ABOUT THE PRACTICE The practice opened in 1991 in Palmerston Road, but was moved to Forest Road in 1997, the practice started with 1 nurse, 1 doctor with 1 morning and 1 evening receptionist. Now in 2011, we have; 4 doctors 4 nurses 6 receptionist (3 part time receptionists) DR DAUOD SHANTIR Qualified in 1971 – MBBCh Diploma in Child Health (DCH) 1977 OPENING HOURS TRAVEL IMMUNISATIONS Take some time to consider any potential risks in the area you are visiting. If you are pregnant or have a long-term condition, seek medical advice from your doctor. Make sure that you have appropriate vaccinations in good time before your departure. You can be vaccinated against; Typhoid Polio, Tetanus and Diptheria Hepatitus A Hepatitus B Remember to have your travel vaccinations from 4-6 weeks before your trip. If you are taking regular medication, remember to have enough for your journey and have the documents at hand if questioned by country authorities. REMEMBER YOUR HOLIDAY KIT; It should include; Paracetamol Travel sickness tablets Plasters Rehydration solutions, such as Dioralyte, to increase your body fluid levels Anti-Diarrhoea, such as Imodium High factor sunscreen HEALTHY EATING Start your day with your 5 a day PRACTICE CHARTER An up-to-date practice charter is available at the surgery CHILDHOOD IMMUNISATIONS Immunisations are the safest and most effective methods of protecting your baby against serious diseases. Immunising your child not only protects them, it also prevents the diseases spreading and protects other children. For more detailed information visit the NHS website or contact your doctor, practice nurse or health visitor. You can also contact NHS direct on 0845 4647. Each vaccination is given as a single injection into the thigh or upper arm. REPEAT PRESCRIPTIONS• To avoid errors, please do not order prescriptions by phone• Please allow 48 hours if collecting and 3 days if enclosing a stamped address

envelope• Patient's on repeat prescription systems should see the doctor at regular intervals as

arranged by the doctor between 6-12 months for reviewing• The counterfoil side of the prescription is the side given when collecting a

prescription• Please note that the consultation time should not be used for repeat

prescription

INFLUENZA VACCINATIONSPlease register your name at the reception as early as possible…

The vaccine is important for the following patients;

• People aged between 65 and over• Low Immunity• Asthma• Renal Transplantation• Heart Disease• Chronic Liver Disease• The delivery of vaccine is usually during late September• Type 1 & 2 diabetes sufferes• Those that require Hypoglycaemic or diet controlled tablets.

MEDICALS for insurance, driving and pre-employment are done by the doctor. Please book an appointment in advance

FAMILY PLANNINGA full range of contraceptive advice services including the pill and injections are available, please make an appointment with the doctor or nurse

From Walthamstow Central, take the 357 bus from Stop KA, towards Chingford Hatch, get off at Jewel Road (Stop BA), walk towards the end of Jewel Road to arrive at the surgery.

From Walthamstow Central, head south east on Station Approach, turn left on Hoe Street/A112 and follow the road turning left at Forest Road.

From Walthamstow Central, head south east on Station Approach (staying on the left), turn left onto Hoe Street, turn left On Greenwood Road, then turn left onto Forest Road.

BOOKED CONSULTATIONS• Consultations are made by appointment only• Each consultation will last for up to 10 minutes• Emergencies and babies will be seen on the same day• Urgent telephone consultations are available between 12:00 and 12:30PM

ONLY• If you are unable to keep your appointment please notify the surgery

giving 24 hours notice where possible, so that another patient can have your appointment

HOME VISITSHome visits are for emergencies ONLY, please ring before 11.30AM to arrange a home visit, this is under the discretion of the doctor/nurse.
