Central Arizona College STUDENT RESPONSIBILITY STATEMENT

In order to meet the requirements of Section II. G. of the Agreement for Clinical Practicum

and/or Internship/Externship experience, STUDENTS shall read the following and indicate their

understanding by signing below. This Student Responsibility Statement is in addition to the

course syllabus, and the student is responsible for being familiar with the content of both

documents.

In consideration of the opportunity to enter into a clinical education/internship/externship

experience, I agree to:

A. Complete and be responsible for the cost of providing all health forms, documents

certificates, immunizations, background checks, drug testing, etc. requested by the

FACILITY.

B. Provide the COLLEGE and FACILITY with written confirmation of malpractice

insurance coverage for the term of the clinical/internship/externship assignment if

required.

C. At all times conduct myself, both at the FACILITY and outside normal business

hours, in a personally and professionally ethical manner.

D. Agree to comply with all applicable Federal, State, and local laws, including those

prohibiting discrimination. Follow the policies, rules and regulations of FACILITY,

including those regarding confidentiality of protected health information or other

confidential information pertaining to client and patient records.

E. Secure my own room and board during my clinical/internship/externship assignment.

F. Conform in my attire and appearance to the accepted standard of the COLLEGE and

the FACILITY, and procure the appropriate and necessary attire required, if any, but

not provided by the COLLEGE and the FACILITY.

G. Procure the appropriate and necessary equipment required as identified by the

COLLEGE, if any, but not provided by the COLLEGE and the FACILITY.

H. Provide my own transportation to and from the FACILITY or to and from any related

illnesses, in consultation with my academic coordinator and clinical or

Dear Potential Preceptor,  You are receiving this letter from a Dietetic Technician student at Central Arizona College (CAC). The student is in the process of identifying a facility and a Preceptor for his/her required supervised practice/internship. The Dietetic Education Program has been offering the distance education Dietetic Technician AAS Degree www.centralaz.edu/nutrition/DTAAS since 1978. This program is accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) www.eatright.org/ACEND.  CAC offers the 16‐week Dietetic Technician Internship course two times per year beginning in August or January.  The intern will complete three (3) projects including an introductory week: 384 hours under your supervision.  Interns typically are on‐site 3 days/24 hours per week.  

Internship Projects – must be completed in this order Food Service Management (5 weeks)  120 hours Education and Training (4 weeks)  96 hours Nutrition Care Process &  Medical Nutrition Therapy (7 weeks)  168 hours

Orientation and Training Requirement for Preceptors 2017 Standard 7.2 ‐ Accreditation Council for Education in Nutrition and Dietetics (ACEND) 

 “7.3a. New program faculty members, instructors, preceptors and teaching assistants must be provided orientation to the mission, goals, objectives and educational philosophy of the nutrition and dietetics technician program.  7.3b. Program faculty members, instructors, teaching assistants and preceptors must receive feedback, and training as needed, based on program evaluation and input from students.  7.3c. Program faculty members, instructors and teaching assistants and preceptors must receive training on the ACEND Standards and required knowledge and competencies.”  CAC will provide all Preceptors with an orientation to the Dietetic Technician Internship.  One hour of CPEU is available for participating in the orientation.  The Preceptor will be contacted by the Dietetic Program Director to coordinate the orientation.  

Continuing Education Requirement for Preceptors 2017 Standard 7 Accreditation Council for Education in Nutrition and Dietetics (ACEND) 

 “7.2b Preceptors must have the education and experience needed to provide appropriate guidance for supervised practice experiences. Preceptors must be licensed, as appropriate to meet state and federal regulations, or credentialed, as needed, for the area in which they are supervising students.  7.2c Program Faculty, including the Program Director, and Preceptors must show evidence of continued competence appropriate to their teaching or precepting responsibilities through professional work, graduate education, continuing education, scholarship/research or other activities leading to professional growth in the advancement of their profession.”  In the table below, please list two to four professional development activities that you have participated in within the last five (5) years. Note how these activities relate to Food Service & Management (FSM), Education & Training (EDU) and/or Nutrition Care Process & Medical Nutrition Therapy (NCP & MNT).

By signing this Memorandum of Understanding, I agree to fulfill the expectations of serving as a Preceptor for the Central Arizona College Dietetic Technician student named above:  As the Preceptor, I agree to: 1. Maintain my status as a Registered Dietitian Nutritionist, Nutrition and Dietetics Technician, 

Registered or Certified Dietary Manager throughout the term of the internship.  2. Make reasonable effort to continue to be an employee at the facility where the intern will

Dietetic Education Program and the intern. 4. Use CAC’s Dietetic Education Program approved internship materials. 5. Communicate assessment and evaluative information including the intern's performance with 

patients, clients, staff and peers to both the intern and the CAC Internship Instructor on a regular basis. A preceptor orientation will be provided.  

6. Provide up to 384 hours of supervised practice according to the schedule established with the intern and CAC Internship Instructor. 

7. Provide guidance and instruction for supervised practice learning experiences and guide the intern as he/she develops the competencies required and completes assignments.  

8. Assist the intern in problem solving and conflict resolution as needed.  9. Abide by the Code of Ethics for the Profession of Dietetics.  

10. Follow the FERPA (Family Educational Rights and Privacy Act which provides procedures for the confidentiality of academic records. College faculty and staff may release information to each other on a “need to know” basis. Other interested parties (parents, guardians, employers) may NOT have access to academic records unless the student gives written permission.  

11. Adhere to the 2017 Accreditation Council for Education in Nutrition and Dietetics (ACEND) Standards for Nutrition and Dietetics Technician Program (DT), Standard 10.2e Requirement that students doing supervised practice must not be used to replace employees.

This form is fillable  The Dietetic Education Program at Central Arizona College (CAC) is seeking placement for a Dietetic Technician Intern at your facility.  CAC and the Accreditation Council for Education in Nutrition and Dietetics (ACEND) require an agreement for clinical practicum and/or internship/externship education experiences.  CAC carries one million dollars in malpractice insurance for each intern. The intern is financially responsible to comply with all of the facility requirements with regards to drug testing, background checks, immunizations, etc.

What are Rubrics?

The Accreditation Council for Education in Nutrition and Dietetics (ACEND) - requires that Dietetic Technician's complete specific competencies. At CAC competencies are demonstrated as the intern completes three projects. Competencies can be described as a common understanding of the essential skills and knowledge that are regarded as necessary to work as a Dietetic Technician. The intent is that the individual should have the level of competence expected of someone prepared to begin a career as a dietetic technician.

Therefore, your project rubrics are essentially a "scoring tool" that your Preceptor will use in your internship to evaluate your skill and knowledge of the required competencies.

Attached are three Project Rubrics your Preceptor will use to evaluate your competencies during NTR 296.

Read over thoroughly - these are for your reference. The three projects are:

• Food Service and Management 135 hours over 5 weeks• Education and Training - 108 hours over 4 weeks• Nutrition Care Process/Medical Nutrition Therapy - 189 hours over 7 weeks

Your Preceptor will have the above rubrics and will email them to the NTR 296 instructor at the end of each of the projects.
