What Is Legionella ?The Legionella organism is a Gram negative, rod shaped bacterium that can cause pneumonia (Legionnaires’ Disease) or a flu-like illness (Pontiac fever). It was first identified and recognized as causing disease during the outbreak that occurred in conjunction with the American Legion Convention in Philadelphia in 1976. There are over 40 species of Legionella and 18 of those can cause disease. It is estimated that the species Legionella pneumophila causes most of the infections. Legionella is a fastidious organism, meaning it has specific growth requirements that need to be met in order for it to survive and grow. Some of these growth requirements are: • Temperatureabove68°F • Iron • L-Cysteine • Biofilm(particularlyprotozoans)Unlikesomeotherbacteria,itcansurviveatlowerdissolvedoxygenlevelsandissomewhat resistant to chlorine disinfection. Certain plastics and organics can provide nutrientsforgrowth.Theseattributesmakeourmoderndayplumbingsystemsagood habitat for the organism. Legionella Overview CommoninfectionscausedbyLegionella are Legionnaires’ Disease (LD), a severe pneumonia,orPontiacFever,aflu-likeillness.Legionnaires’Diseaseismostcom-monlylinkedtoexposuretoLegionella pneumophila. However other species (i.e., L. micdadei, L, longbeachae) can cause LD as well. L. pneumophilahasmanysubgroupscalledserotypes.L. pneumophilaserotypes1,3,5,and6havebeenthecausativeagentsofLegionnaires’Disease.Co-infectionswithdifferentspeciesand/orserotypeshave occurred. Ecology Legionellaarecommonlyfoundinaquaticenvironmentsandsomespecieshavebeenfound in soil. The organisms are found in a wide range of environmental conditions andarerelativelyresistanttolowpH,dissolvedoxygenlevels,androutinechlori-nationtechniquesfordrinkingwater.Temperaturesabove104°Fpromoterapidmultiplicationoftheorganism.Theorganismsareconsistentlyfoundinthebiofilmthatformsinaquaticenvironments,coolingtowersandpotablewatersystems.

Epidemiology The risk factors for legionellosis are people who are immunocompromised by an un-derlying medical condition, those taking immunosuppresive drugs, heavy smokers, those who have chronic lung conditions, and the elderly. Several studies have docu-mented 76 cases of pediatric legionellosis in children under 1 year of age or children with underlying medical conditions such as malignancy or immunosuppression. Legionellosis is not contagious; there is no evidence that the disease can be transmit-ted from person to person. Exposure must be through inhalation or aspiration of contaminated, aerosolized water. Once a person has Legionnaires’ disease, getting it a second time is extremely rare.

Monitoring Guidelines The US Centers for Disease Control and Prevention (CDC) recommends rou-tine monitoring for Legionella in all bone marrow and organ transplant hospitals nationwide.RoutinemonitoringinhealthcarefacilitiesisrecommendedorrequiredinseveralstatessuchasNY,TX,MD,LosAngelesCountyandAlleghenyCountyPA.Canadahasguidelinesformonitoringhealthcarefacilities.TheAmericanSocietyof Heating, Refrigeration and Air Conditioning Engineers (ASHRAE) recommends routinemonitoringofbuildingwatersupplysystems.

Sampling And Analysis Takea1000mLsampleforpotablewateranda250mLsamplefornon-potablewater.Besuretousesterilebottleswithachlorineneutralizingagent.Sincebiofilmsare the actual reservoirs for the bacteria it is also recommended to take sterile swab samples of biofilm in areas where it is present. Samples should be shipped overnight tothelabonfreezerpacks.CulturableanalysiseitherbytheUSCenterforDiseaseControl or the International Standard Organization is the “gold standard” and re-quires10-14days.TestingbyPolymeraseChainReaction(PCR)takes2-3daysandmaybeveryusefulforprovidingfast,presumptiveresultstoreduceliabilityduringanoutbreak.TestingbyNextGenerationDNASequencingcanprovideevidencethat links an environmental isolate to a clinical isolate. Isolating Legionella from environmentalsamplesisdifficult.MakesuretouseanexperiencedlabthatiseitherCDCELITEorHPA(UnitedKingdom)proficient.

An Overview of Legionella AnalysesBackground The first recognized outbreak of Legionnnaires’ Disease occurred in the US at the American Legion Convention in Philadelphia during the summer of 1976. There wereseveralhundredpeoplewhowerestricken.Thirtyfourpeoplediedfromthedisease. As a result of the efforts of the US Centers for Disease Control and Preven-tion (CDC), this was the first time the bacteria was cultured and identified. Earlier outbreaksofthediseasewentundiagnosed.Sincethattime,therehavebeenmanyidentifiedoutbreaksinthiscountryandabroadpromptingprofessionalorganizationsand health departments worldwide to implement guidelines for diagnosing and reporting the disease, and monitoring the organism. Transmission and Epidemiology Ubiquitousinallaquaticenvironments,Legionella bacteria are found in groundwater aswellasfreshandmarinesurfacewaters.Thebacteriaenterourplumbingsystems,whirlpool spas, and cooling towers via these water sources. Unless control measures areconductedproperlyandroutinely,thebiofilm,scale,andcorrosionthatbuildsupovertimeinthesesystemswillprotecttheorganismandallowittomultiply.Contaminated aerosolized water from cooling towers, whirlpool baths, nebulizers, faucets, and showerheads becomes airborne. When a susceptible host inhales the con-taminated aerosol, legionellosis can occur. Aspiration of the contaminated water can alsocausethedisease.Legionellosiscancausetwotypesofillness:1.asevereformofpneumonia(Legionnaires’Disease)oftenaccompaniedbyseriouslongtermhealtheffects,and2.amildflu-likeillnesscalledPontiacFever.Otherinfectedorgans,andasymptomaticinfectionsmayalsooccur.Historically,riskfactorsforgettingthediseaseincludedage,gender,compromisedimmunesystems,andpre-existingmedicalconditionssuchaschronicobstructivepulmonarydisease,cancer,anddiabetes.Menover65yearsofagewhowereheavysmokers and drinkers were identified as being at greatest risk. While that is still true, recentresearchfromNeilandBerkelmanatEmoryUniversityhasidentifiedanabrupt increase in the incidence of Legionnaires’ Disease in the US in all age groups inthelast20years.Thistrendhasalsobeennotedinternationallybyotherresearch-ers.Theyhavenotedanoverallincreaseinthediseaseamongallpeopleaged45to64. Rates of disease in males still exceed the rates in females.

Therehavealsobeencasesofthediseaseinhealthy,youngerpeople.Pre-mature,immuno-compromised,orventilatedneonatesareatriskfromhospital-acquiredinfection.Inaddition,caseshavebeenreportedinchildrenaged15-19yearsold.Although the disease is under-reported, travel (cruise ships), hotel, and resort relatedoutbreaksarereportedeachyear.Thesearemostlyassociatedwiththeuseofwhirlpoolspasandstagnantpotablewater.Whilecommunity-acquiredoutbreaksinvolving cooling towers and whirlpool spas receive the most media attention, studies indicate that building potable water sources account for most of the infections. This isparticularlytrueinhospitalsandnursinghomeswheretherearelargenumbersofimmunosuppressedorcritically-illpeople.Forthesereasons,manystatehealthdepartments have guidelines that recommend routine monitoring for Legionella in critical-carehospitalsandnursinghomes.In2008,theVeteran’sAdministrationpromulgatedadirectivewhichrequiresallVAhospitalsandrehabilitationcenters toimplementmonitoringforthebacteriaintheirpotablewatersystems. Choosing Sampling Methods Propermethodsforcollectingandanalyzingsamplesarenecessarytoensuredefensi-bleresults.Sincethebacteriainwaterarepresentinverylowlevels,1000mLpotablewatersamplesarerecommendedbytheUSCentersforDiseaseControlandPreven-tion (CDC). This sample size allows for the bacteria in the water to be concentrated, allowingforbetterdetectioninpotablewatersamples.Manyprofessionalguidelinesrecommend semi-annual sampling for potable water sources.

Innon-potablewatersourcessuchascoolingtowerwater,a250mLsamplesizeissufficient.Professionalguidelinessuggestthesesourcesbemonitoredquarterly.

Samplingshouldbeconductedinawaythatmaximizesrecoveryoftheorganism.Legionellasamplesshouldbecollectedwhereverwateraerosolizationmayoccur.

Samplingaerosolizedwateralone,however,willlikelymisstherealsourceoftheorganism. This source is the biofilm or slime that is often found in our plumbing systems,coolingtowers,andwhirlpoolbaths.

Where to Look for LegionellaWhenconductingyourbuildingwatersysteminvestigation,walk-through,orriskassessmentusethissectiontohelpyouidentifyallpotentialLegionella reservoirs in the building.Ifconductinganinvestigationduetoasuspectedcaseoroutbreak,alsosurveythesurroundingneighborhoodtoidentifyanycoolingtowers,wastewatertreatmentfacilities,stormwater/graywaterre-usefacilities(i.e.golfcoursesprayirrigationwatersystems),orornamentalfountainsthatmaybelocatednearthebuildinginquestion.Ifnecessary,obtainpermissiontosampletheseoff-the-propertylocations.Alsoidentifyandvisuallyinspectallthebuildingfresh-airintakes/pedestrianwalkwayswithrespecttotheseneighborhood locations. 1. Legionella Sampling LocationsSamplingisthesinglemostimportantactivityforeveryproject.Thepurposeofsamplingistoidentifythegoalsoftheproject,toanswerspecificquestions,ortodevelopahypothesistotest.Whileitisnormalforclientstobeupsetwhentheyencounterasuspectedcaseoranoutbreak, avoid taking random samples without understanding how the data will be used and whocouldultimatelyseetheresultsfirst.Herearesomekeyquestionstoconsiderthatwillhelpyoufocusyoursamplingefforts: 1. Whataretheappropriatetestmethodsandthelevelofdetailneededforyourproject? (See test codes) 2. Wheredoyouneedtosample?Arethereparticulartypesofequipmentordevicesthat needtobesampled? 3. Whendoyouneedtosample?Considertheremaybetimestotakecoolingtowersamples inthesummerandearlyfalltoaccountforworstcasescenariosratherthantakingstrictly periodic samples. 4. Sampling Instructions - Legionella1.Personalsafetyandprecautionsshouldbeobservedduringsampling.AvoidbreathingaerosolsthatmaybecontaminatedwithLegionella bacteria. Avoid generating aerosols or watermistsduringsamplingofthewatersystem.WeararespiratorequippedwithaHEPAcartridge, goggles, and sterile nitrile gloves.2.Prepareorobtainsterile,screw-cappedplasticbottlesforsampling.Sodiumthiosulfateisroutinelyaddedtothebottleasapreservativeandhalogen(chlorineorbromine)-neutraliz-ing agent.

Recommended sample size:

Potable Water Sampling Non-Potable Water Sampling

1 Liter Water Sample 250 mL Water Sample 3.Fordrinkingorpotablewater,suchaswaterfountains,faucets,andshowerheads,collecttwosamplesifpossible.Collectthe“pre-flushorfirstdraw”samplebydrainingthefirst1000mLofwaterfromthefaucetsorflushdrainsintoabottle.Allowthewatertorunforapproximately60seconds and collect the second draw of 1000 mL of water. Leave a one-inch space on top of the water sample. The second sample is also called “post-flush or second draw” sample.4.Whensamplingfaucetaeratorsandshowerheads,removetheaeratorsasceptically.Takeswabsoftheinsidethefaucetandshowerheadsasfarasyoucanreachwiththeswab.Swirltheswabon the inside of the pipe three times. Your swabbing procedure should be consistent between sampling locations. When sampling cooling towers, whirlpool spas or fountains, look for areas of biofilm and take a swab sample of the biofilm. We will provide sterile swabs for this purpose.5.Fornon-drinkingornon-potablesamplesfromsuchsourcesascoolingtowers,chillers,con-densatepans,surfacewaterinreservoirs,sprinklers,etc.,collect250mLwaterfromthebottomor side of the vessel or reservoir. If taking a cooling tower sample, also consider taking a sample inthepackcolumn.Leaveaoneinchspaceontopofthesample.Recordanybiocideusedin water treatment when collecting non-drinking water. If sampling whirlpool spas, consider takingaswabsampleofanybiofilmaswellasasampleofthesandfilter.6. Label sample number on the bottle and record on the sample data sheet. Use a distinctive numberforeachsample.Completeallsampleinformationonasampledatasheetforyourownrecord.Sendacopywiththesamplestothelaboratory.7.Tightlycapthebottles.Makesurethatwaterdoesnotleakduringshippingandtransporting.Tapingofbottlearoundthecapandneckwithelectricvinyltapeisrecommended.Placetapedbottles in a clean plastic bag.

Emergency RemediationRemediating Legionella after a case or an outbreak from potable and non-potable waterisnoeasymatterandmayrequireseveralattemptsbeforethetreatmentissuccessfulforachievingnon-detectableresults.UnfortunatelyduetotheecologyofLegionellaandthenatureofthetreatmentsystems,therearenopermanentsolutions.The best outcome will be to reduce and control the biofilm buildup where the bacte-riaresideandmultiply.Theproperdesign,maintenance,andtemperatureofapotablewatersystemisthefirst defense for preventing the amplification of Legionella.Maintaininghotwaterabove135degreesFahrenheit(57°C)andcoldwaterlessthan68degreesFahrenheit(20°C)andeliminatingdeadlegsorlowflowareasgoesalongwayforprevention.Howeverthisisnotalwaysfeasible.Thereareseveralproceduresthatcanbetakenforemergencyremediation,orroutinetreatmentofapotablewatersystem.However,itneedstobeunderstoodthatthesearetemporarysolutionssincethebacteriawillreboundwithinafewweeks.Thesecleaningprotocolsarelistedbelow:Heattreatmentofhotwatertanksandthecompletewatersystemincludesraisingthetemperatureofthesystemto157degreesFahrenheit(69°C).Thistemperatureneedstobemaintainedinthetankfor3hours.Thehotwaterneedstobedrawnthroughalltheoutletsat157degreesFahrenheit(69°C)startingwiththeoutletsclosesttothehotwatertank.Sequentiallyworkawayfromthehotwatertankdrawingwaterat157degreesFahrenheit(69°C)toalltheoutletsatatrickleflowratefor3hours.Afterthisiscompleted,flushthehotwaterfromthesystemandreturnittonormaloperatingconditions.Ifthecapacityofthesystemistoolowforheattreatment,thenchlorina-tion should be used.Chlorinationisaccomplishedbydrainingthehotwatertankandmanuallycleaningitofdebris.Removealldepositsbyscrapingfollowedbywetvacuumingofthetank.through all the outlets including deadlegs and low use points and risers. Soak at this level

offreechlorinefor16hours.Testtoensurethataminimumof30ppmfreechlorineisobtained at all outlet points for the 16 hours. After this has been accomplished, flush thechlorinatedwaterfromthesystemandtesttoensurethatlessthan2ppmchlorineremainsinthesystem.According to the International Plumbing Code, this chlorination needs to be conducted forallnewsystems.Chlorinationshouldbeconductedafter:majoralterations,asystemtests postive for Legionella, a major outtage, a water main break, the municipal water systemhasbeenflushed.Continuous,proper,routinemaintenanceandtreatmentistheonlywaytopreventtheamplification of Legionella.Severalstudieshaveindicatedthatroutinetestingofapotablewatersystemwillidentifyapotentialrisk.Therefore,thegoalistoestablishahistoryofnon-detectableresults over time.Remediating cooling towers is successful as long as the treatment is conducted in conjunctionwithanongoingmaintenanceprogramofthetower.Currently,thecleaningprotocolsforcoolingtowerscanbefoundintheAmericanSocietyofRefrigeration, Air Conditioning and Heating Engineers (ASHRAE), the Wisconsin EmergencyProtocol(laterwithdrawnbyWisconsinbuttherevisedversioncanstillbe found in OSHA LegionellaTechnicalDocument),andUKHealthandSafetyExecutiveDirectiveforWaterTreatmentHS(G)70.Theseprotocolsemphasizetheneedforroutinemaintenance,inspection,manualcleaningofsystemcomponentsandwatertreatmentbyprofessionals.Minimizingbiofilm,scale,corrosion,algalgrowth, and sediment accumulation in the cooling tower components are critical for preventing amplification of Legionella. Chlorinationisthemosteffectivemethodforemergencycleaningbutexcessivechlorina-tionwillreducethelifeofthesystemcomponents.Depositsandbiofilmreducetheeffica-cyofchlorination.RoutinecleaningandchlorinationwillreducethepresenceofLegionella but it is not permanent solution. The organisms will re-grow in a few weeks.

Unlessyouchangeyourcoolingtowermaintenanceandtreatmentprogram,Legionella will re-appear.Anoverallmaintenanceprogramshouldincluderoutineshutdownofthesystemtomanuallycleanandflushthesystemcomponents.Treatmentshouldincludetheuseofcorrosion inhibitors, biodispersants (chemicals used to breakdown biofilm) and oxidizing and non-oxidizing biocides. Non-oxidizing biocide treatment will penetrate the biofilm ac-cumulation on the tower components. Since Legionellamultiplyintheprotozoanswithinthe biofilm, controlling the biofilm is critical to controlling Legionella. The non-oxidizing biocidesneedtoberotatedfrequentlytoeliminatethedevelopmentofresistantbacteria.

Improperlymaintainedhottubsandwhirlpoolspasareincreasinglybeingassociatedwithlegionellosis. The Association of Pool and Spa Professionals has comprehensive guidelines for maintenance that are available for purchase (www.apsp.org). Considerations for the propermaintenanceofthesefeaturesincludethefollowing:Single-usesystemsshouldbecompletelydrainedbetweenuseandstoreddry.Nonsingle-usesystemsshouldbetreateddailyandcleanedweekly.Theseshouldbecleaned,treatedandstoreddryattheendoftheseason.Watertreatmentandfiltrationisessentialwhetherthesesystemsusepotableorsaltwater.Heavybatherloadwillincreasetheneedforcleaning,treatment,andfiltration.OtherorganismsthatcancausediseaseinthesesystemsincludeMycobacterium avium, Pseudomonas aeruginosa, Staphylococcus aureus, Naegleria fowleri and organisms that are associatedwithfecalcontaminationcausedbychildrenindiapers.

Insummary,alltheremediationprotocolsmentionedhereareforashortterm,immediateresponse;theyshouldnotbeconsideredaspermanentsolutions.Ifyoureturntooperatingyourpotableandnon-potablewatersystemsasbefore,theproblemwillreturn.Continu-ous,proper,routinemaintenanceandtreatmentistheonlywaytopreventtheamplifica-tion of Legionella.

What Are Cryptosporidium and GiardiaCryptosporidium and Giardia are waterborne pathogens which cause the intestinal illnessescryptosporidiosisandgiardiasis,respectively.Symptomsofinfectionincludewaterydiarrhea,stomachcrampsorpain,nausea,dehydration,vomiting,fever,andweightloss.Thesediseasesaregenerallytransmittedbyeatingcontaminatedfood,drinking contaminated water, and swallowing contaminated water while swimming or bathing. Cryptosporidium Overview Cryptosporidiumiscommonlyknownas“Crypto”andwasfirstdescribedbyE.E.Tyzzerin1907.TherearemanyspeciesofCryptosporidium that infect humans and animals.Theparasiteisprotectedbyanoutershellthatallowsittosurviveoutsidethebodyforlongperiodsoftimeandmakesitverytoleranttochlorinedisinfection.Whilethisparasitecanbespreadinseveraldifferentways,water(drinkingwaterandrecreational water) is the most common method of transmission. Finished Water • Samplingafterthefiltrationsystemandpriortochlorineaddition,ifpossible. • Minimum1000liters(264gallons).Collectionperiodshouldencompassafull cyclerun,orfor24hours,includingatleastonebackwashcycle.Backwash cyclescanoccurattheinitiationofthesamplingperiod.

Chlorinated Samples Ifchlorinatedwatermustbesampled,aninjectorsystemwillneedtobeinstalled to add a sodium thiosulfate solution to denature the chlorine. Add sodium thio sulfatesolutionviatheinjectorsystemtoproduceafinalconcentrationof50 mg/L.Settingtheinjectorsystemtoproducea1:100dilutionof0.5%sodium thiosulfatestocksolutionwillresultinafinalconcentrationof50mg/L.

Treatment Plant Evaluation The raw water sampling should be initiated before the finished water sampling. The amount of time elapsed between the beginning of raw sampling and the beginningoffinishedsamplingshouldbeequivalenttothedetentiontimeof thesystem.

Sampling - For Pressurized Water Sources1. Connectthefieldfiltrationunit(Fig.3),withoutthecartridgefilter,tothewatersource.Flushtheunitfor3-5minuteswiththesourcewatertobesampled.Ifcollectingsamplesfrommultiplelocations,runaminimumof50gallonsofsamplewaterthroughthesamplingequipmentbetweensamplestoavoidcrosscontam-ination.Adjusttheflowrateto1gpm(3.8lpm)usingflowcontrolvalve.Adjustpressure gauge to 10 psi using pressure regulator. Adjust both flow control valve and pressure gauge so that 1gpm flow rate and 10psi pressure are maintained. The watermeterfunctionsasbothatotalizerandflowratemeter.Alternatefunctionsbypressingthe“DISPLAY”button.Selectthedesiredunits(gallon(GL),liter(LT),orcubicfoot(CF))forthetotalizerbypressing“DISPLAY”when“CALIBRATION”ispressed at the same time.

Sampling - For Non-Pressurized Water Sources

1. Addapumpbetweenfilterandwatermeter.Besuretoputthesampleintakeina location where the least amount of bottom sediment will enter into the sampling filter giving a distorted view of the sample. Adjust flow rate and pressure gauge as in Section3.1.Note:Collectsampleasneartointakesiteaspossible.

2.Insertfilterintothehousingandhandtightenhousing.Makesureprovidedclamps are in place and tight at both inlet and outlet of the housing. Do not touch thefilterwithbarehands,usesanitaryglovesortheplasticcoverthefilteriswrappedin.Turnwateronslowlywiththeunitinanuprightposition.Invertunittomakesurealltheairwithinthehousingisexpelled.Recordthedate,timeofday,andgallon reading from the water meter before sampling.

3.Whenthefilterhousingisfullofwater,returnunittouprightposition,maintaintheflowrateat1gpm(3.8lpm)andpressureat10psithroughoutthesamplingperiod.

4.Thesamplingunitshouldbeallowedtorunforan8-24hourperiodaccordingtothesamplingparametersinSection2.0.

5.Afterfilteringsampleturnoffthefaucetorpumpanddisconnectthehosefromincomingwatersource.Maintaintheinlethoselevelabovelevelofopeningon outlet hose to prevent backwashing and loss of particulate matter from the filter. PourresidualwaterfromfilterhousingintoaZiplocbag.

6.Removethesamplingcartridgewiththeplasticcoverorsanitarylatexgloves. Donottouchwithbarehands.Placefilterinasecondheavydutyqualityziploc(whirlpak) bag and seal.

7. Record stop time and final meter reading. Subtract the initial reading from the final reading and record the total volume collected. Document the name and loca-tionofeachsamplepoint,samplingsite(raworfinished)andtypeoftreatment.

8. With permanent marker record the sample identification, gallons sampled, collectiondatesandtimes,collector’snameandwaterqualityparametersdirectlyonthe bag or on a waterproof label. Double pack the bags containing filter and residual waterwithziplocbags.Makesureallbagsaresealedtopreventleakage.

9 .Ifimmediateshippingisnotpossible,thesampleshouldbestoredina1-5°Crefrigerator, until shipping.

10. Place freezer cold packs in the shipping container. Place insulating material be-tween the filter and cold packs to prevent sample freezing. Samples that arrive at the laboratoryfrozenwillberejected.Placedatasheetcontainingrecordedinformationin a sealed plastic bag and ship with the filters.
