UK annual monitoring report form

We ask you to complete this form after every year of the grant. These reports allow you to reflect on your progress and share your learning with us, and we use them to check you’re on track. We also use this information with that from other projects to understand what difference we are making and to help us raise money. It is important that you are open about anything that is not working, including your original plans, so that we can work with you to decide on next steps. We are open to you changing your targets and approach where these changes are based upon your learning and will better help you deliver your outcomes.

This form is intended to be completed alongside the accompanying guidance notes and your original start-up form and application form. Please do not feel you have to fill up the word limit specified for each narrative section), do not add additional boxes or fields and if a number is asked for, please provide a single number only. When you have completed your form please upload it to GEM and your grant manager will be in touch with you.

Section 5: Comic Relief’s marketing and communications needs

5.1 Case study (350 words maximum): Please provide us with a case study which highlights the difference your work has made to someone’s life. Here we are looking for a concise story, including a brief background to the issue/need you have addressed, how and why the project helps and the result for the person’s life or the issue addressed. Please also consider whether we might be able to follow up with this person about their experiences (see the next question, 5.2).
