He was admitted on 14-12-2006 and given treatment till his death. During the treatment in SVIMS, Tirupathi he was expired on 19-12-2006 while in service.

I submit that I retired as superintendent at Government Junior College for Girls, Kadapa on 31-05-2002. I was having poor eye vision and unable to see the objects clearly since one year. My Doctor advised me to go immediately to any Super Specialty Hospital for treatment at Bharathi Hospital, Gandhi Nagar, RS Road, and Kadapa.

There at Bharathi Hospital, Gandhi Nagar, RS Road, Kadapa. Several investigations were done and finally diagnosed that I had “Left Eye Surgery”. They also advised me to under go eye surgery for cataract and implantation of foldable in left eye. Accordingly I under went surgery for cataract “Left Eye Surgery” on 26-01-2009. After surgery I was discharged on 27-01-209. Thus for Eye Surgery an amount of Rs.12700/- [Rupees Twelve Thousands seven hundred and only] was spent by me.

Hence I request you sir, to consider my case on humanitarian grounds and I may be given medical reimbursement as per government order in force.

I submit that on the advice of the Physician, I went to Hyderabad to consult nephrologists and on his advice I had to admit at the Care Hospital, Hyderabad for treatment of my Kidneys, the Doctors suggested me for Dialysis

Further, I submit that out of two types of dialysis (CAPD and Haemo Dialysis) nephrology and suggested for CAPD Dialysis for this I met operation as inpatient from 21.08.2009 to 24.8.2009 at City Hospitals, Jubilee Hills, Hyderabad for Rs1, 51,602-76 (Rupees One Lake Fifty One Thousand Six Hundred and Two Only.)

Further, I submit that I have to take up CAPD Dialysis personally every day for which I have to purchase fluid bags from Baxter Company for the entire life period and I have spent nearly Rs 80,000-00 till the date of November 2009 which has been certified by the Doctor in Care Hospital as a out patient.

Therefore, I am herewith submitting the bills for Rs 1, 51,602-76 (Rupees One Lakhs Fifty one Thousand Six Hundred and Two only) (i.e. Rs 61,779-56 + Rs 62,711.00 and Rs 27,112-00)
