RECOMMENDAnON: That your Board approve and sign an amendment to the standard services agreement with Krassons Inc. (Principal: David Platton; Location: Oakland, CA) for the provision of services to allow interface of Clinician's Gateway into the fNSYST Client Data Collection System to increase contract amount from $85,000 to $234,275 for an increase of$149,275 and no change in contract term. (Contract #3995) DISCUSSION/SUMMARY: On June 9,2009, your Board approved a contract with Krassons Inc. for the continuation of the fNSYST Client Data Collection System until such time that a database management software is developed and completed. Federal mandate requires that the State Department of Mental Health contract with an External Quality Review Organization to perfonn annual audits of the Medi-Cal (Medicaid) delivered services. A most recent audit finding supports the use of Clinician's Gateway to comply with the regulatory requirements. Behavioral Health Care Services (BHCS) with the help of Krassons implemented the Clinician's Gateway which allows for the prov ision of Electronic Health Records in regard to delivered services. These notes are completed by clinicians and attached electronically to the department claims and/or billed service. These clinical notes include patient charting and other client data which support the department Medi-Cal and Medicare claims thereby minimizing disallowances. BHCS is requesting an increase in the amount of$149,275 to fund the development and installation of upgrade software modules to sustain the Clinician's Gateway efficiency. SELECTION CRITERIA: BHCS has selected Krassons Inc. after obtaining three informal quotes from the SLEB certified vendor's database. Krassons Inc. is a certified SLEB vendor. FINANCING:

Funding for this contract is included in the BHCS's budget for one-time Mental Health Services Act. There is no net county cost.

AMENDMENT TO CONTRACT

This Amendment is effective on the lSI of March 2010.

Reference is made to that contract made and entered into on the 1st day of May 2009, by and between the County of Alameda, hereinafter referred to as the "County" and Krassons Inc., hereinafter referred to as "CONTRACTOR".

Said Contract is hereby amended as follows:

1. On Exhibit B, is modified to increase the contract in the amount of $149,275 from $85,000 to $234,275 and to increase the hours of service from no more than 739 to no more than 1,143. Exhibit B, items 1 and 5 shall be deleted and replaced with the following provisions:

1. Payment under the terms ofthis Agreement shall not exceed the total amount of Two Hundred Thirty Four Thousand Two Hundred and Seventy Five Dollars ($234,275) for the period ofthe contract. This cost includes all taxes and all other charges.

2. Contractor shall not exceed 1,143 hours ofservice dun'ng this period.

Except as herein amended, said Contract is continued in full force and effect.

IN WITNESS WHEREOF, the parties hereto have executed this Amendment as of the day and year first above written.

misconduct within" the past three years.

If there are any exceptions to this certification, insert the exceptions in the following space.

Exceptions will not necessary result in denial of award, but will be considered in determining contractor responsibility. For any exception noted above, indicate below to whom it applies, initiating agency, and dates of action.

Notes: Providing false information may result in criminal prosecution or administrative sanctions. The above certification is part of the Standard Services Agreement. Signing this Standard Services Agreement on the signature portion thereof shall also constitute signature of this Certification.
