    Guidelines for Completing the Appraisal Form

    The performance of an employee in the current role shall be assessed against the OBJECTIVES set by boththe appraiser and the appraisee at the beginning of the assessment period

    A copy of the set OBJECTIVES must be kept in hand whil3e assessing the performance of the assessee. A copy of the same must also be attached with this performance review along with a list of tasks for

    achievement of these objectives.

    OBJECTIVES must be SMART: Specific Measurable

    Agreed/Attainable Realistic-as per the resources available & Time specific

    This process must be completed through a detailed discussion between the manager Appraiser and theAppraisee. The form must be filled and signed by both Appraiser and Appraisee acoordingly.

    The form must be submitted as per the time schedule

    Assessment against the set OBJECTIVES is to be done using a five point rating scale:

    1. Inadequate performance2. Needs significant improvement in performance. 3. Meets performance expectations; On target; Good performance4. Exceeds targets; Excellent performance 5. Exceptional/Outstanding Performance: Role Model

    I. EVALUATION OF EMPLOYEES WORK AGAINST DEFINED

    OBJECTIVES
