Eligibility
• You
o Must regularly work at least 30 hours per week
• Your spouse and dependent children
• Your domestic partner and their dependent children
o All benefits except life insurance
o Must complete a Domestic Partner Affidavit, which can be found on mercermarketplace.com/SYKES
Note: If you regularly work fewer than 30 hours per week, you may be eligible for the Transamerica medical plans and some Aflac options. SYKES does not contribute to your coverage.

What’s New for the 2016 Benefits Plan Year
• Great news! This year is a passive benefits enrollment for the 2016 plan year
o If you don’t want to make any changes to your SYKES plan elections, dependents, or beneficiaries from the 2015 plan year, then you don’t need to take any action for most plans. However, you must re-enroll annually into spending accounts (Flexible Spending and Health Savings Account).
Your elections, dependents, and beneficiaries from the previous plan year will carry over.
Please note: There could be changes to the rates of which you’ll need to be aware. To check premiums, please see the rate sheets, go online to the Mercer Marketplace (mercermarketplace.com/SYKES) or contact your HR representative.
o In order to enroll, make changes to plan enrollments or to check premiums, you’ll need to take the steps as noted in this presentation and in the enrollment materials.
• There is more good news!
o Only small changes to the plans and plan design – from last year to this year.

• You must enroll either during the Annual Enrollment period (Oct. 22 – Nov. 5), or if you are a new hire, during your benefits eligibility waiting period.
• Coverage takes effect:
– January 1st for Annual Enrollment
– New hires become effective the first of the month after 60 days from the end of your 30-day Orientation Period, but in no case longer than 120 days from your hire date.

Returning Enrollees to Mercer Marketplace:
• Username: Use the username you created in the past. If you have forgotten your username, please contact Mercer Marketplace at 1-855-543-4322.
• Password: Your password will be reset to your last name and the last four digits of your Social Security number (Example: Jones1234).
First-Time Enrollees to Mercer Marketplace:
• Username and Password: You’ll need your Social Security number, last name, and date of birth to identify yourself and then you will be prompted to select a unique username and password that you will use going forward.
• If you have any issues logging in, you can also click on the ‘Can’t access my account’ link or you may call the Mercer Marketplace Benefit Center at 1-855-543-4322.

Smart Ways to Save Money
• Here are two easy ways you can save moneyon healthcare expenses:
1. In-network vs. out-of-network: Before you visit a doctor, dentist, provider, or facility, make sure that the person or facility is in-network, meaning he or she (or it) is part of your health plan. If you see an out-of-network provider, your costs will be higher. You can check on this by calling the provider’s office, or by looking online. Usually, it’s safest to call.
2. Generic medications: Make sure that when you visit the pharmacy or call mail-order for your (or your family’s prescriptions), you ask for the generic equivalent of the medication. Sometimes, however, generic will not be available.
