Addendum No. Five Pioneer Manor Skilled Nursing Facility April 4, 2014
The Construction Document set dated October 31, 2013 as amended by previously issued Addendums 1,2, 3, and 4 is amended further with this Addendum No.5, more specifically described as follows:
CLARIFICATIONS TO VE ITEMS PREVIOUSLY ISSUED IN ADDENDUM 4.
5.1 VE-22: All faucet finishes to be chrome; EXCEPT for the Public Restrooms at the Town Center where the finishes to remain as originally specified.
5.2 VE-27: Alternate Lighting packages by equal manufacturers to those specified will be
allowed, so long as the appearance, performance, and lumen output match the fixtures specified. Add “size” of fixture to criteria above, and note that any substitutions are subject to review and approval by the Design team.
SIGNIFICANT Q & A ITEMS PREVIOUSLY POSTED TO CHRISTENSON CORP WEB SITE, INCLUDED HERE IN FOR EMPHASIS. THEY HAVE NOT BEEN ALTERED SINCE POSTING.
5.3 Note the following additions to the Electrical design. See pdf’s as part of this Addendum. Add Panel Schedule “PC1A” Add Panel Schedule “PC2A” For Emergency Panel “EGEMDB” change the rating from 1200amps to 2500amps. Add Layout of the Emergency Electrical Room
5.4 Following are the base quality standards for the Pioneer Manor Project that must be maintained.
1) Exterior hollow metal doors to be 18g, A60 Galvanealed, Flush tops, Seamless, Exterior
Frames to be 14g, A60 Glavanealed with Corner Face Welds. Interior Hollow Metal frames to be 16g, cold rolled steel, with full face welds. Basis of design Manufacturers: Curries, Steelcraft, Pioneer.
2) Common area wood doors to be Full Flush, Plain Sliced, Book & Balance Match, 5 Ply, White Maple, Factory Finished in a Manufacture’s standard color selected by Architect. Basis of design Manufacturers; Marshfield, Eggers, VT Industries.

3) Interior units doors to be Solid Core, Molded, 3 Panel design, primed for painting with
Timely 18g, Browntone frames and colonial casing. Basis of design Manufacturers; Jeld-Wen, Masonite, and Timely.
4) Exterior hinges to be, Ball Bearing, heavy weight, Stainless Steel, 630 Finish. Interior
hinges Ball Bearing, Standard Weight in a 26D Finish. Basis of design Manufacturers; Ives, Mckinney, Hager, and Bommer
5) Exterior locksets to be Grade I, Extra Heavy Duty, Basis of design Manufacturers: Schlage (ND Series), Sargent (10 Line), Best (93K Series).
6) Interior locksets to be Grade II, Heavy Duty,
Basis of design Manufacturers: Schlage (AL Series), Sargent (7 Line), Best (73K Series)
7) Unit Interior Locks to be Grade II, Basis of design Manufacturers; Schlage (F Series), Sargent (DL Series)
8) Exit Devices to be Grade I, Stainless Steel, 630 Finish with Lever locking trim

5.5 The following answers the questions regarding the Low Voltage systems
coordination for Pioneer Manor.
A) Telephone / data system: General Contractor to furnish and install per dwg “E0.16”
B) Cable TV system: General Contractor to furnish and install per dwg “E0.17”
C) WiFi: General Contractor to provide the following: Provide two wireless local area network infrastructure throughout the entire facility. One is for the residents and visitors, and one is for the staff. There will be a POE wireless access point (WAP) for each system every 50 ft thru all corridors and corridors and no more than 25 ft from the end of the corridor. Include an additional 30 locations in open

space areas. From each WAP for each system route a Cat “6” cable to the nearest tele/com room and terminate in a dedicated patch panel. At each WAP point include a 1 gang flush in the ceiling plaster ring to which the unit can be mounted to. The owner will provide all WAP equipment and the contractor to make all connections and install the units.
D) Closed CKT TV the system is a POE type. The Owner will furnish and install all
cameras, CAT 6 cables, terminations, equipment , monitors etc. General Contractor to provide all power in tele/data rooms. General Contractor to furnish and install (12) exterior flush in the bldg wall 4”sq x 2”d j-box with 1 gang plaster ring and 40 ft of conduit to an accessible point above the corridor ceiling. General Contractor to furnish and install (26) corridor plaster rings for interior camera locations.
E) Nurse Call System: Owner to furnish and install the Nurse Call System; this is a
hardwired system. Contractor to furnish and install 170 4”sq x 2”d box with 1 gang plaster ring and a ¾”conduit (30 ft) stubbed into the corridor to an accessible ceiling location. System has 200 dome lights; GC to provide 2 gang plaster ring mounted in the ceiling. There are 320 pull cord location; GC to provide 4 x sq x 2 gang ring plus a ¾” conduit (30ft each) into corridor into an accessible ceiling location. Contractor to provide all power in tele/data rooms
F) Wander Management system: Contractor to include a 4”sq x 2” j-box with a 1-gang
plaster ring and a ¾” conduit (10ft) stub up at 45 doors.
G) PA system; General Contractor to furnish and install; the locations are shown on drawing “e0.18” . The system is a POE system; Contractor to include a 2’x4” layin ceiling tile with speaker unit in locations as shown. Include all required headend equipment and controls and (20) local speaker volume control units. All devices are wired with cat “6” cable.
Paging speaker shall be IP based, and powered via POE. Speaker should include integrated talk back microphone, that allows for full duplex communications. Speaker shall be able to integrate and register via SIP to a Cisco Call Manager server, for full duplex talk back capabilities. Speaker shall include 8 inch speaker with microphone and integrated mounting of 2x2 tile hardware for drop in grid replacement of existing tiles in ceiling.
Each individual unit shall be capable of producing sound masking programmatically, as requested without the need for outboard sound generation or software for control. IP Speaker should be cross compatible to allow for software based network control, for potential future expansion, and easy upgrade of firmware.

Supported protocols must include:
• SIP • IPv4 • HTTP • SLP • TFTP • NTP • DHCP • IGMP • ICMP • TCP/IP • UDP • MDNS & MDNS-SD
Specifications as follows:
· Weight: 6.5 lbs · Approx: 2’x3”x2’ · POE (IEEE802.3af) · POE+ (IEEE802.3at) · 10/100 Mb network connection
H) Building access control system: The General Contractor to include a 4”sq x 2”d j-box with a 1 gang plaster ring and a ¾”conduit (10ft) up at 48 box locations and stub into the accessible ceiling location.
I) Power: Contractor to include an allowance for (5) 20a, 120v dedicated circuits with 100ft
of conduit and wire. NEW ADDITIONAL ADDENDUM 5 ITEMS
5.6 Drawing AD-4.1: Revise Resident Room Plan issued previously in Addendum 4 as shown on attached Sketch AD-4.1. Revisions are clouded and include changing the Barn Door to a 42” pocket door, providing a termination port for piped oxygen, changing the P-Lam countertop at the bathrooms to a “cultured marble” countertop, and adding a wall mounted task light above the bed. Allowance of $250 is just for the fixture. G.C. to also add for the necessary power and a separate switch.
Pocket Door hardware to be heavy duty type with corresponding track, ball bearing
wheels, pocket stop, and other customary appurtenances. Hardware to be ADA

acceptable 3” x 5” x1/2” OD Wire Pulls. Pulls to be accessible to the resident when door is fully in the pocket.
5.7 Drawing AD-5.1: Revise Resident Room Plans for rooms 1119, 1219, 2119, 2219,
2319 and 2311 to make these rooms Bariatric Rooms complete with ceiling mounted patient lift system. Basis for Design of the lift system to be the ARJO Maxi Sky 2 600# lifting capacity system. System to be a complete installation with all tracks, intermediate supports at the building structure for the track, controllers, slings, and other customary appurtenances for a typical installation. Each room to have a lift system.
5.8 Revise Bathing Rooms 213B and 213C as shown on revised sheet ID-221.3, and revised
sheet A-804, and as follows: Add a complete ceiling mounted patient lift system. Provide 8 LF of ceiling mounted track. Basis for Design of the lift system to be the ARJO Maxi Sky 2 600# lifting capacity system. System to be a complete installation with all tracks, intermediate supports at the building structure for the track, controllers, slings, and other customary appurtenances for a typical installation. Each room to have a lift system.
At sheet P-100, BT-1 Therapy Bath noted shall be changed to “Apollo” brand, model
number WP6000B Bathing systems including a transfer system, UV Infection control water purification system, have a rapid fill reservoir, automatic cleaning and disinfection system, Anti scald temp control valve, minimum of four adjustable and Omni directional hydrotherapy jets, integrated body wash and bath oil dispenser, premium flush mounted “Waterfall” faucets, easy latch entry door, and all other customary appurtenances for a complete installation.
GC to coordinate change in bath fixture type with required electrical and plumbing. 5.9 Drawing AD-5.2: General Contractor to include cost for a complete turnkey piped
oxygen system to each resident bedroom. See attached Drawing AD-5.2 for revisions to Basement Level Zone 4 to accommodate a central oxygen room and for the exterior bulk tank storage location. All changes to the existing building design to accommodate the piped oxygen system to be shown as part of the total system cost.
5.10 Drawing AD-5.3: General Contractor to include cost to revise Zone 3 Basement wall layout to create rooms for a future Laundry, Soiled Linen and a Central Clean Linen. See notes on Sketch AD 5.3 included in this addendum for more detail and notes. G.C. to rough in only the required mechanical, electrical, and plumbing systems to accommodate the future Commercial Laundry. Rough-ins to be based upon having two future Maytag Commercial Energy Advantage Washers; Soft-Mount Front-Load High Speed Extractor, Model No. MFS55PNA with the following requirements:

Load Capacity: 55 lbs (22 kg). Cylinder Volume: 7.8 cu ft (221 liters). Maximum Extract Speed/Maximum G Force: 915 rpm/350. Drive Motor: Inverter drive, variable speed, 4.0 HP (3.0 kw). Electrical Requirements: 208-240V 4Three-Phase. Breaker Size Required: 20 Amps. Operating Pressure 43-73 psi (0.3-0.5 mpa)
Provide rough-in based upon two future Maytag Commercial Energy Advantage On-Premises Drying Tumbler, Model No. MDG78PN with the following requirements:
Load Capacity: 75 lbs, 22.1 cu ft (34 kg, 625 liters). Drive Motor: 0.75 HP (0.56 kw), Reversible. Airflow: 600 cfm (17 cmm). Heat Input: 130,000 BTU/hr (32,760 kcal/hr). Electrical Requirements: 120V, 60 Hz, Single-Phase. Breaker Size Required: 25 Amps.
Provide stub-outs from nearest sources for water lines both hot and cold for the washers. Provide trench drain for the commercial washers, plus under slab drain line to nearest point of contact for sanitary sewer. Electrically, provide a local disconnecting means at the unit and a flex connection and/or receptacle as required. Include 3#12, #12g, ½”c to a panel location in the maintenance room and connect to a 20a/3p circuit breaker.
For the commercial clothes dryers include a local disconnecting means at the unit and a flex connection and/or receptacle as required. Include 3#8, #10g, 3/4”c to a panel location in the maintenance room and connect to a 25A/1p circuit breaker.
5.11 Clarification on various Owner items to be coordinated by the General Contractor. The G.C. will coordinate the following Owner provided items that appear in the documents:
.1. The Owner will be providing a “Smoking Hut” in the north courtyard. G.C. to provide
the slab on grade shown in the documents and provide two conduit stub outs to the Hut location, one for power for lighting, the other for communications.
.2.The Owner will be relocating an existing “Gazebo” to the north courtyard. G.C. to
provide the slab on grade shown in the documents and provide two conduit stub outs to the Gazebo location, one for power for lighting, the other for communications.

.3.The Owner will be relocating an existing stained glass window from the existing Pioneer Manor to the Chapel/Theatre room 108 of the new facility. G.C. to assist Owner with the relocation and then install the stained glass. See Interior Elevation 6 on A-805. The Owner will also be relocating an existing ceiling mounted projector and ceiling mounted (recessed) screen to the Chapel/Theatre room. Install both and provide power to both. Provide typical controls such as a wall switch to lower the screen from the ceiling cavity.
.4.The Owner will be relocating an existing “bingo board” from the existing Pioneer Manor to the Multipurpose room 106, west wall, of the new facility. G.C. to assist Owner with the relocation and then install the bingo board. Provide and coordinate power at the wall for the board. .5.The Owner will be purchasing 10 to 12 flat screen TVs to be placed at various locations around the facility. Provide an allowance of $2,500 for the required power and conduiting. Locations to be confirmed at a later date.
5.12 See Revised Wall Finish Plans, sheets ID-211.1 thru ID-211.5, and ID-221.1 thru ID-
221.5 for wall finish clarifications requested through the Q&A. 5.13 See Revised Interior Elevations, sheets A-800 thru A-807; except A-803, for finish
