As you make your way to Chinatown for a soba noodle salad, be sure to stop at one of our bars for a cold beer or glass of wine before hitting Dupont for some iconic street tacos. Make sure you save room to sample the mini cupcakes from Georgetown as you cap off your neighborhood tour! Community Transition Program (CTP) provides improved access to housing resources, vocational services, peer support and other recovery services for citizens with histories of substance use disorders who are re-entering the community from prison.

Telemedicine, in partnership with MYidealDOCTOR, provides our members with convenient remote access to care through virtual medical consults.

Substance Use Disorder (SUD) programs have allowed us to be at the forefront of the opioid epidemic. Through our partnerships we’ve seen positive improvements in addiction including:

Medication Therapy Management (MTM) program identifies members with opportunities to prevent or address medication-related problems, gaps in care, duplication of therapies, cost opportunities, non-adherence and a number of other quality-focused interventions.

Pharmacy Lock-In program requires members who use excessive health care services to fill their prescriptions at one pharmacy. This ensures prescription drug use is coordinated and reduces over-prescribing and prescription drug abuse.

Care4U is a game-changing, holistic population health model. Through tailored care plans, CareSource can address the needs with the greatest impact for each individual member. The model fully integrates our commitment to Primary Care & Prevention, Care Management, Behavioral Health and Life Services, promoting health and wellness across the entire continuum of the populations we serve. Services and support include but are not limited to:

JobConnect is a benefit offered by CareSource Life Services to help our members get and keep jobs they need to improve their lives. This service is offered at no cost to our members. This first-of-its-kind, award-winning program works to move people toward a life of self-sufficiency.

We help with employment assistance, interview skills, transportation, job training and more to help our members balance life and remove any barriers that stand in the way of reaching their dreams. We also partner with employers and organizations in the community to connect our members with additional resources and opportunities.
