Welcome to beautiful Northern Michigan where the views are spectacular and the sunsets are amazing!

I am delighted that you have chosen to participate in this

year’s Annual Conference: Enriching Patient Care in an Era of Emergencies and Epidemics. With the help of our dedicated board members, staff and community partners, this year’s conference has been designed to challenge, excite and inspire. In addition to the formal sessions, I encourage you to take advantage of the opportunities to network, share ideas and best practices with your colleagues from throughout the state. Today’s health care environment is on a fast track with a focus on quality improvement, measurable outcomes, integrated service delivery and addressing the social determinates of health.

I believe that Michigan Health Centers are the key to a successful transformation that not only begins to reduce the cost of health care, but also has at its core the health and wellness of the more than 615,000 residents that rely on our health centers as their medical home.

The Michigan Primary Care Association is proud to be the voice for 42 Michigan Health Center organizations which provide quality, affordable, comprehensive primary and preventive health services at over 260 delivery sites across the state. Our members are the medical homes for individuals who are low income, uninsured, underinsured, elderly, minority, migrant and seasonal farmworkers, homeless, and those living with HIV/AIDS. If not for Health Centers, many of these individuals would remain isolated from health care due to lack of insurance, inability to pay for services, and transportation, language, and cultural issues.

I hope that you will leave Traverse City with renewed energy and dedication to face the challenges of shaping health service delivery for the next generation.

Experience [Governors AB] During this session, the Flint water

disaster will be discussed in the context of environmental justice and health equity. Participants will be able to: 1) Advocate for public health and safety to be the final determinants in decisions regarding the environment (both natural and built), and economic development, using the principles of environmental justice and health equity; 2) Apply the precautionary principle before introducing new substances into the environment; 3) Explain the evolution of knowledge regarding sources of lead in the environment and the consequences of low level lead exposure across the life course. Admin / Enabling TrackPerspectives from the Field: Lessons learned from primary care and public health partnerships working toward population health improvement – PANEL[Governors AB]The integration of primary care and public health can provide a powerful partnership to address today’s public health system challenges. Efforts to promote collaborative primary care and public health initiatives have been underway for several years; however, opportunity remains to build capacity at the local level to foster integrated population health improvement. To help facilitate partnership endeavors, the Institute of Medicine (2012) identified five principles that enable primary care and governmental public health to work together: a shared goal of population health; community engagement; aligned leadership; sustainable systems; shared and collaborative use of data and analysis. Applying these concepts is easier said than done and more examples are needed to share best practices, challenges and lessons learned in creating and sustaining partnerships. To help foster increased awareness and efficacy toward partnership, the HRSA Region V Great Lakes Public Health Training Collaborative (GLPHTC)—a six-state training collaborative in the Midwest—conducted a process to better understand the challenges and opportunities around primary care and public health integration. In doing so, GLPHTC identified 65 success stories that aligned with the IOM’s factors of success. This presentation will highlight the key findings of the GLPHTC success stories and in addition, three Illinois and Michigan centers will present on their lessons learned from current partnerships aligned with the IOM’s factors of success. Participants will hear practical approaches to developing and sustaining partnerships to improve population health. Clinical TrackPCMH and Health Homes: Integration of Physical and Behavioral Health[Governors AB] Integration of services has different meanings to different organizations and different meanings for different population groups served as well. Cherry Health stands as a marker of the highest levels of integration. They are also sorting out what levels of integration lead to best health outcomes for the different populations they serve. General TrackUsing Data to Understand and Address Workforce Challenges in Michigan[Tower B]The Solutions, Training, and Assistance for Recruitment and Retention (STAR²) Center, a project of the Association of Clinicians for the Underserved, was funded by the Bureau of Primary Health Care to help health centers address their clinician workforce challenges. Using its unique health center recruitment and retention data, the STAR² Center is able to identify specific workforce need at the health center, state, and regional level. Health center staff who are involved with their organization’s recruitment and retention efforts are invited to this breakout session to learn about free resources and assistance; how to use and understand their organization’s STAR² Center data profile; state workforce trends; and suggestions for meeting relevant challenges. The STAR2 Center will be hosting a technical assistance booth in the Exhibition Hall to provide one-on-one training on best practices for using their unique, free resources to boost recruitment and retention at your health center.Presenter: Allison Abayasekara, Director Training & Technical Assistance, Association of Clinicians for the Underserved

Admin / Enabling Track Social Determinants & Community Partnerships/Outreach and Enrollment[Tower C]Community Health Centers provide high-quality, accessible, affordable, and comprehensive health care to underserved populations, without regard for ability to pay. Robust, community-driven outreach and enabling services are critical for reaching populations with the greatest unmet need, connecting them to a broad scope of services, and ensuring their ability to fully access health care and achieve good health. During this session, the facilitator will describe a vision

for what a comprehensive outreach and enabling services model can achieve and introduce a strategic framework and toolkit that demonstrate the potential return on investment for implementing such programs. By the end of this session, participants will be able to: 1) Identify at least two ways to effectively integrate outreach and enabling services programs into health center priorities; 2) Apply the Outreach Business Value (OBV) strategic framework in order to effectively structure relevant outreach and enabling services; 3) Use theOBV toolkit to assess the financial implications of investing in specific outreach and enabling services.Presenter: Oscar Gomez, CEO, Health Outreach Partners

General TrackAdult Immunization in Michigan: Using the Standards to Increase Coverage [Tower B] This presentation focuses on current issues related to improving adult immunization in Michigan and the United States. Dr. Ryan will address these key issues, outline the need for the Standards for Adult Immunization (The Standards), and provide the recommended practices to integrate immunizations into routine adult patient care. Enabling / Finance TrackAligning our Practices with our Communities: Screening for Social Determinants of Health in Michigan Health Centers– PANEL DISCUSSION[Governors AB]Both the Institute of Medicine and the Centers for Medicare & Medicaid Services have called for evidence-based initiatives to better screen for and address the social determinants of health (SDH) in clinical settings. Recognizing and understanding variations in current SDH screening is critical for identifying best practices to address patients’ unmet social needs. In the context of an academic-community partnership between the Michigan Primary Care Association and the University of Michigan, we documented current screening practices across Michigan Health Centers. This session will present those findings and compare how the statewide implementation of a standardized SDH assessment aligns with existing practices. We will also specifically discuss the Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE) and its role in delivery system transformation in Michigan and nationwide. This session will highlight how CHCs can establish their own potential roles in planning for, responding to, and recovering from an emergency or disaster. The sessionwill include examples of CHC integration into local efforts,and how EPM activities benefit business continuity strategies. Clinical TrackTrauma Informed Care [Tower B]Trauma Informed Care (TIC) refers to an organizational structure and treatment framework that involves understanding, recognizing, and responding to the effects of all types of trauma. By using trauma informed care, a trauma survivor’s medical experiences can be positive and avoid iatrogenic harm. TIC uses a number of core assumptions: 1) Psychological and emotional trauma is central and pervasive; 2) Medical practitioners should use universal precautions (e.g. people should not have to disclose in order to receive TIC); 3) Behavioral symptoms are often attempts to cope with the trauma; 4) A goal involves returning a sense of autonomy and control to the person served.Presenter: Tom Moore, Founder and Owner, Two Moons, LLC

If your staff care for and care about each individual they serve, patients will receive superior service and have an extraordinary experience. In the wake of trauma associated with emergencies and epidemics, patients need empathy, hope and help; for their mind, body and spirit. But under the mounting pressure of productivity requirements, financial stresses, compliance mandates and inflexible EHRs it is difficult for staff to stay focused on the patient’s needs. Inspiring staff to demonstrate that level of personal caring (especially with the pressures on you)is a daunting task, but if you don’t do it, who will? And if you are like most leaders in healthcare, inspiring staff is an aspect of your job that you have had little training for. Clinical TrackThe Park Project: Extended Care Team Model [Tower B]This session will be a discussion of an extended care team model (ECTM) on the Park Practice at Family Health Center, Inc. in Kalamazoo. The ECTM included a Care Manager, Triage Nurse, and Referral specialist being added to the already existing team of, one DO, two NPs, one PA, four medical assistants, and two team assistants. This project utilized daily team huddles and PDSA cycles to improve interdisciplinary collaboration. The expanded care team model demonstrated improved quality measures in comparison to the organization as a whole. You Fit in and What is Your Liability? [Governors AB]

This session will help describe the paradigm shift in the U.S. involving substance abuse; describe current substance abuse statistics and trends; discuss what physician liabilities exist for a patient’s addition to or overdose from prescription drugs; discuss current litigation trends as it pertains to physician liability for particular actions; discuss the potential liability for authorizing others to dispense without supervision; issues relating to a physician’s failure to act, fraud, conflicts of interest and other actions which have been deemed improper in a malpractice action; actions medical professionals can take to better identify and prevent prescription drug misuse/abuse and diversion. Speakers will cover eight specific steps that would help protect a physician’s practice if they should have a negative result from their prescription writing. They will also discuss the current trends in the use of medication assisted treatments for opioid addiction and the argument for and against their use.

Prior to this position, she served as CEO and Public Health Administrator of a public-entity Federally Qualified Health Center in Illinois. Heidi has a Bachelor’s degree in Biochemistry and a Master’s in Public Health, both from the University of Illinois at Chicago. She served as a United States Peace Corps Volunteer in Turkmenistan in 2004-2006, and in 2009, became a Certified Public Health Administrator.

Renee Canady, PhD, MPACEO, Michigan Public Health [email protected] Renée Branch Canady PhD, MPA, serves as Chief Executive Officer of the Michigan Public Health Institute (MPHI); an academic, government, and research institute that strives to maximize positive health conditions in populations and communities through collaboration, scientific inquiry, and applied expertise. Prior to joining MPHI in 2014, Canady served as Health Officer for the Ingham County Health Department, Michigan’s first public entity local health department and federally qualified health center. In that role she provided oversight on innovative efforts such as the Ingham County Health Equity Social Justice program.Canady earned a PhD in Medical Sociology from Michigan State University, a master’s degree in Public Administration from Western Michigan University and a bachelor’s degree in Public Health Nutrition from the University of North Carolina at Chapel Hill.

Melissa DaSilva, BS, BSRN, MSChief Innovation Officer, Central City Integrated [email protected] An ever-expanding understanding of the social determinants of health has driven Melissa’s career along a path that now barely reflects academic training. As Chief Innovation Officer, she focuses on all aspects of public health administration in addition to Federally Qualified Health Centers administration, including Health Resource and Services Administration site visit management, Uniform Data System report development, electronic health record incentive program administration, direct patient care, clinical management and health education.

He currently serves as Director of Interprofessional Practice at DentaQuest Institute where he oversees national initiatives that aim to improve the health of all. Dr. Boynes previously served as the Director of Dental Medicine at a Federally Qualified Health Center in the Pee Dee Region of South Carolina. He has authored or co-authored over seventy-five publications relating to dentistry, public health, pharmacology and anesthesiology and has been recognized by the National Health Service Corp as one of the 40 Top Clinicians and the National Children’s Oral Health Foundation lists him as a Dentist of Distinction.

Corinne Bradley MSN, RN Clinical Director, Family Health [email protected] Bradley has been working in healthcare for the past 18 years in multiple capacities. She graduated from Kalamazoo Valley Community College in 2006 with her Associates Degree Science: Nursing, and Ferris State University in 2015 with her Bachelor Degree in Nursing Science and her Master Degree Nursing Science with a focus in administration. Since becoming a nurse, Corinne has had a multifaceted career including Medical/Surgical nursing, Labor and Delivery, Women’s Health, Home Care, Quality and Safety, and nursing leadership. Corinne has been performing as a nurse leader for the past eight years. Corinne is also involved in multiple professional organizations including the Michigan Nurses Association, the American Nurses Association, Golden Key International Honors Society, and Sigma Theta Tau International Nursing Honors Society. Corinne joined the Family Health Center in January of this year.

Greg Deacon, M.A. LLP CCSDirector of Outpatient Services, Cherry [email protected] Deacon started as an advocate on an assertive community treatment team at Harbinger in Grand Rapids, Michigan. He became a supervisor of the Fully Integrated Treatment Team at Touchstone Innovaré in 1998, after receiving his Masters of Arts in Counseling Psychology. During his tenure, simultaneously managing both Case Management and ACT teams, he was trained in Cognitive Therapy by the Beck Institute and Dialectical Behavior Therapy by Behavioral Tech. In 2006, Greg started the Counseling Center for Touchstone Innovaré and managed this program until Touchstone Innovaré merged with Cherry Street Health services (Cherry Health) in 2010. In 2011, Greg became the Director of Outpatient Services managing Leonard Street Counseling, Heart of the City Counseling Center, and Southside Health Center (Cherry Health’s medication assisted treatment program).

Kayla Doyle, LMSWDirector of Integration, Cherry [email protected] Doyle joined Cherry Health in 2008 for her MSW internship. She simultaneously served with Cherry Health AmeriCorps for 2 terms of service. She continued on to become a child and adolescent therapist at Burton Community Health Center, assistant to the Community HealthCorps program, and coordinator of migrant and homeless outreach. In 2010, she became the manager of the Cherry Health AmeriCorps team. During this time, she also took leadership of implementing the outreach and enrollment team in response to the Affordable Care Act and opening of the Marketplace. In 2013, Kayla accepted the Manager of Integration position at Cherry Health, focusing primarily on pediatric integration. Kayla became Director of Integration in February of 2016, focusing on all aspects of integration. Doyle received her B.S. in Psychology and Masters of Social Work with a clinical concentration from Grand Valley State University. In 2015, she completed the Certificate in Integrated Behavioral Health and Primary Care at the University of Michigan.

Lee Francis, MD, MPHPresident and CEO, Erie Family Health [email protected] Dr. Lee Francis is the President and CEO of the Erie Family Health Center. He is a board-certified internist caring for adult patients at Erie Family Health Center and an Assistant Professor of Clinical Medicine at Northwestern University Feinberg School of Medicine. Recognized for his leadership, Dr. Francis is the current Chair of the board of the Bulletin of the Atomic Scientists and the Chair of the board of the Illinois Primary Health Care Association and is involved in many other civic, community and professional organizations.

Stacey R. Gedeon, PsyD, MS Clinical Psychopharmacology, Director of Behavioral Health and Integrated Primary Care, MidMichigan Community Health [email protected] Dr. Stacey Gedeon is the Director of Behavioral Health and Integrated Primary Care for MidMichigan Community Health Services, a Federally Qualified Health Center located in Houghton Lake, Michigan. She is a clinical health psychologist who has worked in health care for 20 years. In addition to developing an integrated primary care program, Dr. Gedeon has established multidisciplinary programs to assist patients with chronic pain, diabetes and chronic complex diseases.

Phoebe Goldberg, MPH, CHESProgram Manager, Michigan Public Health Training Center [email protected] Goldberg is the Program Manager for the Michigan Public Health Training Center, a part of the HRSA-funded Region V Great Lakes Public Health Training Collaborative. In this role, she supports student development through coordinating collaborative projects and field placements that pair students, faculty and community partners in capacity building activities. In addition, Phoebe implements training needs assessments for public health workforce continuing education. Phoebe earned her MPH in Health Behavior and Health Education at the University of North Carolina Gillings School of Global Public Health and her BA in American Culture at the University of Michigan. She is currently the President-Elect of the Great Lakes Chapter of the Society for Public Health Education.

Oscar GomezCEO, Health Outreach Partners [email protected] Oscar Gomez is the Chief Executive Officer of Health Outreach Partners. Born to Mexican immigrants, he is bilingual in English and Spanish. Oscar holds a Bachelor of Arts in International Studies from Pepperdine University and has 23 years of community health experience. In 1992, he was an administrative assistant with HOP and in 2000, Oscar was named CEO. As CEO, he led HOP through two company re-designs including an expansion from a regional scope into a national scope where they focus on safety net organizations serving low income, marginalized and vulnerable communities. In addition to serving on numerous community and national board of directors, Oscar has a certificate from the UCLA Anderson School of Management, Health Care Executive Program and is a former LeaderSpring Executive Fellow.

Jaspreet Malhotra, MPHProgram Specialist, Michigan Primary Care [email protected] Jaspreet Malhotra is a Program Specialist with the Michigan Primary Care Association serving on clinical and enabling services teams. She has Bachelor of Arts in Sociology from the University of Michigan and a Master’s of Public Health from the University of Florida. Jaspreet worked as a Navigator prior to joining MPCA as a Certified Application Counselor. Her education and experience has been instrumental in her current role where she provides training and technical assistance on health insurance coverage, enrollment and other enabling services, as well as supporting the maternal and child health program expansion, model transformation and quality improvement.

Lynda Meade, MPADirector of Clinical Services and Michigan Quality Improvement Network, Michigan Primary Care [email protected] Lynda Meade is a health professional with extensive experience in program management and network development. As a professional in the field of health and human services for over 18 years, she has been a champion for improvement in policies, systems and supports that improve health, safety and wellbeing for the undeserved. As the Director of Clinical Services at the Michigan Primary Care Association, Lynda provides leadership and oversight in the day-to-day operations of projects carried out by the Clinical Services Team including the Michigan Quality Improvement Network (MQIN), practice transformation strategies, leadership committee support, and key partner development to maximize policies and resources. Lynda is currently serving as the Vice President of the Michigan Coalition Against Homelessness Board of Directors, member of the National Association of Community Health Centers LGBT Primary Care Task Force, nominated to the Michigan Health Information Network (MiHIN) Operations Advisory Committee. Dennis E. Upon graduation, Pyper’s passion for community health nursing led her to a small rural health center in the Mississippi Delta, where she served as nurse and medical clinic administrator for 13 years. Pyper joined Cherry Health in 2007 as the nursing supervisor at Belknap Commons Health Center after moving back to Michigan to be near family. She was later promoted to site manager of Cherry Health’s Burton Community Health Center and in January 2016 assumed the role of Director of Health Center Programs. Jodi is passionate about serving the underserved population and is dedicated to leading and inspiring staff to ensure a culture of excellence, quality outcomes, and collaborative practice with patient centered care.

Larry Reynolds, MD, FAAPPresident/CEO, Mott Children’s Health [email protected]. Reynolds was appointed to the MI Governor’s Flint Water Advisory Task Force in 2015; in 2016, he was appointed by the Governor to the Flint Water Interagency Coordinating Committee to help oversee the implementation of necessary steps for the resolution of the Flint water issues. He served as President of the Genesee County Medical Society for 18 months through December 2005. He is also past President of the Michigan Chapter of the American Academy of Pediatrics. Dr. Reynolds earned his BA from Wayne State University in 1974 and his M.D. from Howard University College of Medicine in Washington, D.C., in 1979.

P. Ann Ryan, DO, PhDFamily PractitionerIn 2011, after 16 years in rural Family Practice, Dr Ryan returned to East Lansing and MSU. From 2011 through 2014, she provided primary care at the MSU Student Health Center. In 2013, Dr Ryan joined the Physician Peer Education Project on Immunization, a collaborative effort between the CDC and the Michigan Department of Health and Human Services, coordinated through MSU Extension. As a presenter for the project, she provides live updates on vaccine preventable diseases to medical providers throughout Michigan. She currently devotes her time to this project and to her Complementary Medicine Practice in Okemos, Michigan.

Chris SheaCEO, Cherry [email protected] Chris Shea is the Chief Executive Officer of Cherry Health, a Federally Qualified Health Center based in Grand Rapids, Michigan. He has presented to diverse organizations on the integration of care including the Michigan Primary Care Association, the National Association of Community Health Centers, the National Network for Oral Health Access and the National Council for Behavioral Health.

Ken SikkemaSenior Policy Fellow, Public Sector [email protected] Sikkema is a senior policy fellow at Public Sector Consultants. His areas of concentration at PSC include public finance, environment, and energy policy. His background in state government and experience in the legislature are particularly useful to organizations seeking to move the state’s public policy agenda forward in innovative and creative ways. Prior to joining the firm, Mr. Sikkema served in the Michigan House of Representatives for six terms and in the Michigan Senate for two terms. Robert StutmanFormer Special Agent with DEA, Former Head of NYC DEA OfficeTaking to the front lines of the drug wars, Bob Stutman made a 25-year career as one of America’s highest profile drug busters, five of which he worked undercover. As head of DEA’s largest office, he became so visible the Columbian Cartel had at one time targeted him for assassination. Bob has appeared on hundreds of television shows including Oprah, and is a drug consultant for both CBS and PBS News. His autobiography, Dead on Delivery, was published by Warner Books and was the basis for the television movie Mob Justice. He has been a drug consultant on a number of films including Traffic, Training

Day, Blow and American Gangster. He was recently featured in the PBS Frontline documentary entitled Drug Wars, the VH-1 special entitled The Drug Years and the VH-1 documentary, Planet Rock: The Story of Hip Hop and the Crack Generation. Bob has been named Outstanding Speaker of the Year by numerous associations.

Judge Jodi Debbrecht SwitalskiAppointed to her judicial seat by the Governor of the State of Michigan, Judge Jodi Debbrecht Switalski has witnessed firsthand the devastating effects of interpersonal violence, substance abuse, and dependency within our families, our children and our communities. For over 25 years, she has been on the front lines of mid-America pioneering an early state response to synthetic drugs and the opioid epidemic, orchestrating town hall forums and seminars in schools and private institutions, reaching out to judges, correction officers, medical professionals and more. Judge Jodi co-founded the Regional Anti-Drug Education and Outreach organization and founded a regional veteran’s treatment court in the State of Michigan. As senior associate with the Stutman Group, Judge Jodi speaks frequently to thousands of students, parents, medical professionals and more regarding the devastating drug epidemic our country faces today.

Patty Wagenhofer-RuckerIntegrated Health Director, Genesee Community Health [email protected] spending 15 years providing direct clinical services to individuals with developmental disabilities and co-occurring mental health and addiction disorders, Patty Wagenhofer Rucker has spent the last 10 years creating innovative new programs and strategies for practitioners in an integrated primary care, addiction and mental health system of care. She is currently the Integrated Health Director at Genesee Community Health Center. Her current projects include focus on the following areas: a) reducing avoidable ED visits, b) expanding the role of recovery coaches/community health workers, c) increasing the utilization rates of women who are screened for breast and cervical cancer, and d) increasing availability of medication assisted treatment (Vivitrol and Suboxone) for individuals who are opiate and/or alcohol dependent.

Welter, DrPHClinical Assistant Professor of Community Health SciencesUniversity of Illinois at ChicagoDr. Christina Welter is a clinical assistant professor of community health sciences of the UIC School of Public Health and director of the MidAmerica Center for Public Health Practice. She also is associate director of the school’s DrPH Program in Leadership and an IHRP Fellow. She leads the training core of the Illinois Prevention Research Center,

Shortly after the tragedies of 9/11, her career took a major detour into the world of emergency preparedness and response, and focused on helping Health Centers figure out where they fit into this newly defined arena and advocated for Health Centers to be accepted as equitable response agencies alongside public health, hospitals, emergency management systems and long-term care facilities.
