Co Limerick. This module is

specific to the Mid-West Training Scheme and is designed to address the challenges that General

Practice will confront the Trainee with and help our Trainees develop in a personal and professional

capacity. Psychotherapist Mr Michael O’Mahony and PD Dr Michael Griffin facilitate these sessions.

Some of the main aims of the Human Doctor Programme are:

the double meeting in February which was held in The Falls Hotel, Ennistymon

All meetings followed a similar format to previous years, that being educational followed by business

sections.

Topics covered during the educational sections included presentations/discussions on contraception,

common ENT problems, frailty, mental health, acupuncture, refugee health and debriefing after

sessions. There were also presentations conflict resolution, complaints in GP, compassion and

management of the “heartsink patient”.

The agenda for the business portion of the meetings reflected topical training scheme and Trainer

issues.

Steering Committee Meetings are held three times a year in February, May, and October. The main functions of

Steering Committee as per the terms of reference in the Criteria Document are as follows:

1. To devise and operate schemes of training subject to criteria and standards set down by the Irish

College of General Practitioners

2. To select and maintain a registrar of suitable hospital posts and maintains an accreditation process

for each post.

3. To select and maintain a registrar of suitable teaching practices and maintains an accreditation

process for each trainer and trainee.

4. To devise and implement a policy on trainee recruitment and selection under relevant legislative

frameworks whilst promoting the principles of fairness and equity

5. To ensure that trainees are offered the appropriate GP trainee contact, as negotiated between the

Irish Medical Organisation and the Department of Health and Children

6. To issue registrars with a Certificate of Satisfactory Completion of Training where appropriate and

to identify the further learning needs of those registrars who fail to achieve satisfactory completion of

training

7. To ensure that the “Certificate of Satisfactory Completion of Specialist Training in General Practice”,

are signed by the Programme’s Steering Committee Chairperson. (It is the responsibility of the

Steering Committee to ensure this Certificate is signed, if the candidate is deemed to have

satisfactorily completed training; and refused, if the candidate is not deemed to have satisfactorily

completed training)

8. To ensure that funding for remediation purposes be made available following failure by a trainee to

achieve satisfactory completion of training where remediation is considered appropriate.

9. To liaise with the College and other bodies with an interest in postgraduate training for General

Practice.

10. To facilitate, encourage and support all teaching personnel in the further development of their

teaching skills.

11. To devise and implement a policy on Staff Recruitment.

12. To arbitrate when required on problems within the training programme and in disciplinary

matters to adhere to personnel policies of the appropriate Health Board employing authority.

13. To ensure adequate funding and resources for the operation of the programme and to frame an

annual budget with appropriate financial projections

Review the syllabus/implement teaching programme

1.1 Induction

All new trainees are inducted into the scheme and as part of this process they are introduced to the

Learners Charter, Core Curriculum, ICGP Criteria and the HSE terms and conditions of employment.

1.2 The Day Release Course

The day release courses are designed to maximise the general practice experience and expertise. The

afternoon sessions have 3 groups of 12 Trainees mixed from 1st, 2nd and 3rd years. There were 40

teaching session in 2016-2017. The 3rd and 4th year Registrars do a full day release with a dedicated

summer schedule. There were 42 teaching sessions in 2016-2017.

The day release continues through the summer for the 3rd & 4th years. The 4th years have an average of

42 teaching days per year, the 3rd years an average of 42 and the 1st 2nd years 39 teaching days per year.

1.3 Aim of the day release process.

The production of competent GP’s who are able to meet the needs of the community, whilst attending

to their own needs of both personal and professional development.

To develop a commitment to lifelong learning

To develop an ability to foster self-awareness

1.4 The Format of the Day Release

The 1st,2nd & 3rd year PM sessions work from topics/issues/problems identified by the trainees on the website

discussion board. They all vote at the curriculum planning meeting and volunteer to prepare the presentations

in pairs. They also write 3 to 4 learning outcomes. The method of teaching is also decided i.e. problem

case/role play/quiz etc.. The PD Team facilitates this process. All presentations are posted on the website. A

half hour critical reading starts each session; the paper is presented by the trainee and read by the group who

then evaluate the selection choice. This process allows trainee centred learning and encourages trainees to take

responsibility for their own learning needs- an essential prerequisite for the development of the skill of lifelong

learning. In order to promote this, an interactive teaching model with a strong emphasis on self-study in order

to stimulate the trainee’s creativity is used. Within the small group work the trainee learns teamwork, how to

keep secrets, how to accept changing truths. Through the Human Doctor Programme the trainee develops

counselling skills and achieves human growth. The four-year programme combines challenge with support so

that the trainee develops the competence in dealing with disease and develops a professional identity.

1.5 Educational Strategies used in the day release

A trainee centred, integrated, community–orientated approach is used in the day release and the following

teaching methods are used. The half day release for 1,2 & 3rd years is based on common important problems.

Brain storming takes place to generate at the most four learning outcomes. Teaching methods are then selected

i.e.

Case based scenarios for clinical problems

Quiz based sessions for information transfer

Role play for attitudinal sessions

Critical reading to evaluate research

Audit to teach how the trainees how to evaluate clinical practice.

1.6 The 3rd year programme

The 3rd year morning sessions are run as an evidence based problem session where the trainee presents the

evidence for General Practice Problems. Social, ethical, and legal issues are covered. All 3rd year trainees did

videoed consultations, evaluated by their trainers in the practices and written feedback on assessment is

provided. All 3rd years attend the Cardiac First Response Course. This is also assessed and certificates of

satisfactory completion are attained. All 3rd year trainees submit a complete audit project, which are presented

at the ICGP joint Christmas meeting.

The 3rs year registrars have now completed their summer schedule. They had sessions with input from a

numbers of our community disciplines such as Speech Therapy and dietetics to familiarise them with these

services to improve future GP integration with Primary Care teams in practice. They have been attending day

release in UL covering the Trainee driven curriculum including Journal review, GP topics and have completed

their ICSP smear taking theory course.

During the summer they also began their audits which are now near completion for poster presentation at the

ICGP faculty meeting in December

1.7 Summer Curriculum for the 3rd years

The 3rd year trainees engage with and complete a summer curriculum schedule Last year.

The aims are:

Introduction to the 3rd and 4th year curriculum

Introduction to the community services with which they will liaise with during their General

Practice work i.e. Community Dietician, Speech Therapist, Community Welfare Officer

Initiate their Audit Project

Initiate their Video Analysis

Continue with their personal and professional development with the scheme through the

Human Doctor Programme

1.8 The 4th Year Programme

Dr Johnny Loughnane facilitates the morning sessions and covers “special skills” with an emphasis on

General Practice. Where possible skills are taught in a General Practice setting and include the following.

In early July the fourth years met and decided among themselves areas they wished to cover during the year.

Dr Loughnane then went through the list of their objectives with them in a group discussion. Teaching

sessions that are chosen included management of various chronic diseases in Primary Care, Dermatology,

ENT, Ophthalmology, Practical Procedures, Sexual Health, Paediatrics, Practical Practice Management,

Palliative Care and investigations in Primary Care.

During the summer months the 4th years attended the Wednesday morning sessions. Areas covered were

drawn from the above agreed list.

Dr Agnes Kelly also met with the 4th year GP Registrars to plan sessions that she would cover with them

during the coming year. The summer sessions ran seamlessly into the autumn term from September

onwards.

1.9 The 4th Year afternoon sessions has 3 components.

1.9.1 Practice Management - Facilitated by Maria Leahy.

The Management Module is going from strength to strength, as the trainees are now more actively

participating in the practice management audits thus learning more from the process. There is also an increased

awareness from trainees that General Practice is a business like any other in their market place and that there

is no exemptions for the GP employer when external reviews are carried out by NERA and the HSA

There were 12 afternoon sessions last year and within that time the Trainees got a comprehensive overview

of Recruitment, Industrial Relations, Business Management, Health and Safety and Finance. These teaching

session were complemented by all relevant information and templates given for their future use in General

Practice. The module was evaluated and there was no recommended changes proposed for the coming year.

REPORT ON OPERATIONAL PLAN 1.9.2 Women’s/Men’s Health – Facilitated by Dr Agnes Kelly

This module is based on the ICGP Core Curriculum. The Trainees select the topics and choose the learning

outcomes. The sessions can be problem based; “show and tell” or case scenarios and they are resourced by

the trainees. These sessions are focused on the common health issues seen in practice and in particular the

methods and mechanisms of dealing with problems that are not “clear cut”. The sessions also include a

dilemma topic that is chosen by the trainees. Evidenced based solutions source by the trainees are explored

where possible. This helps the trainees to make informed judgements on issues using guidelines but also taking

into account all other issues influencing the particular dilemma.

1.9.3 Management of Musculoskeletal Problems in GP – Facilitated by Dr Michael Griffin

This year the emphasis was on MSK assessment. It was based on joint examinations with plenty of hands on

experience.

At the completion of each learning module, which involved a different joint, the Trainee, were able to

demonstrate all the learning outcomes. The facilities in our main office were satisfactory but going forward

the use of the GEMS skills laboratories would be better.

Since January 2017, Dr O’Connor has been working as a Senior Research Fellow in the UL Graduate Entry

Medical School.

He also working one day a week in general practice.

He lecturers in the newly established Paramedics undergraduate and graduate entry course.

Is the medical director of the newly established Medical Exercise Referral Programme. This is a new

programme jointly established between the UL Graduate Entry Medical School and the UL Dept of Physical

Education and Sports Science.
