Fourth Street Clinic helps homeless Utahns improve their health and quality of life

by providing health care and support services. By increasing homeless Utahns’ access

to primary care delivered in a coordinated clinical setting, Fourth Street Clinic is a

major partner in promoting community health, ending homelessness and achieving

across-the-board health care savings.

HISTORY

The mid-1980s brought a dramatic rise in Salt Lake City’s homeless population. During this

time, Salt Lake City’s downtown was revitalized and several substandard, single-room

occupancy hotels (SROs) that offered rooms for just a few dollars a night and required no

leases were razed and redeveloped. In total, 800 low-income housing units were torn down,

displacing 1,000 very poor residents from their homes and jobs. With no public plan to

rapidly re-house or support these residents, homelessness moved from a sporadic,

temporary experience to endemic and permanent.

To fill the service gap, nonprofits such as Fourth Street Clinic began to form. Founded by

medical anthropologist Allan Ainsworth, PhD in 1988, Fourth Street Clinic began as a triage

clinic staffed with one part-time nurse who relied heavily on hospitals for patient treatments.

Today, with a staff of 45 and a volunteer network of more than 200, Fourth Street Clinic is

a comprehensive health care home that serves 5,300 homeless men, women and children

with integrated primary care, behavioral health, pharmacy and case management services.

For 23 years Fourth Street Clinic has pioneered health care services for Utahns experiencing

homelessness. Our founder, Allan Ainsworth PhD, gave shape and form to the moral value that even

the sickest, poorest members of our community should have access to primary health care. We at

Fourth Street Clinic believe that providing health care is not only the right thing to do, it is also a

long-term, time-tested, and cost effective solution that breaks the cycle of homelessness. As Dr.

Ainsworth begins his retirement, I wish to assure him that the mission of Fourth Street will remain

strong and that the long-term partnerships established within the community will endure so that one

day Salt Lake City will experience an end to homelessness.

As I settle into the Executive Director position, I see the Fourth Street Clinic and the national

network of community health centers as one of the essential components of America’s health care

system. It is health care as it should be—data and high quality-driven practices where patients

seeking services are treated with dignity and respect. I am motivated each day by our patients’

courage and strength as they better their health and embark on a brighter future.

I am also struck by the changing face of homelessness. While Salt Lake City’s chronic

homelessness shows signs of decreasing, Utah’s overall homeless population is growing – up

14.5% since 2005 with families now comprising nearly 50% of all homeless Utahns. The recent

economic downturn continues to erode hope, strip away personal safety nets, and threatens to

propel a new generation into a lifetime of financial instability. Fourth Street Clinic is a daily reminder

that people provided with regular health care are given the means to stay healthier and improve the

quality of living for themselves and those around them.

Fourth Street Clinic is responding to these challenging times by expanding services that provide a

vital lifeline in the form of support services including assistance with insurance eligibility, permanent

supportive housing referrals, and education on financial programs. These services, coupled with

quality health care, make Fourth Street Clinic a remarkably effective provider, comprehensively

addressing the many inextricably linked needs of the homeless population.

I am deeply honored to be serving the Salt Lake City community as Fourth Street Clinic’s Executive

Director and look forward to growing the rich legacy that is the Fourth Street Clinic – a strong,

community based organization, providing health care and hope to Utah’s homeless population.

Fourth Street Clinic is governed by a volunteer board of directors that meets

monthly. Members of the board serve up to two, three-year terms and then rotate

off the board for at least one year. They are also asked to serve on one

of six committees and donate personally or raise funds for Fourth Street Clinic.
