Introduction
The American Chiropractic Neurology Board, Inc. (ACNB) retained the services of Shoreline
Psychometric Services, LLC. for scoring the October 2015 Diplomate of ACNB Written and
Performance Examinations and providing an analysis of the psychometric properties of the
examinations. This technical report provides a summary of the psychometric properties of
the ACNB 2015 examinations.
Taking and passing the examinations are required for obtaining the Diplomate of ACNB.
ACNB developed the Written and Performance Examinations for administration in October
2015. The 2015 Written Examination consists of 300 multiple-choice questions that was
administered in two parts (i.e., a morning session and an afternoon session), with each part
consisting of 150 test questions. The test questions consist of 4 or 5 options, with one
option identified as the correct answer. Total test score was based on the number of test
questions answered correctly. The examination was administered in October 2015 via
paper-and-pencil testing and candidates marked their responses to the test questions on
Scantron forms; a form for Part I and a form for Part II.
The 2015 Performance Examination consists of a physical examination and a case
(scenario) examination. The physical examination was administered to all candidates and
there were 9 forms of the cases (scenarios) that were assigned to candidates based on test
site location, session (morning or afternoon) and test date. The examinations were scored
using checklists that were completed by trained examiners who observed the candidates
during the performance examination. The physical examination consists of 278 checklist
items and the case (scenario) examinations consists of 7 checklist items that were
completed by two examiners per candidate.
Summary Statistics for the Written Examination
Sixty-six candidates sat for the written examination, which was administered in October
2015. The mean raw score of the written examination was 199.95 (or 66.65%) with a
standard deviation (SD) of 25.08 and standard error of measurement (SEM) of 7.48. The
raw scores ranged from 147 to 251. The pass point was 186 (raw score) and the pass rate
was 76%. Cronbach’s alpha coefficient, which is a measure of internal consistency and a
form of reliability, is .9109 (Cronbach, 1951). This coefficient is high, suggesting strong
internal consistency, and is within an acceptable range. The decision consistency estimate
(Livingston, 1972) was .9323. This estimate indicates strong decision consistency. These
statistical data are displayed in Table 1. A frequency distribution of scores is displayed in
Table 2.

Table 3 presents the following statistical summary data on Parts I and II of the Written
Examination: highest score, lowest score, median, mean, standard deviation (SD),
reliability, and the standard error of measurement (SEM). The highest and lowest scores on
Parts I and II are similar, as Part I’s scores range from 64 (42.67%) to 127 (84.67%), and
Part II’s scores range from 71 (47.33%) to 125 (83.33%). The mean scores differ by 4.54
points or by 3.03%, as the mean score of Part I is 97.70 (65.13%) and the mean score of
Part II is 102 (68.16%). The SDs for the parts are similar, as the SD for Part I is 13.77 and
the SD for Part II is 12.74. The reliability for Parts I and II are 0.85 and 0.83, respectively,
and the SEMs are 5.36 and 5.19, respectively. The data suggests that the parts of the
examination are comparable in terms of their statistical properties.

Analysis of Subcomponent Data
As shown in Tables 4 and 5, the written examination is composed of the following two
elements: 1) Work Activities and 2) Content Domains. The tables show the number and
percentage of test questions per element and the test specifications.

Since the ACNB provides candidates with sub-scores for the components comprising Work
Activities and Content Domains, it was recommended to the ACNB that a review of the
components comprising the Work Activities and Content Domains be conducted for the
purpose of determining whether some of the components are related and could be
combined for the purpose of score reporting. The reason for this recommendation was to
enhance the reliability of the data, as some of the components require a small number of
items, and the reliability of those components would be low. For the work activities, ACNB
combined Special Studies and Referral, as they are related. A review of the components
comprising the Content Domains by ACNB resulted in the following combinations:
1. Head and Face Pain/Pain (HFP/P)
2. Brain and Its Environment/Basal Ganglia/Lobes of the Brain/Limbic System
(BE/BG/LB/LS) 3. Brainstem/Cranial Nerves/Cerebellum (BS/CN/CVS)

5. Neuron Theory/Receptor Systems/Reflexogenic Systems (NT/R/RS)
6. Spinal Cord/Peripheral Nerves (SC/PN)

The combinations indicated above were used to produce score report data and are
described and presented in this section of the report. Tables 6 and 7 present the following
statistics for each subcomponent of the Work Activities and Content Domains: number of
items, minimum score, maximum score, mean (M), standard error of the mean (SE),
standard deviation (SD), variance, reliability, and the SEM.
For the Work Activities, the reliability indices for the subcomponents range from 0.24 for
History to 0.74 for Diagnosis. The SEM ranged from 2.08 for Physical Examination to 4.77
for Diagnosis. (See Table 6 for these data.) Appendix D presents the frequency
distributions of test scores by Work Activity.
For the Content Domains, the reliability indices for the components ranged from .17 for
Ethics to .71 for BE/BG/LB/LS. (See Table 7 for these data.) Appendix E presents the
frequency distributions of test scores by Content Domain.

Overview of Item Analysis Statistics
The item analysis includes difficulty and discrimination statistics. These statistics provide
information about the relative difficulty level of test questions and how well test questions
perform in relationship to total test score (i.e., discrimination). It is recommended that
item analysis statistics be used as a guide to help subject matter experts in their review of
test questions when finalizing the answer key and for future revision of the examination.
P-values indicate the difficulty level of the test questions. It is the proportion of candidates
selecting a test question’s option. P-values may range from 0.00 to 1.00. P-values are
produced for each option of a test question. As a general rule, test questions in which the p-
value for the correct answer is 0.90 or higher indicates that the majority of candidates (90%
or greater) selected the correct answer. Test questions with p-values of 0.90 or greater
are considered easy test questions. In contrast, test questions with p-values of 0.30

or lower indicate that a small number of candidates (30% or less) selected the correct
answer. Test questions with low p-values for the correct answer are considered to be
difficult test questions. Subject matter experts should review test questions with low p-
values (e.g., 0.30 or less) to rule out the possibility that there is more than one correct
answer, or that the question is mis-keyed (e.g., option “A” was keyed as the correct answer,
but the actual correct answer is option “D”). If subject matter experts rule out problems
with the correctness of the answer key, they may need to assess the clarity of the question’s
wording and/or the appropriateness of the test question for the examination (e.g., the
subject matter was accurate but the question was not appropriate for the purpose of the
examination). P-values were produced for both forms.
A discrimination statistic was produced for test questions on the written examination. The
discrimination statistic (i.e., point bi-serial correlation) shows the relationship of
performance on the test question and total test score. The statistic is “corrected item-total
correlation” because the score on the individual test question is not included in the total
score when calculating the correlation. If the test question score was included in the
calculation, the correlation would be inflated. The statistic, which is a correlation, may
range from -1.00 to +1.00. It is desirable to have correlations that are at least 0.10 or higher
(0.20 would be desirable), as the correlation indicates a positive relationship between
performance on the test question and total test score. Test questions with correlations
less than 0.10 suggest a poor or inverse relationship between performance on the
test question and total test score. Subject matter experts should review these questions
to verify their accuracy and clarity, and the accuracy of the questions’ answer keys.
These data were produced, reviewed and items with undesirable statistical properties as
described above (i.e., items with low p-values and low discrimination indices) were flagged
for review by subject matter experts to finalize the answer key. See Appendix A for the
preliminary item analysis. A complete preliminary item analysis (i.e., data for all options)
was provided to ACNB separately.
On November 12, 2015, a meeting was held via web conference to review the test questions
that were flagged for review. In advance of the meeting, the ACNB provided the committee
members with password-protected access to the examination, a description of item analysis
data and the purpose of the review (See Appendix B – Item Analysis Orientation), and
a list of items that were flagged for review. The meeting resulted in altering the
answer key to some of the questions as well as flagging questions for future review and
possibly revision. See Appendix C for the list of items that were flagged for review and the
final answer key changes for the examination. See Appendix D for the list of committee
members. The complete preliminary item analysis and the final item analysis were
provided to ACNB separately.

Summary Statistics for the Performance Examination
The performance examinations were scored using checklists that were completed by
trained examiners who observed the candidates during the performance examination. The
physical examination consisted of 278 checklist items and the case (scenario) examinations
consisted of 7 checklist items that were completed by two examiners per candidate. (See
Appendix F for a list of the physical examination and case items on the 2015 Performance
Examination.) Seventy-five (75) candidates took the performance examination.
The approach for scoring the performance examination was based information that the
ACNB provided Shoreline Psychometric Services, LLC. for use in scoring the examination.
To score the examination, the number of checked items was summed to derive the
candidates’ raw scores for the physical examination and for the case. The two sets of raw
scores (i.e., the scores resulting from the two examiners’ checklists) were averaged (i.e., an
average score for the physical examination and an average score for the case). The Physical
Examination is weighted as 50% and the Cases as 50%. The cut score that was used for the
2014 Physical Examination was applied, as that examination was not amended by the ACNB
for the 2015 test administration. Since the cases were developed for the 2015 test
administration, passing point studies were conducted in December 2015 to derive a passing
score for each case. (See the 2015 Pass Point Study Report for details.) The final cut
score for each performance examination is derived from the weighted cut score (i.e., the
physical examination pass point and the case pass point).
Overview of Item Analysis Statistics
As a result of reviewing summary data of the performance on the examination (i.e.,
percentage correct on the checklist items on the physical examination and the percentage
correct on the checklist items on the cases), the ACNB did not alter scores (i.e., akin to
altering the answer key on a written examination). See Appendix H for the Item Analysis
for the Physical Examination and Appendix I for the Item Analysis for the Cases. It should
be noted that prior to viewing summary data, ACNB instructed Shoreline Psychometric
Services, LLC. to give candidates credit on the Review & Clarification of Prescreen History
checklist item for the Cases. The reason for giving credit is that the ACNB decided that
there was sufficient information provided in the cases that would not necessitate the
candidate having to ask the examiners for clarification, as the ACNB wanted to maintain
standardization of the test administration by not having the examiners respond to a
candidate’s request for additional information.
Nine cases were administered during the October 2015 test administration. As shown in
Table 8 below, there were small numbers of candidates taking each case, as the number of
candidates per case ranged from 3 candidates for Case #2 to 12 candidates for Case #1.
Therefore, the statistics for these examinations should be interpreted with caution, as they
are based on small numbers of candidates. ACNB indicated that since the examination is
administered in three locations, over a two-day period in morning and afternoon sessions,

to ensure the security of an examination (i.e., case), the cases were assigned to candidates
based on location, test date, and session.
The data in Table 8 show that the pass rate was high for the examination, as only one of the
75 candidates taking the performance examination failed the examination. The total mean
(weighted) scores ranged from 70.32% for Case #9 to 98.66% for Case #11. The table also
presents the weighted pass point for each examination, the weighted total score standard
deviation (SD), the weighted total standard error of measurement (SEM), the mean score
(raw) for the physical examination (PE), the SD (raw), and the mean (raw) scores and SDs
for the cases. The reliability indices and decision consistency indices for all examinations
were .99, respectively. Appendix J presents a frequency distribution of total scores.

Quality Control Checks of the Data
The responses to the written examination were completed by candidates on Scantron
forms and Shoreline Psychometric Services, LLC. scanned and scored the examinations.
Since the written examination was administered in two parts, each candidate completed
two Scantron forms: a form for Part I, and a form for Part II. Each Scantron form was
checked against the candidates’ rosters to ensure that all forms were received. The
Scantron forms were checked for completion (i.e., candidates’ name, ID number) and for
stray markings that would be flagged by the scoring software. To check the accuracy of
scoring process by the scoring software, several Scantron forms were hand scored,
including those that scored near the pass point. To further ensure the accuracy of scoring,
a new scoring file was created and the Scantron forms were scanned for a second time and
the scoring was checked against the original score file.
The candidates’ responses to the performance examinations were marked on checklists
used by examiners who observed candidates’ performance. There were two checklists
used by the examiners: 1) a physical examination checklist, and 2) a checklist for the case.
Two examiners observed each candidate; therefore, there were four checklists per
candidate. Each checklist form was checked against the candidate roster to ensure that all
checklists were received. The data on the checklists were entered into an Excel
spreadsheet. Checklists in which the markings required verification (i.e., hard to read)
were sent to the ACNB for reconciliation. After entry of the checklist data into the Excel
spreadsheet, several candidates’ checklists were rescored and the scores were checked
against the original scores. The score data were provided to the ACNB in Excel
spreadsheets, as the ACNB prepared the candidates’ individual score reports and sent them
to the candidates.
Summary and Recommendations
Overall, the written examination summary statistics indicate that the examination has
reliability and the decision consistency indices within acceptable ranges for certification
examination programs. The statistics for the subcomponents show low to moderate
reliability indices, which is typical of subcomponents of most certification examinations, as
there are smaller numbers of test questions among the components and reliability is
sensitive to test length. When reporting sub-score performance data to candidates, it is
recommended that an explanation of the reliability of the data be provided, as performance
on the sub-areas are based on small numbers of items.
The item analysis for the written examination resulted in identifying test questions that
have low p-values (i.e., difficult) and/or have poor discrimination. Subject matter experts
reviewed these data and the test questions to finalize the examination’s answer key. As
described previously, it is recommended that item analysis statistics be used as a guide to
help subject matter experts in their review of test questions and revision of the
examination. In addition to reviewing test questions with low p-values and poor

discrimination indices, test questions that have high p-values should be reviewed when

and low performers. As with difficult test questions, it is recommended that easy questions
be revised or replaced when updating examinations because they do not distinguish
between those who have mastered knowledge from those who have not.
Akin to the written examination, the summary statistics and item analyses for the
performance examination should be taken into consideration when reviewing and
updating the examination for 2016. However, given the small numbers of candidates
taking each examination by case, the statistical summary results for the cases should be
reviewed with caution.
As with any certification examination program, the performance of examinations should be
monitored on an ongoing basis, and the statistical data presented in this report should be
considered when assessing the quality of examinations and developing new examinations.

Appendix B - Item Analysis Orientation
INTRODUCTION TO ITEM ANALYSIS STATISTICS
The item analysis includes difficulty and discrimination statistics. These statistics provide
information about the relative difficulty level of test questions and how well test questions
perform in relationship to total test score (i.e., discrimination). It is recommended that item
analysis statistics be used as a guide to help subject matter experts in their review of test
questions when finalizing answer keys and revising exam forms.
DIFFICULTY: P---values indicate the difficulty level of the test questions. It is the
proportion of candidates selecting a test question’s option. As a general rule, test questions
with p--- values of 0.30 or lower indicate that a small number of candidates (30% or less)
selected the correct answer. Test questions with low p---values are considered to be
difficult test questions.
1) Test questions with low p---values (e.g., 0.30 or less) should be reviewed to rule out the
possibility that there is more than one correct answer, or that the question is mis-
keyed (e.g., option “A” was keyed as the correct answer, but the actual correct
answer is option “D”).
2) If there is no problem with the correctness of the answer key, then assess the
appropriateness of the test question for the examination (e.g., the subject matter
was accurate but the question was not appropriate for the purpose of the
examination). DISCRIMINATION: The point biserial correlation is a discrimination
statistic that shows the relationship of performance on the test question and total
test score. The statistic, which is a correlation, may range from ---1.00 to +1.00. It is
desirable to have correlations that are at least 0.10 or higher (0.20 would be
desirable), as the correlation indicates a positive relationship between performance
on the test question and total test score. Test questions with correlations less than
0.10 should be reviewed by subject matter experts to verify their accuracy, clarity
and the questions’ answer keys, as such a low statistic suggests a poor or inverse
relationship between performance on the test question and total test score.
INSTRUCTIONS: The test questions listed in the tables below were flagged for
review because they have item analysis statistics indicating that they are difficult
questions and/or they have poor discrimination properties. Please review the test
questions to determine the following:
1) The item was mis-keyed (e.g., A was indicated as the correct answer, but C is correct).
2) The item has more than one correct answer (e.g., A and D are correct).
3) The item was unclear; therefore, a correct answer could not be identified.
4) The item was clear, appropriate for the examination, and the keyed answer is correct
(e.g., item is “ok as is”).
