Regional Coordinator
Pathways Home Health and Hospice, Sunnyvale Coordinates the delivery of medical equipment ordered for Home
health patients, assuring timely and accurate processing of order. Obtains and places Medical equipment orders for Home Health
department. Sends weekly updates and reports to each vendor and ensures that all
updates are received weekly for follow up to doctors, clinicians, and Home health Management.
Data entry of all Home health orderss to Allscripts and Excel spreadsheet.
Communicates insurance and/or other clinical delivery problems to appropriate clinical staff in a timely manner.
Problem solves and communicates with physician any delivery problems involving physician documentation.
Acts as the primary back up to the Hospice DME coordinator. Adheres to all Federal and State regulations, agency policies and
protocols relating to the DME functions. Participates as a member of the administrative services support team;
rotates to cover receptionist breaks, lunches and absences; assists with coverage of other team members’ desks as appropriate with skills and training; rotates through weekend and holidays staffing as needed
