ALLOPATHIC PRIVATE MEDICAL CARE REGISTRATION Allopathic Private Medical Care Registration is the process of registering hospital. Here, hospitals are categorized into following types Clinic or Consultation Rooms/polo clinics/ Hospital or Nursing home less than 20 beds/ Hospital or Nursing home 21 to 50 beds/ Hospital or Nursing home 50 to 100 beds/ Hospital or Nursing home 100 to 200 beds/ Hospital or Nursing home above 200 beds. The DM & HO is primarily responsible for sanctioning the temporary registrations for the Allopathic Private Medical Care Establishment (Hospitals) in all districts. Any Citizen or the Institution or the Society can apply for the Hospital registrations at Meeseva centers with all the supporting documents that are prescribed by the Department by paying the prescribed fee. Category Type: B User Charges: Rs35/- Documents Required: 1. Application Form* 2. Photo Copy of Lease Document* 3. Doctor Particulars in Annexure A* 4. Nursing Staff Particulars in Annexure B* 5. Paramedical Staff Particulars in Annexure C* 6. Supporting Staff Particulars in Annexure D* 7. Details of Specialties Available* 8. Details of Equipments and Furniture* 9. Audit Report of Last Two years* 10. Declaration on Stamp paper* 11. Demand Draft towards Registration Fee* 12. One set of photographs of the premises with its Functional areas (hard copy and Soft copy)* 13. Self addressed envelope, acknowledgement and affixed Registered postage fee stamps * Denotes mandatory requirement of documents. Procedure for Applying:

♦ Operator clicks on Health Department services to go to Health Department services.

♦ In Health Department Services Select “Allopathic Private Medical Care

Registration” Service like in the below screen.

♦ After selecting the “Allopathic Private Medical Care Registration” Service in the Health Department services, Allopathic Private Medical Care Registration Application Form will be displayed.

♦ Operator enters all the Hospital details such as Hospital Name Door No/H.No, Street Name , Locality/Land Mark , Mandal , Village , Pin Code, Own/Rent, Lease Period In Years, Hospital Estd Date, Correspondent Name, Correspondent/PRO/

Administrator Mobile No, Correspondence Email Id, Description of Open Area, Description of Constructed Area , Financial Position Brief Description , Other Information Brief , Strength of Beds and Hospital Category, Service offered details such as Basic Service either yes/no, Specialty Service either yes/no, Super Specialty Service either yes/no, Diagnostics Service either yes/no, Pediatric Service either yes/no, Physiotherapy Service either yes/no, Labour Room with Pediatric Care Facility either yes/no, Operation Theatre either yes/no, Diagnostic Facility Including Clinical Laboratory Imaging Facility either yes/no, No of Facilities Available and Delivery Type, Society details such as Society/Trust Name, Address and Society Registered Date, Amount details such as Registration Fee, Diagnostics Fee, Physiotherapy Fee , Total Fee and Demand Draft details such as DD Number, DD Date, DD Amount(Rs), Bank Name and Branch Name …etc. like shown in the below screen.

Note: Here Operator can select Strength of Beds either <20/20 to 50/50 to 100/100 to 200/ 200 above and Hospital Category either Clinic or Consultation Rooms/polo clinics/ Hospital or Nursing home less than 20 beds/ Hospital or Nursing home 21 to 50 beds/ Hospital or Nursing home 50 to 100 beds/ Hospital or Nursing home 100 to 200 beds/ Hospital or Nursing home above 200 beds.

Note: Please note that Registration fee is generated based on hospital category. Like shown in the below Screen.

Note: Please note that based on number of facilities entered in No of Facilities Available field that much amount of text boxes are generated. Like shown in the below Screen.

Note: Please note that based on the customer request, the delivery option (either through Manual or Speed Post - Local or Speed Post - Non Local at the Franchisee) has to be selected properly. Like as below Screen.

♦ Operator select bank name from bank drop down. Like shown in the below Screen.

♦ Operator collects the necessary supporting documents as indicated in the service page and scans the same documents and uploads into the system. Like shown in the below Screen.

Please note that all the mandatory fields have to be entered, otherwise request will not be accepted by the system. Please Note that Operator has to select the Permanent Address of the applicant correctly as the concerned officials visit this place during the verification of his/her Particulars.

♦ After entering all the mandatory fields and uploading the necessary supporting

documents, Operator clicks on “Show Payment” button like shown in the below screen.

♦ After Clicking on “Show Payment” It will go to Conform Payment. As shown in

the below Screen.

♦ Based on the delivery option chosen by the citizen, the service charges will be

calculated and displayed. The charges for Postal (Local) is Rs. 33/- and for Postal Non Local is Rs. 46/-.

♦ Operator collects the amount from the citizen and submits the request by

Clicking on “Conform Payment” button. Like shown in the below screen.

♦ After operator submits the request and on confirmation the receipt is generated as shown below along with the Certificate Delivery Date.

Please note that in case if you do not have sufficient balance with your service provider (SCA), system will not accept the request.

♦ On submission, the request will go to respective officer workflow for further processing.

♦ Once request is approved in the process then, Hospital Registration Certificate will be dispatched through courier to hospital address.
