IMMEDIATE ACTION TAKEN

South drive - Water accumulation at the split nect to the drive

South drive - Pump cable on floor (concrete) walk way at south drive and it is a trip hazards for people

Sec 42 travelling road - There is alot of water in sec 42 travelling road. The B/bye must pump the water out

Safety - The gloves is till not up to standard. The talk topic says we must wear gloves that fits a person. But the gloves don't fit some of the people

Hallway near control room - The area near the controll room is very dirty. Old computer screens lying around and old junk. Sort out that area

Near control room - Old florecent tubes in a dust bin near control room. The old globes must go to Ettiene and he will do the discharge of it, the hall way is still dirty

Sec 42 - Te S/C operator damaged 3 cables in the section. The 1st cable they bumped the 2nd cable pulled out of the socket and 3rd cable they drove over it

Sec 42 - The C/M operator on Day shift sec 42 damaged the scrubber fan of the C/M. This is not the 1st time it is the 6th time. It cost the mine R450 000.00 to fix

Road to sec 22 - Alot of water of one part of the road. Pipe burst and alot of water went to the road. Need to be pumped

Laundry - I saw that the laudry at the back and around is very dirty . No good house keeping

Sec 41 - Install telemeter system not working, reading incorrectly

Sec 41 - Section is very dirty, too much coal spillage, and not being swept with Eimco. All roads are affected

Report to miner to get Eimco and sweep

Sec 23 - The new drive for sec 23 inline not yet concreted and the roof bolts after blasting roof in way for new drive

B/Bye shiftboss need to cut roofbolts

Service - People that do scrap metal can not cut or take away what they want. Someone must be in control of it.

Road way to sec 21 - Hilti nails on 2 Seam in travelling road and left the new LDV flat tyre.Took it away but there's new nails in road way

2Seam drive - Scrap at all belt drive are not removed after cleaning

Safety - The gloves is still not up to standard. The size 10 gloves is to small. Must get bigger gloves please

E19West - The guide rope on E19West at split 5-10 is lying on the ground. Shiortwall and sec 42 travelling road

Underground travelling road - Lot of dust at the travelling road of sec 22

Road to section - Road to section 22 is dry and need to be wattered

Surface - Dustbins at containers offices are not clean completely on regular basis

2 Seam - Please to all employees working on 2 Seam , stop stealing my sub-station fire extinguisher

2 seam - 1 Eimco driver who was cleaning next to interseam sub-station bumped outside fire extinguisher

4 Seam - There is lot of water in the travelling road on sec 42

Sec 21 - Bumping of bore-hole using (new r/bay) with an Eimco

Sec 21 - Sect belt ext went very well for the first time everything was plannedgood

HW lange and JC Mine capt is doing very well in restoring section standards

Sec 21 travelling road- Travle roads was extreamly dusty when going into the section

James Prinsloo ensured roads was watered down at 11:30

Sec 21 - Travelling road to sec 21 in very bad conditions damage to bakkies will be enormous

H.W Lange said he will start repairing road

Sec 23 - The shuttle car cable is covered by a heap of coal of which it can't be pulled by hand

Called for a Eimco to come and remove the coal but ni Eimco was available

Sec 23 - A fletcher damaged most of its components while pulled or towed from old section 23 to the new section

Sec 23 - There's a lot of water accumulation in the refuge bay to sec22 which is used by sec 22 and 23Sec 23 - There's no toilet in the refuge bay to sec 22

Main gate - at 05h15 I went through the turnsatll at the main gate the security was not there he was inside the security office sleeping. How safe is our cars in the parking lot if the security sleeps?

M/G 18 - Belt drive operator did not close the water sprays at E.O S on Friday night after shift and it left the place flooded

I closed the valve at the belt drive

M/G 18 Splt 22 - Driver of tractor 3628 put a cable bin ontop of pump cable busy pumping split 22

M/G 18 Split 3 - Tractor driver of tarctor 3628 riding over 8kw pump pumping in travelling way. Glimpoles are hanged to indicate position of pump

Picked up pump and tested for for any damages, spoke to driver. Says he is sorry

M/G 19 - 6' fresh water line been damaged where pulled compressor. Don't close water don't report to anyone. Causing accumulation of water in travelling way

M/G 19 - Contractors taking spare cables not belonging to them also not their jobs, cauing the pump people to run out of cables for pumping

S/W split 20 - The water tanks are overflowing continiously and it causes increasing accumulation of water

Sec 42 - The manifold at sec 42 belt drive is cracked causing accumulation of water

Reported to S/O and closed water

Sec 41 travelling road - The floor condition to sec 41 is in very poor conditions for machinery to drive on . Need road building

Sec 41 split 10-28 - The life line in sec 41 T/Road from split 28 lying on floor

Sec 42 - There is a lot of water in sec 42. Maingate 20. The miner/shiftboss must pump the water out

4 Seam - The wash bay on 4 Seam is very dirty. The miner must clean and wash bay at 4 Seam as soons as possible

4 Seam - Both power washer guns on 4 Seam is broken.Nobody can use the power washer at 4 Seam

E19West split 8-15 - The guide rope is lying on the ground. From split 8-15 on the E 19 West road to section 42 and s/wall

Sec 41 - There is a huge pile of coal next to the Fitters. The S/C can not reach F/B safely, no Eimco in sec to clean area

Inform miner to get an Eimco to clean area

Sec 41 - C/M 1440 sleeves in bad conditions, no sleeve maint is done, +-30 sleeves to be replaced

Store - On Friday 16 Oct at +- 15h00 a rain storm occured. The drain infront of the Joy store overflowed causing all the water to accumulate infront of the store and flooding the store inside

Reported to Lieb to assist with action to divert water

Surface - 6 People assult Thasasa employee on Mine premises asking for cables (copper). They also told him that they are going to kill him.
