Reactions 1444 - 23 Mar 2013 O S Amitriptyline/citalopram/venlafaxine overdose Refractory haemodynamic instability: case report A 53-year-old woman experienced haemodynamic instability after an overdose of amitriptyline, citalopram and venlafaxine. The woman had previously attempted suicide, and was receiving venlafaxine [dosage and route not stated] and lorazepam for depressive disorder. Her husband brought her to an Emergency Room after he found her with a reduced level of consciousness; she had superficial cuts to her left wrist and her Glasgow Coma Score was 3. Her respiratory rate was 8 breaths/minute and arterial blood gas sampling indicated respiratory acidosis. Urine analysis identified benzodiazepines and tricyclic antidepressants. Acute antidepressant poisoning was considered the most likely diagnosis. The woman was intubated and received haemodynamic resuscitation. Her HR dropped to 35 beats/minute despite receiving isoprenaline; normal sinus rhythm was returned following administration of lipid emulsion. Toxicological analysis determined toxic levels of amitriptyline, citalopram [dosages and routes not stated] and venlafaxine. She was transferred to a ward after 10 days in an ICU. Author comment: "In this case, we present a patient with combined tricyclic antidepressants, selective serotonin reuptake inhibitor and serotonin and noradrenaline reuptake inhibitor overdose. The haemodynamic instability caused by the antidepressants proved to be refractory to extensive ionotropic support." Scholten HJ, et al.
