1. PROJECT HSE PLAN

1.1 Introduction

The Project HSE Plan presents the CONTRACTOR’s HSE organization, the

responsibilities and the activities to support and implement the CONTRACTOR’s

HSE Policy and OWNER’s HSE requirements, as described in the contract.

All contractors engaged during this project shall comply with and meet or exceed

the standards and specific criteria detailed in this document.

The Project HSE Plan details the health, safety and environmental policies,

objectives, standards and procedures to be used during construction and outlines

the manner in which these policies, standards and procedures will be implemented

and maintained.

The CONTRACTOR places great emphasis on maintaining the highest standards of

health, safety, welfare and environmental protection during the execution of the

Project.

1.2 HSE Policy and Objectives

The Contractor recognizes responsibility to protect and maintain the health and

safety of employees, OWNER, the general public.

The following are Project Objectives.

No (Zero) Significant Property Loss.

Exclusion of unsafe conditions/situations on the site.

Exclusion of situations that could have a negative impact on employee's health,

safety or welfare or the environment.

The objectives of the Project HSE Plan are:

define the HSE organization and responsibilities for the Project Site preparation,

Construction and Commissioning phases of the Project;

define the primary HSE activities for the Project Site preparation, Construction

and Commissioning phases of the Project;

describe the scope and objectives of the planned activities;

summarise the methodologies and procedures to be utilised;

define the responsibilities for each activity;

list the HSE deliverables and to track these deliverables to completion.

1.4 Project Description

The project contains a number of features located in the Owner’s South and East

Kuwait Assets, which are within an Owner controlled restricted area. The features

include sludge pits, effluent pits and gatch pits.

Key objectives of the Project include the following:

To repair damage done to the natural landscape as a result of oil operations

over many decades and;

To rehabilitate a selection of contaminated features to ecologically functioning

areas that are able once again to support a typical range of native plant species;

Appropriate Health & Safety protocols and Explosive Ordnance Detection (EOD

appropriate to the associated risks noted with the features;

To recover oil contained within the features (where applicable)

To apply a risk based methodology of remediation concurrent with recognized

international protocols adapted to the prevailing environmental conditions in

Kuwait;

To initiate soil remediation techniques associated with the nature of the oil

contamination existing within the features;

To establish a selection of native plant species to initiate ecosystem recovery

and reduce desertification at the selected features;

To monitor and compare the effectiveness of various remediation approaches

using the established native plant species as performance indicators.

1.5 Definitions

1.6 Legislation and Industry Standards

The CONTRACTOR will comply with OWNER standards, industry standards and

prevailing legislation. The safe work practices and procedures in this Project HSE

Plan for provide guidance for the most common hazards associated with this type of

project. It is expected that all Contractor, Subcontractor employees, Vendors and

visitors shall also adhere to the following regulations, codes and related industry

standards as applicable:

1.7 Responsibilities

1.7.1 Project Manager(PM)

1) Ensure the project is performed in accordance with the HSE Management

System.

2) Approve the Project HSE Plan and HSE Procedures.

1.7.2 Engineering Manager(EM)

1) Be responsible for coordinating and reviewing the production of Engineering

HSE deliverables as per contract requirements.

2) Prepare and review Engineering HSE Plan and procedures.

3) Initiation and organization of the engineering HSE design reviews and

engineering HSE studies.

1.7.3 Project HSE Manager

1) Establish the Project HSE objectives and prepare a Project HSE Management

Plan to achieve them.

2) Provide support to the project team as necessary and participate in risk

assessment, HAZOPS and designated project meetings.

1.7.4 Site Manager (SM)

1) Ensure that HSE Management System is implemented throughout the site life

cycle.

2) Be overall responsible for the implementation of risk assessment

recommendations.

3) Collect information on HSE, and distribute it to the relevant organizations.

4) Ensure completion of accident investigations and approve the recurrence

prevention plan.

5) Act as Chairman of the HSE Committee and appoint a committee secretary.

1.7.5 Site HSE Manager

1) Manage the Project HSE Plan and HSE Procedures throughout the site life

cycle..

2) Track the implementation of risk assessment recommendations.

3) Select appropriate supplies when purchasing personal protective equipment

and defense devices for machines, tools and equipment.

4) Coordinate and participate in HSE inspections and recommend necessary

control measures.

5) Participate in Incident Investigations to determine the causes of incidents and

provide technical direction and guidance to prevent recurrence.

6) Provide guidance and advice to Site Manager, Superintendents and

Supervisors on disciplinary measures for employees who violate legal

regulations on safety, project HSE requirements, or employment regulations.

7) Identify the needs of the project for HSE Education and Training, develop and

implement the HSE Education and Training programs as necessary.

1.7.6 HSE Supervisor

1) Assist the Site HSE Manager to ensure that Project HSE Plan is understood

and implemented.

2) Monitor the site works to ensure conformance to HSE requirements.

3) Verify the safety and identify deficiencies of machines, tools and equipment

relating to the work under the project scope.

4) Assess the working uniforms, protective equipment, and protective devices of

the project personnel, educate and guide them on how to wear and use them

correctly.

5) Report on any incidents that occur during work and take emergency measures.

6) Cooperate with the guidance and advice of the Site HSE Manager.

7) Inspect implementation of risk assessment recommendations and report to Site

HSE Manager.

8) Conduct on the job HSE Training.

9) Provide support for Superintendent and Supervisor site HSE activities

1.7.7 Explosive Ordnance Disposal (EOD) Specialist

1) Provide EOD construction support during intrusive / waste separation activities.

2) Develop and give EOD awareness training to all project personnel.

3) Develop EOD action plan and instructions for the project.

1.7.8 Radiation Protection Supervisor (RPS)

1) Perform walkover surveys for all specified locations.

2) Develop and give NORM awareness training to all personnel if required.

3) Develop instructions & action plan for NORM contaminated materials handling.

4) Provide support during NORM contaminated material handling and disposal.

1.7.9 Site Security Supervisor

1) Manage day to day security operations and ensure enforcement of the

applicable regulations, codes and standards for the protection of the project.

2) Provide ongoing training for security personnel.

3) Manage site security patrols, report the discrepancies to the HSE Manager,

and take corrective actions as required.

4) Report to Site HSE Manager if any worker violates the security regulations.

5) Arrange security meetings and follow-up on required actions as necessary.

6) Ensure all security records are kept complete and current as per Security Plan

requirements.

1.7.10 Site Superintendents

1) Ensure that the requirements of Project HSE Plan are known, understood and

implemented by Subcontractors at every stage of construction.

2) Review and comment all method statements and risk assessments as applied

to critical tasks and activities such as heavy lifts, complex rigging feasibility

studies, unusual tasks which are not covered by existing procedures.

3) Conduct Project HSE inspections, record findings and implement corrective

actions.

1.7.11 Site Supervisors

1) Ensure implementation of all method statements and risk assessment

requirements on the work site..

2) Identify and correct unsafe actions and conditions.

3) Conduct Project HSE inspection and ensure preventive measures are

implemented..

4) Ensure relevant Project HSE procedures are understood and implemented by

the subcontractors.

1.7.12 Sub Contractor

1) Shall comply with local regulations, and Project HSE Plan and procedures

requirements.

2) Shall prepare method statements and risk assessments and submit to the

Contractor for approval.

3) Carry out site HSE inspection and cooperates with Contractor’s Site HSE

Supervisor.

1.8 HSE Studies, Surveys and Reports

The Contractor will undertake all required studies and survey necessary for the

project. All studies, surveys and subsequent reports will conform to Owner and

prevailing legislation requirements.

Recommendations shall be incorporated into designs, plans and procedure as

necessary to ensure project integrity is maintained.

Studies and surveys performed shall include the following:

Project HSE Reviews (PHSER)

Hazards and Operability Studies (HAZOP)

Final Environmental Risk Assessment (FEIA)

Pre Construction Safety Case

Radiological Surveys

UXO/EOD Surveys

1.9 Orientation and Training

To meet the training and competency requirements for each worker on the

construction site, an orientation and training program will be provided. This will

include Site specific and area specific orientation training and will be complemented

by a Subcontractor on the job training to ensure that each worker is competent to

perform the assigned tasks safely.

All Supervisory personnel will attend Site Supervisor Safety Training.

Each Site Supervisor is responsible for assessing the competency of his crew and

for ensuring that each worker is competent to perform assigned tasks safely. A

mentor system shall be utilized to ensure that each new worker, who is unfamiliar

with site hazards and does not have adequate experience, is under the direct

supervision of a competent worker.

than 60 days

2) Competent Worker

Worker who is adequately qualified and trained and has an experience level to

perform assigned tasks safely and with a minimum level of supervision

3) Competent Supervisor – Experienced, qualified and capable person who has

the skills to safely supervise workers in their area of responsibility

4) Visitor

Any non-project person(s) visiting the plant site and may include but not be

limited to couriers, delivery personnel, regulatory personnel, media personnel

and Contractor Group or Subcontractor personnel not assigned to the site

5) Host

Person directly responsible or authorized to invite a non-project employee to

the plant site

1.9.2 Site HSE Orientation

Site HSE Orientation Training sessions will be mandatory for all personnel working

on the site. All employees, staff, labor, vendors, service agents or other persons

requiring a permanent site access gate pass will undergo this orientation prior to

issue of the permanent gate pass.

This HSE Orientation will be conducted by the Site HSE Manager or HSE

Supervisor with the responsible supervisor(s) who will confirm attendance for all

attendees before authorization for the issue of the permanent gate pass, the

necessary PPE and the Contractor shall maintain these records.

The following subjects will be addressed:

1) HSE Policy and Objectives

2) HSE Rules and Regulations

3) Employee and Supervisory responsibilities

4) Hazard identification and communication

5) Site-specific hazards and precautions

1.9.3 Visitor HSE Orientation

All visitors shall undergo this orientation.

The following subjects will be addressed:

1) HSE Policy and Objectives

2) HSE Rules and Regulations

3) Site-specific hazards and precautions including restricted areas

4) Location of First Aid Station; incident reporting procedures

5) Emergency Response Action Plan (Incident alarms identification)

1.9.4 HSE Training for Supervisors

The Site Supervisor and Foremen have the most direct impact on the safety of the

work site. The Site Supervisor and his Foremen are the key persons for incident

prevention at the work site. They are given direct responsibility and held directly

accountable for, the health, safety and welfare of the work force under their

direction.

In order for these first line Site Supervisors, Foremen, and their counterpart HSE

Supervisors to meet their responsibilities, they will be fully versed in the latest

practices and applications of incident prevention, safe working methods and

incident response techniques.

This HSE training for the Construction and HSE Supervisors will cover the following

specific HSE issues:

1) Familiarization with the site HSE Policy, Objectives and Goals.

2) Site HSE Rules and Regulations (including local regulations)

3) Site HSE Procedures

4) Safety Incentive Program details

5) Basic on-the job training and motivational Skills

6) First Aid, resuscitation and rescue training.

7) Proper use of safety equipment and PPE

8) Incident reporting and investigation procedures

9) Emergency response Procedures and action for his area

10) Methodology of Risk Assessment

The course will be conducted by the Site HSE Manager. Attendance is mandatory

for all Construction and HSE Supervisors and a select number of craft foremen.

A Safety Manual will be issued to each participant containing detailed information

on Project policies, procedures and rules.

1.9.5 On the Job HSE Training

All workers will be monitored for proficiency in their area of skill. This includes our

existing work force and workers who are assigned to new tasks. This also applies to

the Subcontractor work force.

In general, further to the HSE Orientation, the following subjects will be reinforced

on site by Site Supervisors with HSE support;

1) Site Safety Rules and Practices

2) Methods of reporting accidents, incidents

3) Importance and use of PPE

4) Importance of first aid treatment

5) Locations for first aid

6) Emergency Response situations; notification, alarms

7) Response action required in emergencies

1.9.6 Specialized Safety Training

Those supervisors, foremen and craftsmen directly responsible for implementation

of the Work Permit System, Lock-out Procedures, Emergency Response action

plan, Heavy Lift Procedures, etc. will receive special training and instruction in the

specifics of these procedures and systems as they apply to this project.

This also applies to the HSE Supervisors assigned to each area as well as

Subcontractor’s supervisory staff with responsibility in these activities.

The Contractor’s Site HSE Manager shall initiate the following specialized safety

training where determined necessary;

1) Excavation Permit procedures

2) Confined Space Work Permit procedures

3) Heavy Lift procedures; equipment and set-up inspection

4) Emergency Response procedures; action required

5) Fire Response training (first line response)

6) First Aid training (first line care, resuscitation, respiratory care)

7) PPE and Safety Equipment; care and use

8) Scaffolding erection safety

9) Excavation safety

10) Radiographic protection & safety

11) Welding and cutting safety

12) Electrical safety and tag and lockout procedures

13) Vessel Entry, Hot Work etc.

The Site Manager acts as Chairman of the HSE Committee to review and discuss

all matters with regard to Site HSE management and shall convene the meeting.

The HSE Committee shall convene on a monthly basis at the work site. The

designated Secretary shall present the agenda for the Chairman and record the

minutes of meeting, for issue to all members of the HSE Committee.

The HSE Committee shall conduct monthly HSE inspections of the work-site, noting

all items needing corrective action.

The members of the HSE Committee shall include following as a minimum;

1) Site Manager

2) Superintendents

3) Site HSE Manager

4) Administration Manager

5) Subcontractors’ Site & Construction Manager

The major functions of the committee may include:

1) A review of the current site safety statistics and report on incidents, if any.

2) Disseminate information on all Site incidents and near misses.

3) Conduct scheduled safety inspections in all areas.

4) A review of status and effectiveness of the Risk Assessment; confirmation of

ongoing analysis needs and participants.

5) Review status and any action required for the Safety Promotion Program.

6) Review of HSE Training Status and upcoming programs.

7) A review of Site housekeeping status; any needs identified on site.

8) A review of any tools, equipment, supplies or PPE needs identified on site.

9) Review the HSE activities planned & implemented for effectiveness.

10) Establish common safety activity targets and actions necessary to achieve

those targets.

11) Identify hazardous activities and co-ordinate necessary measures among the

parties involved in the work.

1.10.2 Subcontractor Safety Meetings

The meetings will be initiated by the Contractor Superintendants assisted by the

Site HSE Supervisor, and Site Supervisors and will be attended by Subcontractor’s

craft workers and laborers.

The meetings may be divided by craft or work location to avoid too large a group

gathering.

The meeting agenda will consist of the following:

1) Disseminate information on all Site incidents and near misses

2) A review of compliance to Risk Assessment results

3) Presentation of Safety Incentive awards

4) “Town hall” session where workers can ask questions and raise concerns

5) Presentation by one sub contractor on a specific safety or health topic

1.10.3 Daily Toolbox Meetings

Contractor and Subcontractors will hold regular Toolbox Meetings with the project

work force throughout the course of the work. These regular toolbox meetings are

used to address the work force, in the field, on job safety related matters.

Every day, prior to work start, the Foremen will gather their crew(s) at the job site for

a brief review of the day’s work plan and a confirmation of the roster at each work

front. Contractor practice includes a very brief set of exercises. These daily Toolbox

Meetings which normally involve small groups of no more than 20~30 workers, will

typically last no more than 10 to 15 minutes.

These daily morning meetings will also include at least three safety related topics;

1) A review of the specific hazards presented by today’s tasks and the protective

measures to be taken,

2) A review of the Work Permits, if any, that may apply to the days work and

confirmation of the permit requirements and precautions,

3) One topical item or “Safety Moment” which will be given to each crew site wide,

4) Report on Incidents/Accidents including causes and corrective action taken,

5) Site Housekeeping status and required action, etc.

Although conducted by the Foreman or Supervisor in charge, both the Contractor

and Subcontractor HSE Managers and HSE Supervisors will make a point of being

in attendance at one of these meetings every day.

In addition to the “Safety Moment” and HSE Topic for the Month, safety topics of

interest to be addressed at these toolbox sessions will include work-site

housekeeping, working at heights, use of lift equipment, excavation precautions,

permit to work procedures, confined space work, sharing of Near Miss Reports etc.

1.10.4 Bulletin Boards

At least one project bulletin board will be maintained to communicate important

safety, health and environmental information to all personnel on the plant site. The

bulletin board will be full size, weather proof and lockable to prevent unauthorized

use. The bulletin board will be strategically placed in well lit, high traffic and

frequently used area.

All safety bulletins and safety, health and environmental information shall be

approved prior to posting by the Site HSE Manager. Each posting will include a

“posted date” and a “removal date” to make efficient use of the boards. One person

will be assigned to post approved items and to check that all project bulletin boards

are current.

The Site HSE Manager will issue timely Safety Bulletins to summarize significant

issues and unacceptable, major or serious incidents that have occurred on site.

These bulletins will be made available to each Subcontractor’s line management

and will be posted on the site bulletin boards. The bulletins should be added to the

agenda of the daily toolbox meetings or monthly safety meetings.

1.11 Safety Motivation and Incentive

The Contractor’s comprehensive Safety Promotion Program will be used to promote

site safety activities. The program is based on the achievement of project safety

goals and it rewards contributions made by both individuals and groups.

This program is nominally tied to achieving statistical goals based upon OSHA

recordkeeping guidelines. However, it also takes into account manning and field

conditions in different work areas and more subjective assessments regarding

contributions made by the individual, or group, to such items as near miss reporting,

area housekeeping and/or general safety awareness issues.

Awards are given on a monthly basis and include monetary as well as other

incentives. A cumulative points system is sometimes employed to select Quarterly

or Semi-annual group winners who qualify for generous awards; either monetary or

other incentives including, for example, paid time off.

Announcements and notices are used to publicize the achievements of the safety

program, and the award winners, as a way of keeping everyone informed and

involved.

The Contractor’s program also includes a penalty clause that will be assessed on

those individuals, or groups, that commit infractions of the site safety rules,

practices and regulations. Serious infractions are dealt with disciplinary action;

however, minor infractions (or oversights) are also noted and publicized for the

education of the work force.

1.12 Inspections

Safety Inspections shall be carried out to ensure safe working conditions and

evaluate compliance with relevant safety rules, procedures legislation and

regulations for construction site activities. Site safety inspections consist of the

following:

1.12.1 Daily Inspections

The daily inspections are part of the regular workload of the relevant Site

Supervisor who is assisted by the HSE Supervisor as available.

The workplace will be inspected on a routine basis to check that equipment and

procedures are as they should be and that hazards and risks are being adequately

controlled at the work site.

Any non-conformance will be reported directly to the person with the authority to

take action and influence the required action or, failing that, to the Site Manager or

designate.

Subcontractor management, supervision and HSE personnel should also follow this

schedule as a minimum requirement.

1.12.2 Formal Workplace Inspections

Work areas will be inspected weekly by Superintendents, Site HSE Manager, HSE

Supervisor and relevant Supervisors with Subcontractor management.

The Site HSE manager will develop a site specific inspection checklist. The

inspection checklists will be completed for each inspection and the Site HSE

Manager must regularly review and update the checklist as required.

The completed reports including inspection description, inspected area,

participants, inspection result, recommended action and due date shall be

forwarded to the Site Manager and relevant Superintendents for review on

completion of the inspection.

Each superintendent must ensure that corrective action is taken so that the hazard

and/or risk is eliminated or controlled.

Subcontractors shall take corrective actions when their inspection activities identify

deficiencies and, maintain records of inspections and corrective actions taken.

1.12.3 HSE Committee Inspections

An HSE Committee Inspections shall be conducted by Safety Committee members

monthly at minimum. An inspection report shall be prepared and copies sent to the

responsible Superintendents(s), HSE supervisor(s) the Safety Committee for

review.

The responsible area superintendents must ensure that corrective action is taken so

that the hazard or risk is eliminated or controlled.

1.12.4 Regulatory Inspections

Local Government regulatory officials may require access to the project site to

conduct an investigation or an inspection. All visits, whether planned or

unannounced, will be directed to the Site Manager. The overall responsibility for

coordinating these visits will be that of the Site Manager and the Site HSE Manager.

During site visits, regulatory officials will be requested to attend a pre-inspection

meeting and a post-inspection meeting with the Site Manager, Site HSE Manager

and other personnel as designated. The Site HSE Manager will prepare a report

summarizing the visit, findings and the actions that are required. Corrective action

will be completed promptly, regulatory officials will be kept informed and copies of

the report will be issued to CONTRACTOR Project Management and to OWNER

Project Management.

1.13 Safety Rules and Discipline

To ensure that all personnel work safely on the construction site, a wide ranging set

of general safety rules and regulations will be applied. The rules will apply across

the Project and are a reflection of industry standards and best practices. Working

safely is everyone’s responsibility.

1.13.1 Progressive Discipline System

Providing positive reinforcement and recognition for those employees who

consistently comply with site rules and safe work procedures is important. All

Contactor Group and Subcontractor employers are responsible for ensuring their

employees are knowledgeable regarding general rules and regulations. Failure to

comply with the general rules and regulations by an employee on an ongoing basis

is a serious violation and will require application of a progressive discipline system.

A progressive discipline system that is objective and performance based will be

implemented for addressing substandard behaviour by an employee(s).

Management and site supervision will be responsible for monitoring compliance

with site policies, general rules, standards and safe work procedures and then

applying the progressive discipline system as needed.

An effective progressive discipline system must meet the following requirements:

1) Use complete and objective evidence

2) Be based on rules which have been communicated to workers

1.13.1.1 Verbal Warning

At the first level of discipline, the Site Supervisor will meet privately with the

employee to present objective evidence regarding the non-compliance. The general

safety rule(s) that apply will be clearly stated. The employee will be warned, with

due respect for the employee’s dignity, regarding the progressive system that exists

for continued non-compliance. The results of the meeting shall be documented to

the employee’s file.

1.13.1.2 Written Warning

At the second level, the Site Supervisor will again meet with the employee privately

and present further objective evidence. A written warning will be issued to the

employee and then the supervisor and employee will review and sign the written

warning. In the event that the employee refuses to sign the notice, two witnesses

shall be requested to sign the notice to confirm that it was effectively served. The

supervisor shall document the results of the meeting to the employee’s file.

1.13.1.3 Suspension

At the third level, further violation of site policies, general rules, standards and safe

work procedures will result in the suspension of an employee. Suspension is

without pay. The Site Supervisor shall review the suspension with the discipline

superintendent prior to convening a review meeting with the employee. The minutes

of the meeting shall be documented to the employee’s file.

1.13.1.4 Termination

A serious offence may require that the employee be removed from site. This is the

final step of the progressive discipline system and shall require careful review by

Site Management and Site Supervisors prior to taking this action. With their

approval, the immediate supervisor and discipline superintendent will participate in

the dismissal meeting.

A number of serious offences including drinking, use of drugs, sleeping on the job

and theft may require immediate suspension or termination. Refer to “Prohibited

Behaviours”. The Site Supervisor will document the circumstances requiring the use

of this level of discipline. Other reports such as incident, near miss or security

reports may be prepared from a resulting investigation

1.13.2 General Safety Rules

General safety rules will be communicated to all employees and visitors during Site

HSE Orientation. General safety rules include rules that are common to

construction and industrial sites. As needs develop, other site rules may be

evaluated, approved and communicated to project management, subcontractor

management and workers. General rules are as follows:

1) Jewelry including rings, bracelets or neck chains is not recommended as injury

may be caused by catching or entanglement.

2) Shorts, cut-offs and tank tops are not permitted. Shirts must have a long sleeve

3) Use of personal radios or equipment with stereo headphones or earphones is

not permitted.

4) Use of cellular telephones on the site is not permitted. Exception will be made

for a specific purpose as approved by the Site Manager. When approved

phones are in use in a vehicle, the vehicle must be at a complete stop.

5) All photography requires prior approval of the OWNER Project Manager or his

designate. Any picture taken in an operating area will require an approved safe

work permit.

6) All workers must meet the minimum PPE requirements including, approved

safety helmet, approved safety glasses with fixed side shields and approved

steel-toed safety boots.

7) Vehicle operators will strictly comply with all traffic signs, wear seat belts and

obey posted speed limits.

8) Vehicles driven on site must be parked so that traffic flow is not blocked or

excessively restricted. The operator must leave keys in a parked vehicle for

emergency situations

9) All vehicle servicing and wash down will be conducted in approved areas only.

10) Protective eyewear and a hard hat will not be required in an enclosed cab of a

vehicle.

11) Equipment operators in an enclosed cab must wear protective eyewear and a

hard hat only if an injury hazard exists or may exist.

12) A worker(s) will not board or disembark a vehicle or unit of equipment while it is

in motion

13) A worker will not be transported in a vehicle unless approved seating is

available to him. Seat belt must be worn at all times.

14) A signaler shall be used to assist in backing up a vehicle that has an obstructed

rear view.

15) Smoking will not be permitted in common transportation to and from the Project

site.

16) Solvent waste, oily rags and flammable liquids shall be kept in fire resistant

containers.

17) Site waste will be sorted and placed in containers which have been designated

and appropriately located. Burning of waste materials on the plant site will not

be permitted.

18) Domestic waste will be stored in closed containers and regularly removed from

the site.

19) Lunch or eating areas will be kept clean and free of food scraps, containers

and other disposable items.

20) All environmental spills must be immediately reported and investigated with

appropriate cleanup and disposal of the contaminated soil.

21) Adequate sanitary facilities will be provided and must be used as designated.

1.13.3 Prohibited Behaviours

Some behaviours are considered unacceptable and are prohibited. These actions

may lead to disciplinary measures including termination of employment and banning

from the construction site.

As needs develop, other actions that may be prohibited will be evaluated, approved

and communicated to Site management, Subcontractor line supervision and

workers.

1.14 Personal Protective Equipment (PPE)

GS E&C will specify the full range of personal protective equipment and clothing

required to ensure the safety of the work force. As noted, no personnel will be

admitted to the working areas prior to receiving suitable PPE. This applies to

visitors to the site who shall also be issued with the PPE required for their specific

activity.

All PPE will be “purpose approved” for service on the specific task and selected

based on the advice of the Site HSE Manager and Safety Supervisors. The

selected equipment will conform to OWNER’s requirements.

1.15 Safe Work Processes

A hazard identification and control process is a critical part of a safe work

procedures program. It is through the control of hazards that the frequency and

severity of accidents is reduced thereby reducing financial costs and impact on the

worker.

Hazard identification and control strategies on the will include risk assessments,

daily toolbox meetings and work permitting. This process is complemented by the

use of safe work procedures and ongoing inspection programs.

1.15.1 Definitions

1) Hazard

Any condition or circumstance which poses the risk of an incident.

2) Risk Assessment

A risk evaluation of a construction site, project or operation for the purpose of

identifying the actual and potential hazards that exist and the loss control

measures to reduce the hazards to acceptable levels.

1.15.2 Risk Assessment (RA)

A Risk Assessment of the Project Works shall be conducted prior to set up of the

new work site. The Site Manager or designate and each senior Subcontractor area

manager or superintendent shall conduct a hazard assessment of their designated

works to identify, assess and control existing risks or potential risks to people,

property or the environment.

Each risk assessment will identify the actual and potential hazards and will consist

of a three-step process:

1) Identifying the upcoming tasks or work activities

2) Identifying the exposures to people, property and the environment. Property

exposures are related to the equipment, tools and materials used by

employees in completing job procedures and work tasks

3) Identifying the actions required to eliminate or, mitigate risk to acceptable levels

using ALARP principle (as low as reasonably practicable)

Each risk assessment will establish a preliminary safe work plan for the jobsite and

identify those jobs and tasks with additional risk. Those identified as high risk jobs

will require further detailed risk assessment. Updating of the risk assessment will be

completed regularly as additional work plan details become available. Regular

updates will allow line supervision time to prepare job work plans and obtain the

necessary equipment, tools and materials for upcoming jobs.

Risk assessment should be performed for high risk activities including, but not

limited to the following:

1) Confined Space Entry

2) Trenching or Open Excavations

3) Electrical Isolation

4) Encroachment on Overhead Electrical Systems

5) Encroachment on Underground Utilities

6) Critical Crane Lifts

7) Hazardous Materials Handling

8) Work in Process/Operating Areas

9) New jobs with unknown hazards

10) Non-routine jobs or tasks

11) When after preliminary analysis, an existing procedure or plan does not

adequately cover the work

12) Deviation from accepted work practice or existing safe job procedure

13) New equipment and machinery procedures

14) Jobs or tasks with repeating loss incidents

The completed RA must be reviewed with work crew members before a job or task

begins, at daily toolbox meetings or at a weekly safety meetings. Deviations from

accepted work practices will require completion of a RA with approval from the next

higher level of supervision. Contractor will maintain comprehensive records of all

RA activities.

1.15.3 Work Permit

The contractor and all subcontractors will strictly comply to all OWNER Work Permit

requirements while operating in OWNER controlled areas.

1.16 Incident Reporting and Investigation

1.16.1 The Contractor follows the GS Incident Reporting and Investigation Procedure

(GMP-178-006) and OSHA (Occupational Safety and Health Administration)

procedures for the maintenance of occupational injuries and illness records for

internal purposes. During the course of the project the Contractor will comply to

Owner reporting requirements and cooperate with any Owner investigation

activities.

1.16.2 Project Incident Reporting

Owner Incident Reporting requirements shall be met as follows:

1) Notification

All incidents, including near miss incidents shall be reported to all concerned

parties on the site and the local authorities if required, as soon as possible, in

order for the correct response action to be taken.

The verbal alerts and calls for assistance will be carried out as required in the

Emergency Response Plan for incident action and KOC.GE.007 HSE Incident

Reporting Procedure. Immediate reporting will include alerting the Owner local

Fire Station, Site Management Team and shall be done within several minutes

of the occurrence.

2) Initial Incident Report:

The HSE Manager will issue an Initial Incident Report on all incidents by

completing the KOC Accident – Incident Report Form 1: KOC.GE.007A.The

report will be issued to the Site Manager and will indicate the action being

taken regarding investigation and full reporting. The Site Manager will also

report to the Owner, Project Manager and Project HSE Manager at Head Office

as follows;

Minor incidents (level 1) – within 24 hours of incident occurrence.

After completion of the investigation and analysis, the HSE Manager will issue

the Investigation Report to all concerned parties where it will be the subject of

review for approval and implementation of required corrective action on an

urgent basis. The Site HSE Manager will issue the completed Investigation

Report to the Site Manager, Owner, Project Manager, and Project HSE

Manager at the Head Office. This is typically done within 4 days of most

incidents.

1.16.3 Reporting Responsibilities

1) Employees

Every employee must report any incident in which they are personally involved

or have observed to their immediate supervisor..

2) Safety Supervisor or Site supervisor

The responsible area Safety Supervisor or Site supervisor must report an

incident occurring during his working hours as soon as possible, both to obtain

assistance and take emergency action and to alert the proper members of the

site team. They must also complete an incident report, recording their

observations and actions as soon as feasible. This must be completed and

submitted to HSE Manager and their immediate supervisor before leaving the

site.

3) Foremen and Witnesses

The area Safety Supervisor shall arrange to record the observances of the

foremen or other witnesses, or assist them in making reports of record. These

are to be submitted to Site HSE Manager.

4) Site HSE Manager

Site HSE Manager will collect the reports related to an incident. He will

maintain records in accordance with this plan and report the incident to the Site

Manager who will report to the Owner and GS E&C Head Office as per

procedure.

1.16.4 Investigation Procedure

The investigation process includes an objective evaluation of all the facts

statements, physical and medical evidence and related information which can be

assembled for analysis.

The quality and usefulness of the information is directly related to the degree of

thoroughness and conscientiousness of the investigation team. The key objective is

to find the root cause(s) of the incident so that proper steps can be taken to prevent

recurrence

Under the guidance of the Site HSE Manager, due care will be taken during the

course of the investigation to insure that the investigation and analysis process is

directed towards this end and that an action plan or steps to prevent or control a

similar recurrence is developed as expeditiously as possible.

1.16.5 Investigation Committee

As noted, an Investigation Team is formed for all incidents. The participants will vary

depending on the type of incident with the key participants being the person(s)

directly involved the line Supervisor for the area, the HSE Engineer, the area Safety

Supervisor and any key witnesses.

Other personnel such as Discipline Managers, Subcontractor Management and

Supervisory personnel as well OWNER’s HSE personnel may be involved

depending on the magnitude of the incident. Any person who can contribute to the

investigation can be called on to be a member or give testimony.

Organizing a division of responsibility toward investigations should include a broad

range of experience, responsibility, and authority. The HSE Manager has full

authority to call upon any personnel he deems necessary for the investigation.

Participation by an individual so called is mandatory.

1.17.4 Definitions

1.17.4.1 Health assessment

Applying appropriate procedures and tests in examining an individual to enable a

medical professional to decide whether he or she is fit for the specific working and

Where a person has a condition under circumstances whereby the condition would

cause the person to be a safety or health hazard to him or herself or to others and,

where the condition cannot be controlled.

1.17.5 Medical Facility Operations

1.17.5.1 Medical staff

1) A professional Medic shall be available at the site Medical Facility at all times

while site work is in progress.

2) The medical staff should be familiar with local medical conditions and have

broad general experience.

3) The medical staff should have good communication skills and be familiar with

local medical facilities that could be used for further referral.

4) The medical staff shall be capable of rendering first aid or emergency treatment

for any industrial injury, illness or exposure to toxic substances being used on

the work site.

1.17.5.2 Medical Facility and Equipment Requirements

1) The normal equipment for conducting consultations, physical examinations and

minor treatments will be in place. Such equipment would include:

1.17.5.3 Administration

1) The Medical Facility shall be open to treat ill or injured persons at all times

when work is in progress on site.

2) The Site HSE Manager shall provide information on use of Medical Facility

services to all staff and sub-contractor personnel.

3) The Medical Staff shall perform diagnosis and treatment for a patient. If the

symptoms or conditions of illness/injury of the patient are serious and beyond

the capability of the Medical Facility, the patient will be sent to the designated

local area Health Centre or Hospital after receiving basic medical treatment for

their ailment.

4) The Medical Staff will record the results of diagnosis, symptoms and treatment

for each patient by paper file or computer files.

5) The Medical Staff shall replace the used medical supplies in the Medical

Facility and maintain the inventory of first aid kit status on site daily.

6) The Site HSE Manager shall liaise with local hospitals and clinics and develop

an action plan for the expeditious handling of serious injuries or accidents. This

plan must detail communication lines, transport arrangements etc.

1.17.6 First Aid Training

1) First aiders will be trained in first aid in order to support the Medical Staff in

case of multiple injuries or simultaneous accidents in different places of work.

2) These personnel will receive basic training in first aid at the work site, in

particular for electrocution, burns, fractures, heat stroke, bleeding and CPR.

3) All first aiders shall be provided with identification for first aider e.g. green

helmets with a first aider crest on the helmet/vest and high reflective vests.

1.17.7 Medical Examination

1) Medical examination of all employees shall be conducted prior to their starting

work at the site. This examination shall include an assessment of the

employee’s fitness for work, note any pre-existing conditions, and screening for

intoxicants.

2) The Contractor shall ensure that all personnel under Contractor’s supervision

are not in an intoxicated state from the influence of alcohol, chemicals, or

drugs.

3) Persons found to be in an intoxicated state will be removed from Site and

forbidden to work further on the Project.

4) Contractor shall keep a record of the blood type of all its employees.

5) Medical staff shall record all cases seen or treated by them. Records shall

include:

1.17.8 Sanitation

The Contractor ensures that employees use the temporarily installed sanitary

facilities on the site. The facilities must comply with statutory regulations, be

reasonably accessible from all workplaces on site, and must be maintained to an

acceptable standard of cleanliness at all times.

Site messing facilities shall be operated and maintained to international standards

by approved and licensed catering companies.

Each Subcontractor is held fully responsible for seeing that his personnel abide by

the applicable sanitary requirements and proper use of the onsite facilities. The

Contractor will ensure, where required, subcontractors provide and maintain toilet

and washing facilities at the construction site for common use, including sanitary

and washing facilities in their work areas such as workshop, lay down areas, etc.

1.18 Emergency Response

1.18.1 Purpose

The object of this plan is to safely evacuate all personnel from the immediate area

of the emergency situation and reduce the severity of loss in the safest manner

possible.

1.18.2 Scope

To be used by all members of the Project Management Team, Supervisors and

Subcontractor personnel who have a duty of care to protect and assist all members

of the workforce in the event of any emergency situation.

It is not expected that any person put themselves or other persons at risk to contain

an emergency situation.

is required.

1.18.5.1 Site Manager

1) Provision of adequate resources in terms of personnel, time and finance for

implementation of the requirements for providing an emergency response

service on site.

2) Acts as, or appoints the Incident Controller.

3) Active participation in the scheduling and enactment of drills and exercises.

4) Shall monitor the arrangements through the HSE manager to satisfy that the

services are satisfactory and adequate for the needs of the project.

5) After an emergency and before activities resume, the Site Manager,

Superintendents and HSE Manager shall check the damage and loss to

facilities and also take measures to rectify any unsafe conditions.

1) The Site HSE manager is responsible for ensuring that provisions are in place

for emergency response, including:

Arrangement, through site management for the resources for conducting

head counts.

Identification and mobilization of emergency response teams.

Training of emergency response teams.

Setting up drills and exercises in coordination with the local emergency

services.

Advise on procurement of fire and medical equipment.

2) In the event of any emergency the Site HSE manager or HSE Supervisor shall

take the following actions and provide full assistance to the Incident Controller:

Evaluate the scale of the incident, determine whether additional

resources are required to adequately deal with it and advise the Incident

Controller accordingly.

Liaise with site supervision for withdrawing any permits.

Coordinate activities as directed by the Incident Controller with the site

emergency response teams.

Make safe the area by placing barriers or other means of preventing

unauthorized access.

Prepare a full report.

1.18.5.3 Incident Controller

The incident Controller shall be identified and shall have the following

responsibilities: (The senior person present at the site of an incident shall act as

Incident Controller, until the designated Incident Controller can take charge.)

1) Shall take overall charge of any emergency or exercise.

2) Shall attend the site of an incident, assess the situation and issue direction to

the concerned parties and supervision of the emergency response team.

3) Ensure that messages have been communicated to the outside local

authorities and concerned parties and supervision of the emergency response

team.

4) Count for all personnel and assign search/rescue teams in the event of a

missing person.

5) Ensure that any mutual aid agreements are followed and assistance is sought

in time.

6) Informing the Site HSE Manager of the emergency at the earliest opportunity.

7) Direct site supervision for the mobilization of any plant and equipment

necessary for dealing with the emergency.

1.18.5.4 Fire Fighting Team

1) Fire fighting teams consisting of Contractor and Subcontractor personnel will

be established and trained to deal with most eventualities.

2) The Incident Controller shall appoint fire team leaders who will give direction to

personnel under his control. Personnel appointed will have had some training

and where possible previous experience in emergency response actions.

3) All the members of the fire team shall report to the fire team leader and shall

work according to his direction. The operation on fire extinction shall be taken

according to the fire fighting procedure laid down for which training shall be

given at regular intervals.

4) An electrician/instrument technician will also be included as a member of the

fire squad with responsibility for the isolation of electrical power if necessary.

5) The maintenance of fire fighting appliances shall be the responsibility of the

Site HSE Manager, who shall make arrangements to recharge the used fire

extinguishers or other equipment.

1.18.5.5 Rescue Team / First-Aid Team

1) Rescue teams shall be fully trained in different rescue techniques and

procedures e.g. removing a victim from electrical sources, different types of

handling techniques, the ABC’s of first aid etc.

2) The team shall be responsible for recovering a casualty and transporting them

to the Medical Facility without aggravating injuries.

3) The first-aid team shall also assist the nurse/medic on duty in rendering

medical aid to injured parties. If nursing staff is not available, the leader of the

first-aid team shall take charge by administering first aid and sending injured or

sick person(s) for further treatment to the company medical centre or local

hospital.

1.18.5.6 All Personnel

All personnel should familiarize themselves with the locations of all emergency exits

from their work area and the location of their nearest Assembly Point.

1.18.6 Instructions

1.18.6.1 General

Any emergency occurring on construction sites or at accommodation camps can

have serious consequences: people are injured or even killed, buildings/structures

are destroyed; plant and equipment are damaged; production is held up and

completion dates are not met.

Such situations demand adequate protection, with rescue and relief measures to

handle such events safely, quickly and efficiently.

It is therefore necessary to ensure that all persons on site and at camps are aware

of what they have to do in the event of an emergency and response procedure the

responsibilities and duties of key personnel must be made.

Analysis of the emergency response procedure must be carried out by practical

training complete with a program of regular drills and exercises.

The implementation of an emergency and response procedure is not a substitute for

maintaining high standards of HSE requirements.

1.18.6.2 Evacuation Procedure

1) In the event of a fire, major plant failure, explosion, bomb threat, H2S release

or the need to evacuate the construction site, the actions listed below should

be followed:

On the continuous sound of the alarm siren (bells), STOP all activities

and vacate the building or area without delay, by the nearest exit.

Move quickly, but do not run.

Do not return to a work area to collect belongings.

Keep left in corridors and stairs.

Do not overtake others along the route.

Assemble at the designated Assembly Point.

At the Assembly Point report to the responsible warden.

Do not enter the building or work area under any circumstances until the

all clear is given.

2) All new staff, as part of their induction, shall be given instructions on the

evacuation procedures for the site.

3) All visitors and contractors are to be advised of the Site's Evacuation

Procedure and the location of Assembly Point when they are admitted to the

site.

1.18.6.3 Assembly Points

Designated Assembly Areas will be marked on site layout maps, and be displayed

on bulletin boards throughout the site next to the Evacuation Procedure.

1.18.6.4 Incident Completion

All clear signals shall be sounded by the incident controller following a thorough

examination of the area, initiated by the Incident Controller and other team leaders.

The Project Management & Site HSE Manager shall visit all locations to ensure that

the duties assigned to the responsible parties are understood and adhered to.

1.18.6.5 Training and Exercises

Exercises in each facility shall be conducted at least once every three months,

observations shall be recorded by the Site HSE Manager and any remedial actions

required shall be identified. The report should be forwarded to the Site Manager,

OWNER HSE representative and all concerned parties.

The Emergency Response Plan may be updated from time to time depending on

the observations made.

Training shall be conducted on regular basis for the teams involved in the

emergency procedures.

Training and information via notices in relevant languages will be provided for all

persons working on site and at camp accommodations.

1.18.7 Raising an Alarm and Communication

Communication is a critical factor in handling an emergency. To control the situation

by the earliest possible action, any employee must be able to act and raise an

emergency alarm.

On Hearing an alarm:

1) All personnel shall vacate the building or work place. Before leaving, the

individuals concerned shall leave the work place as safe as possible e.g.

switching off of welding machines, gas cylinders, running machines, gas

burners, any electrical light or appliances in their rooms.

2) All personnel shall assemble at the assembly point according to the posted

evacuation instruction. Fire wardens shall provide guidance and assistance for

mustering at the correct assembly point.

3) All the members of respective teams shall report to their leaders.

4) The leaders shall take direction from the Incident Controller.

1.18.8 Fire Prevention and Fire Fighting

1.18.8.1 General Instruction

1) Materials and equipment shall be maintained in an orderly manner that reduces

or prevents the possibility of fire spread.

2) Materials shall not be stored in a manner that obstructs fire points, sprinkler

heads, alarms, emergency exits, electrical panels and walkways.

3) Materials will not be stored close to, or in a manner that conceals, floor

openings or hoist way.

4) Combustible and flammable materials shall be stored in compliance to

Flammable and Combustible Liquid Storage, Hazardous Chemical Use and

Materials Storage and Handling.

5) Doors provided for emergency escape will open outwards in the direction of

travel.

6) Equipment will not be fuelled while the engine is running.

7) Smoking is prohibited while refueling activities are taking place.

8) Incompatible materials will not be stored in proximity to each other.

9) Smoking is prohibited onsite in the work areas. Smoking will be allowed only in

designated shelters where, safe receptacles shall be provided for smoking

materials.

1.18.8.2 Waste Disposal

Accumulations of combustible waste material, dust, and debris shall be removed

from the site and its immediate vicinity at the end of each work shift, or more

frequently as necessary for safe operations. Good housekeeping shall be

maintained, and access routes will be kept clear at all times.

1.18.8.3 Fire Alarm Reporting

Instructions shall be issued to notify the Contractor Site HSE Manager or

Supervisor immediately in case of fire, including those already extinguished by

construction site personnel.

1.18.8.4 Access for Fire fighting

All-weather roads for fire fighting equipment shall be maintained, kept clear of any

obstructions.

1.18.8.5 Flammable Liquid Use

Flammable or combustible solvents shall not be used for cleaning purposes without

specific instructions from the supervisor and such instructions shall include the site

and conditions of such use.

Gasoline and similar flammable liquids will be stored in secure approved containers

and in a designated storage area. Keep all heat sources away from combustible

liquids, gases or other flammable materials. When not in use, materials shall be

removed immediately from the construction site.

Fire fighting equipment must be kept free from obstacles, equipment, materials and

debris that could delay emergency use of such equipment. Portable fire

extinguishers of the appropriate type, which will be positioned as required

throughout the site buildings and work areas, fire blankets, sand buckets and water

barrels will be placed around the site to suit the work/fire hazard type.

All equipment will be regularly inspected, and arrangements for maintenance made,

by the HSE Team. This equipment will not be used for any other purpose, except for

fire fighting. No one will be allowed to tamper with the seals, labels, operating

instructions etc., wherever provided on the equipment by the HSE Team.

The boxes containing fire-extinguishers, hoses, blankets etc. shall not be used for

storing any other material.

1.18.8.7 Portable Fire Extinguishers

Portable fire extinguishers shall be available throughout the Site within 15 meters of

the following:

1) Work involving welding, burning or use of open flames

2) At the entrance to all buildings

3) On all equipment having diesel or gasoline engines

4) At all fuel and combustible materials storage areas

5) Other areas as required by Risk Assessment

1.18.9 Toxic Releases and Other Airborne Releases

In the event of a major toxic substance airborne release, smoke or hazardous

emissions from nearby facilities, the steps listed below shall be followed.

1) All personnel to vacate the area to upwind Assembly Areas immediately.

Remember to observe the direction of the wind socks.

2) Roll call taken to ensure all persons accounted for.

3) Essential staff will wear appropriate personal protective equipment, enter the

area, shutdown equipment and evacuate any injured personnel.

4) If the toxic substance concentration or other hazardous emissions remains

above Occupational Exposure Limits, consider initiating evacuation of site.

1.18.10 Explosions

In the event of an explosion at the site:

1) Initiate evacuation procedure.

2) All staff to assemble in Assembly Area.

3) Account for all personnel, visitors, contractors.

4) If fire, initiate fire procedure.

5) Incident Controller to initiate the Emergency Response Plan to minimize plant

damage, if able to do so safely.

6) Await further instruction from the incident Controller or designate.

1.18.11 Chemical / Fuel Spills

1.18.11.1Ensure personnel are safe

1) Report the incident immediately to the Site Supervisor, foreman or initiate the

Emergency Response notification alarm if necessary.

2) If personnel are injured and can be removed from the area safely, do so.

3) Check Material Safety Data Sheet (MSDS) as to hazards and first aid

measures.

1.18.11.2Stop the flow of leaking material

1) Reposition the drum to stop the flow.

2) Shut off the source of the leak.

3) Equipment that is found leaking should be removed from service as soon as

possible.

1.18.11.3Contain the spill

1) Bund and protect all open grates, sumps, manholes that discharge to into the

sewer system.

2) If the chemical has entered the drains, advise supervisor and environmental

Supervisor immediately.

3) Dyke major spills with soil or other material.

4) Prevent spilled material entering drains, waterways or on to the ground.

5) Where it is not possible to remove equipment from service immediately, some

means of collecting the leaking material should be used, such as metal trays,

buckets, polyethylene sheeting.

1.18.11.4 Protection of Personnel

1) Personnel entering the leak or spill area shall be provided with and use

appropriate protective equipment, as set out in the Material Safety Date Sheet

(MSDS) for that particular chemical.

2) All non-essential personnel shall be kept out of the immediate leak or spill area.

1.18.11.5 Actions of Responsible Person

The foreman and/or supervisor shall assess the situation quickly to determine the

need for external help.

1) Minor spills or leaks can be cleaned up using absorbent material, sand, or

sawdust. Check MSDS to see what the correct material is.

2) All liquid and contaminated material must be collected for disposal.

3) Transfer contents of leaking drum to new clean drum.

4) Ensure collected material is placed in a container which will not leak during

transportation off site for disposal.

5) Check MSDS that disposal route is appropriate, e.g. to landfill.

1.18.11.6 Preliminary Incident Investigation

1) Supervisor to prepare a short report on the incident.

2) Comment on contingency plans; Were they effective in dealing with the

situation?

3) If any improvements identified, inform the personnel responsible.

1.18.12 Vehicle Accident

In the event of a vehicle accident, the actions listed below should be followed:

1) Send for help if anyone is injured.

2) Try to prevent other accidents from happening by warning other vehicles of the

vehicle accident by placing accident hazard warnings in the area.

3) Administer first aid to injured person.

4) As soon as possible, record details of the accident.

5) Report accident to Site Manager as soon as possible.

6) Site Manager shall immediately inform the OWNER and others as required.

1.18.13 Bomb Threat

1) Treat all bomb threats seriously.

2) Investigation may reveal a hoax, but until certain there is no risk, act with

extreme caution.

3) Telephone threat.

Keep calm. It is not possible to think clearly when panicking.

If possible, attract the attention of another person, and have that person

contact the police.

Delay the caller. The more the caller talks, the more chance there is of

obtaining useful information.

Try to elicit as much information as possible.

Note: Any characteristics of the caller, e.g. sex, accent, speech (fast, soft,

well-spoken, etc), background noises (music, street noises, aircraft, etc.)

If caller hesitates for more than a few seconds, ask another question.

4) When a caller hangs up:

Do not hang up, keep the line open.

Evacuate premises if there is only a short time until stated explosion time.

5) Suspicious object found:

Leave the object alone. Do not interfere with it in anyway.

Do not give a shock, or move the object.

Do not cover it with water, or put water on it.

continuously integrate environmental protection measures into construction

activities.

The Site HSE Manager is responsible for coordinating and managing all the

environmental activities during the construction phase. The HSE Manager

environmental responsibilities in general are given below, but not limited to:

maintained across the Project Site activities.

Management and disposal of wastes

Control of noise and dust pollution

Sewage water and drainage control

Air and water pollution control

2) Develop and maintain environmental documents (e.g. project environmental

plan and control plans or procedures) and records.

3) Monitor adherence to the project environmental policy & objectives, alerting

management of non-compliance, and providing advice on remedial actions,

through environmental audits, reviews, inspections etc.

4) Monitor and verify closeout of actions arising from environmental audits.

5) Report periodically to the project management on the performance of the

project environmental management system.

6) Include Environmental items in HSE action item tracking to ensure all actions

from investigations, inspections and risk assessment are completed in a timely

manner.

1.19.3 Dewatering Activities

Ground water and surface water drainage and diversion activities shall be controlled

in accordance with the approved dewatering plan. Water diversion into sewer

systems, drainage ways, and settling ponds shall require OWNER Environment

Dept. approval.

Dewatering discharges into the natural water system will require prior approval of

government regulators, the OWNER Environment Dept and may require water

sampling tests by the OWNER Environment Dept. or an approved agency. A record

of water samples will be and water test results will be maintained as per the

approved dewatering plan requirements.

1.19.4 Dust Control

During dry conditions, excessive dust may be generated form the work site and

access roads. To minimize the nuisance impact on surrounding communities of dust

generation during construction activities, the Contractor shall carry out the following

activities:

1.19.4.1 Material Handling

The height from which excavated materials are dropped shall be controlled to a

minimum practical height to limit the fugitive dust generation from unloading.

1.19.4.2 Vehicle Dust

1) Effective water sprays shall be used on the site to dampen potential dust

emission sources such as unpaved areas used by site traffic and active

construction area.

2) Materials transported by vehicles shall be covered entirely by clean impervious

sheeting, to ensure that the materials do not leak from the vehicle.

3) In order to prevent earth movement from the site due to construction activity

and run off into public roads, drains, the site shall be inspected regularly.

4) On-site vehicle speeds shall be controlled to reduce dust re-suspension and

dispersion by traffic within the sites.

5) By using a water-spraying tanker upon the unpaved road, the generation of air

borne dust can be prevented.

1.19.4.3 Excavation

1) The working area of any excavation or earth moving operation shall be sprayed

with water before and after the operation so as to maintain the entire surface

wet.

2) The amount of soil exposed and the dust generation potential shall be kept as

low as possible, this can be accomplished by surface compaction, temporary

fabric covers, minimizing the extent of exposed soil and the prompt re-

vegetation of completed earthworks.

3) Excavated soil and rock which will be re-used for back-filling will be stored in

the designated area and surplus soil and rock will be transported to a

designated site for final disposal.

1.19.5 Non-hazardous waste

To maximize the internal and external recycling of materials and to minimize the

volume of solid waste requiring disposal by landfill, solid waste control shall be

implemented during the construction activities.

1) The system to contain, store and remove waste materials from construction

activities shall be clearly established and implemented.

2) The waste shall be disposed to OWNER designated location or suitable dump

area as advised by OWNER.

3) Contractor shall ensure that the waste control subcontractors are suitably

qualified and equipped to manage the waste generated during construction.

4) No on-site burning, burying or dumping of any waste materials, vegetation, litter

or refuse will occur.

5) The waste skips/containers holding the waste material to be clearly labeled for

easy identification of material.

6) Non-hazardous wastes should not be mixed with hazardous wastes at any

time.

1.19.5.1 Excavated inert materials

1) Excavated materials are not considered likely to cause adverse impacts, since

they may be possible to be used as reclamation fill, which is considered a

useful reuse of the material. Any uncontaminated inert material may be

delivered to a designated fill site approved by the owner.

2) Surplus excavated material, quarry overburden, rock rejected for aggregate,

and aggregate surplus to the requirements shall not be discarded

indiscriminately.

1.19.5.2 Construction and demolition waste

1) Careful planning and good site management can minimize over ordering and

waste of materials such as concrete, mortars and cement groups. The design

of formwork could maximize the use of standard wooden panels so that high

reuse levels can be achieved. Alternatives such as steel formwork or plastic

facing could be considered to increase the potential for reuse.

2) Disposal of construction waste can either be at a specified landfill, or a public

disposal site.

1.19.5.3 Non-hazardous waste storage

1) Suitable storage area (adequately designed to protect from rains and to

prevent any run off) and collection skips to be provided for segregated

collection of wastes. The sizing of such areas and skips shall be in accordance

with the expected waste quantities and the frequency of disposal.

2) The waste skips/containers holding the waste material to be clearly labeled for

easy identification of material.

3) Non-hazardous waste should not be mixed with hazardous wastes at any time.

1.19.6 Hazardous Waste

The management of hazardous waste causes number of problems in practice. The

wide range of materials and chemicals involved such as oil, lubricants, cutting oils,

sludge, paints etc. Hazardous waste shall be identified, classified, handled and

disposed of safely.

Hazardous Waste will be collected by each waste-generating subcontractor in

designated containers. Hazardous waste containers will be selected to be able to

maintain their integrity for numerous transportation cycles and satisfactorily hold the

waste for an extended period of time while awaiting final disposition

The Contractor shall complete all required documents for every truck load of

hazardous waste sent for disposal.

A copy of the completed documents shall be forwarded to the OWNER.

1.19.6.1 Types of hazardous waste:

1) Fuel, waste oil, paints and oil contaminated material

2) Corrosives, including acids and alkalis

The storage area for hazardous chemical waste shall;

tested and disposed as chemical waste if necessary)

4) Be arranged such as to separate incompatible materials

Disposal of chemical waste shall;

1) Be treated by a licensed waste collector

2) Be transported to a facility licensed to receive chemical waste, such as a

chemical waste treatment facility

3) Hazardous waste shall not be dumped onto the ground, into storm sewers,

seawater, or into a sanitary sewer system

1.19.7 Transport, Loading, Unloading Chemicals

1) During chemical loading operations, minimize release of chemical spills and

vapours to the environment by use of proper devices.

2) The Contractor shall strictly follow the guidelines provided in the MSDS of the

specific chemicals being handled and provide all PPE and safety equipment.

3) Refer to Hazardous Chemical Use;

1.19.8 Noise and vibration nuisance

During construction, excessive noise and vibration from construction equipment

may become a nuisance to nearby residents. Noise emissions from construction

sites can be minimized through good site practice, selecting quiet plant and quiet

working methods and through the use of temporary barriers. To reduce impact of

noise to the residential area from operation of vehicles and equipment for site, the

speed of vehicles and other equipment shall be limited and noise from operation of

the equipment shall be minimized.

Noise levels generated during construction shall not exceed the levels stipulated in

OWNER standard.

1.19.8.1 Noise Control

1) Mobile plants shall be sited as far away from noise sensitive receivers as

possible.

2) Construction equipment / machinery and vehicles shall be maintained in good

working condition. The operators of all vehicles strictly follow traffic signs and

regulations.

3) High noise generating equipment and generators shall be kept within

enclosures wherever practicable.

4) PPE shall be provided to workers in high noise areas

5) Equipment will be turned off when not in use.

6) Pumps, generators, compressors and saws will be positioned in sheltered

locations as necessary.

7) Equipment will be operated within specifications and capacity.

1.19.9 Spill Control and Cleanup

To minimize negative environmental impacts through rapid and effective

containment and clean-up actions, pollutant spill control procedures shall be

implemented.

1.19.9.1 Containment of spills

1) All equipment that leaks onto the ground shall be repaired immediately or

removed from site.

2) If servicing and fuelling of vehicles and equipment takes place on site, this shall

occur at dedicated and properly equipped facilities.

3) If diesel or oil refueling is to take place on the construction site then the surface

under the refueling area shall be protected against pollution to the ground or

seawater.

4) Changing oil or lubricants is prohibited any where on site except at designated

locations, except if there is a breakdown or an emergency repair where drip

pans and absorbent materials can be used.

1.19.9.2 Spill Clean-up

1) Emergency procedures shall be developed and communicated to all personnel

such that they are aware of the procedures to be followed for dealing with

spills. The procedures shall include identification or responsible personnel and

reporting procedures, contact details of emergency services, etc.

2) Procedures detailed in the MSDS for hazardous materials shall be followed in

the event of an emergency situation.

3) Any waste water or spilled fuel collected within the bund around the refueling

area shall be disposed of as hazardous waste.

4) If spillage of materials occurs during transportation, spillages shall be cleaned-

up immediately and transported to the shelter for possible use or disposed of at

the appropriate disposal facilities.

1.19.10 Training

Environmental training shall include:

1) Chemical waste and sewage hazards

2) Health and hygiene precautions

3) PPE requirements, use and care

4) Spill cleanup

1.19.11 Environmental Incident Investigation and Report

The Contractor shall identify record, investigate and analyze any environmental

incidents that occur on site following normal incident investigation practices. The

investigation and analysis shall identify the root cause and contributory causes of

the incidents and formulate measures accordingly to prevent future recurrence of

similar incidents.

The investigation of the incident or non-conformance shall include:

1) Identifying and handling any legislative compliance issues associated with the

environmental incident or non-conformance.

2) Identifying root causes and taking appropriate action to manage the resulting

environmental impact occurring from the incident or non-conformance.

3) Initiating any corrective and preventive actions and confirming effectiveness of

actions taken. Any actions taken will be appropriate to the magnitude of

problems and commensurate with the environmental risk.

4) Recording required actions for follow up and close out tracking.

1.20 Security controls

1.20.1 Purpose

The purpose is to limit and control access to the Site to authorized personnel,

equipment and vehicles and to provide secure conditions and to prevent injury,

theft, or damage.

1.20.2 Scope

This plan applies to personnel and equipment belonging to the OWNER,

CONTRACTOR and its Sub-contractors to protect all project assets from injury,

damage, theft and entry by unauthorized personnel entry to the project site.

1.20.3 Definitions

1) Authorized Escort

The CONTRACTOR or Subcontractor representative who has completed site

orientation and has the authority to accompany visitors.

2) Host

The CONTRACTOR or Subcontractor representative who has been authorized

to invite non-project personnel to the project site.

3) Security Badge

A card bearing the holder’s name, ID No. Valid Period, photo etc.

4) Shall ensure Security Guards are carrying out their assigned duties.

5) Recruiting and training additional security personnel as necessary.

6) Provide information on security requirements to Subcontractors and supplier

personnel.

1.20.5.4 Security Guards

1) Shall be in attendance 24 hours a day in the security facility at the entry gate.

2) Shall patrol site during off work hours and at other times as required by Site

Manager.

1.20.6 Rules

1) Personnel under the effect of alcohol or drugs are not allowed to enter the site

areas.

2) Matches and cigarette lighters are not allowed to enter the hazardous areas.

Visitors are requested to deliver, upon their entry, any matches and lighters

they may be carrying, to the guard on duty at the gate and to take such again,

on their departure.

3) No one carrying intoxicating or alcoholic liquors or substances is allowed to

enter the site area.

4) Security guards under the control of Security Supervisor to patrol warehouse,

office, storage yards, etc., for 24-hours a day which are enclosed by the

security fences.

5) Project Parking and staging areas outside the fenced areas are also subject to

security patrols, checks and rules

6) Photographing and carrying cameras are prohibited the site without approval

from the OWNER and Site Manager.

1.20.7 Site Access Control

1.20.7.1 Site Admittance

The Site shall be totally enclosed with a 2.4 meter perimeter security fence and

controlled access gates for personnel, vehicles/equipment and materials under

Contractor’s control.

Off-Site storage/warehouse areas shall also be totally enclosed by a security fence

and access. Access to office, warehouse, and storage areas shall be strictly

controlled

A security service Subcontractor will provide the personnel, vehicles and equipment

for security and guarding duties on the site perimeters, at access points and where

applicable, restricted internal areas.

A main Security Station shall be located adjacent to the main gate.

The Site shall have gates for the entrance and exit for all Site personnel, materials

and equipment required on the project.

No one shall be allowed to enter site or camp areas unless they show valid

identification or approved Security Badge.

All personnel included visitors shall be issued temporary badges that must be

displayed at all times in order to enter and remain on site and camp areas.

1.20.7.2 Personnel Control

The Security Team shall issue to all site personnel numbered identification badges

that each employee must wear upon their clothing.

The security supervisor must maintain a close control of all the badges issued.

A badge status list shall be maintained and status changes must be submitted to

Security Team whenever a new employee begins work or an employee is

terminated.

The badge system will be in line with the Induction Training Program. Personnel

who pass Induction Training will be issued with badges.

Additionally, all personnel, equipment and materials shall be subject to the Owner’s

security requirements.

1.20.7.3 Visitor’s Control

All visitors must report to the Security Station.

The guard shall perform the following:

1) Check identification.

2) Request visitor’s signature in the Visitor’s Register.

3) Record the name of the Host that the visitor wishes to visit.

4) Notify (by telephone/radio) the Host that the visitor wishes to meet to determine

if that person is available.

5) Check current visitor listing to determine if the visitor will be allowed on the site

without escort.

6) Issue visitor’s badge

7) Issue PPE, as required.

8) Provides safety orientation covering emergency alarm, exit route, assembly

point and general safety instruction.

If unescorted visitor, direct the visitor to the correct location. If visitors are to be

escorted they must wait at the security station until Authorized Escort arrives.

When the visitor returns to the security station to leave, the guard shall perform the

following:

1) Log time out on Visitor’s Register

2) Recover visitor’s badge

3) Recover PPE if issued

At the end of the normal working day, the guard shall determine if any visitor is still

on the site. If the guard is not able to find the visitor’s location, he shall notify the

Security Supervisor.

1.20.7.4 Vehicle Control

All Subcontractors shall provide a complete list of all permanently assigned work

vehicles, including tools and equipment normally carried in or on the vehicle and

copies of registration, insurance for commercial licenses vehicles and mechanical

inspection for all vehicles and equipment. Upon approval, Contractor Security will

issue a “Vehicle Pass” for each work vehicle.

The Security Supervisor will issue a list of all Authorized Vehicles with complete

descriptions and details of registration identification for use by the guard at the

gates for site entry.

A Temporary Vehicle Pass shall be issued for temporary vehicles and equipment of

a material supplier or vendor.

1.20.7.5 Material and Tools

The Administration Manager’s approval is required prior to material and tools

entering the site.

The Subcontractor has to submit a list of tools and equipment and the areas of use

to the Administration Manager for approval.

All material including tools and equipment shall be registered "Equipment, Tools and

Material Entry Request, Equipment, Tools and Material Out Permit".

Tools and Equipment will only be allowed entry to site if they have been inspected

and are satisfactory for use.

1.21 Plant Operations

Plant facilities will be operated in conformance to equipment manufacturers’

specifications and in keeping with good industry practices. A Plant Operations HSE

Manual will be developed as part of the operating philosophy requirements.

Owner requirements shall be incorporated as identified during Risk Assessment and

other required studies and review processes.

1.22 Safe Work Procedures

Ensuring compliance to safe work procedures is a line management responsibility

and includes all those personnel who direct the work of other workers including the

Site Manager, Construction Manager, Superintendents, Supervisors, Foremen and

Sub-foremen. To achieve the HSE performance objectives, management,

supervision and each worker on site must fully participate to achieve industry best

practices and a level of safety excellence.
