Relate Bradford - Caring and Sharing Bradford Trident Business Centre, 1st Floor 11 Edward Street Wakefield Road BD4 7BH Caring and Sharing is a confidential specialist counselling service which offers carers a safe space to talk about the impact of their caring role on their own emotional wellbeing and how it affects them in their close intimate and family relationships. Appointments can be made for individuals, couples or family members as needed. Talking can help carers to care for themselves by: exploring how they are really feeling about things; acknowledging and managing difficult feelings; building confidence; gaining extra support outside their network of family and friends; planning for the future and keeping themselves well.

Carers Resource Telephone: (01274) 449660 email: [email protected] www.carersresource.org Carers’ Resource is a Dementia Friendly award-winning charity supporting unpaid carers in Bradford, Airedale, Wharfedale and Craven. Staff listen to carers and understand the chal-lenges they face. Carers can be helped with wellbeing reviews to address their health, emo-tional, social and financial needs, one-to-one support, emergency/contingency plans, groups and drop-ins, employment and training advice, access to Carers Wellbeing Grant, volunteer-ing opportunities, and support to access breaks, grants, education and leisure. Support for referrals to Adult Social Services is also given. Many of the staff are Dementia Friends. It also includes the Home from Hospital service for adults who live in Bradford, Airedale and Wharfedale who may need some extra support after being discharged from hospital. It in-cludes patients who may be at risk of readmission, as well as people with dementia or long-term conditions, and people who are isolated. Referrals are made by health professionals. Meri Yaadain My Memories Dementia Team Email: [email protected] Telephone: 01274 431308 Meri Yaadain is a Social Services (Adult Social Care) led initiative in partnership with Bradford and Airedale Teaching Primary Care Trust, Age UK Bradford and Alzheimer’s Society Bradford. The team consists of a manager and two part-time staff who undertake a wide range of activities to reach out to and engage with older people who have dementia and their carer’s and families who need educating about the condition as well as the services available to help them.

Home visits – to help carers have someone to talk to and get some information from. This is to help them feel supported

Support Group – a monthly get together of like minded people, both carer’s and people suffering from dementia, enables sharing of information, some activities, relaxation and a bite to eat

Telephone advice – for those who need a quick chat or have question. This often allows a quick referral onto an appropriate agency.

Run by volunteers it is open 10am-4pm Monday-Friday Excluding bank holidays. It offers support to carers of BDCFT patients and service users including access to information and advice as well as activities and drop in sessions. Clinical staff will deliver educational sessions on self-care/management for carers, which will help support them in their caring role. Telephone Information line available and monthly Caring and Sharing meetings.

Memory Assessment & Treatment Service

Supported Languages: English, Urdu (Family Support Worker)

Specialist assessment, diagnosis, treatment and support clinic for people experiencing

memory problems and living within Bradford City PCT and Bradford South & West PCT

catchments. Clinics take place weekly at Daisy Hill House and at Holmewood Health Centre.

You need to be referred to this service by a Consultant Psychiatrist.
