Knox Inclusion Support Agency, in partnership with Yarra Ranges Council, employs Inclusion Support Facilitators (ISFs) through the Inclusion and Professional Support Program funded by the Australian Government Department of Social Services. ISFs support educators in Long Day Care, Occasional Care, Outside School Hours Care, Vacation Care and Family Day Care to promote inclusive practises and assist services to build their skills base and capacity to include children with additional needs within the municipal areas of Knox Maroondah and Yarra Ranges Council.

Provides a range of home and community based services that support Knox residents who are frail and older (65 and over) or who have a disability and their carers to remain living in their own home, stay connected to their communities and maintain a healthy and active lifestyle. Services include domestic support, personal care, respite, delivered meals, home maintenance and Occupational Therapy.

Service provision is based on an assessment of eligibility and the availability of resources.

A Housing Support Officer is available through the Active Ageing and Disability Services Department to work with Knox residents who are frail, older and/or have a disability and are homeless or at risk of becoming homeless. This is not a crisis accommodation service.

For children with disabilities and developmental delays from birth to 6 years (school entry) and their families.

Specialist staff offer assessment, education, therapy, family information and support programs in our centres, in homes and community settings including preschool and childcare. Available to children whose families live, work or study in the municipality of Knox (as a priority). Parents pay Program Support Fees along with government funding. Referrals are now all coordinated through Department of Education and Training, Eastern Metropolitan Region, Central Intake on 1300 662 655.

A transport service provided for residents of Knox who are frail aged, mobility impaired or transport disadvantaged to access local senior citizens clubs, programmes and shopping. Eligible community groups can make bookings for a program of social outings for the following year. Knox offers outings programs which include day trips to different destinations across Melbourne.

Knox City Council’s Leisure Services Unit is responsible for the management of all council sporting facilities, as well as managing the contracts of user groups which occupy these facilities.

Amongst a wide range of roles, the Unit is also responsible for increasing growth in sport and recreational pursuits within Knox, implementing club development initiatives, policy review and strategic planning through a leisure lens.

Free and confidential family centred practice is provided to all families with children from birth to 5years.

All children are offered regular assessments and referrals to other support services as needed. The service offers information, guidance and education.

Maternal and Child Health Nurses hold general nurse, midwifery and Maternal and Child Health qualifications and are good source of local information and community supports including community playgroups and supported playgroups.

MetroAccess is a partnership initiative between the Victorian Department of Human Services and Local Government. The aim of MetroAccess is to build the capacity of local communities across metropolitan Victoria so that they are more welcoming and inclusive of people with disabilities.

There are five main objectives of MetroAccess:

To support people with disabilities to participate in community life.

To build and strengthen the ways in which the local community supports people with disabilities and their families.

To ensure that local planning and services engage and involve people with disabilities and their families, disability service providers and community groups.

To work with disability support providers to help them to support people with disabilities and their families.

Knox Council provides a service for eligible residents aged over 65, their carers and people with a disability aged under 65 years, to participate in activity groups.

Group members will be able to participate in craft, games, discussion, entertainment, outings and gentle exercise activities that provide mental, social and physical stimulation in a friendly and safe environment.

Playgroups

Knox City Council has playgroup sessions where parents, carers and grandparents with babies and children 0-6 years are welcome to attend. You’ll find a fun, friendly playgroup operating from many locations across the municipality, with times and days to suit you. In addition, Supported Playgroups are available for parents 25years and under, families with children 0-6 years of age who have additional needs, developmental delay, disability and chronic medical conditions.

Knox Youth Services actively promote, develop and encourage the well being of young people through providing, facilitating, planning, funding and advocating the identified needs and issues of the young people, their families and the community.

Disability Services fund a range of services for people with intellectual, physical, sensory and neurological disabilities. The division works in partnership with families and carers, with non-government and government service providers to advance the wellbeing and promote the quality of life for people with disabilities.

Carer Allowance

An income supplement for carers who provide additional daily care and attention for someone with a disability or medical condition, or who is frail aged.

Child Disability Assistance Payment

An annual payment to assist parents with the costs of caring for a child with a disability.

Continence Aids Payment Scheme

Helps people aged 5 years or more who have permanent and severe incontinence to meet some of the costs of incontinence products.

Disability Support Pension

Provides financial support to people with a physical, intellectual or psychiatric condition that prevents them from working, or for people who are permanently blind.

Essential Medical Equipment Payment

The Essential Medical Equipment Payment helps with the additional costs of running essential medical equipment, medically required heating or cooling, or both.

Ex-Carer Allowance (Child) Health Care Card

Enables concessions on prescription medications. You must claim the card and reapply annually.

Household Assistance Scheme

Ongoing assistance is available for eligible households through a number of different payments.

Mobility Allowance

A payment for people with disability, illness or injury who cannot use public transport without substantial assistance and who participate in approved activities.

Pensioner Education Supplement

Helps students with the ongoing costs of full-time or part-time study in a secondary or tertiary course.

Rent Assistance

Financial help to people who pay rent and receive a Centrelink payment.

Sickness Allowance

A payment for people aged 22 years or older, but under age pension age, who temporarily cannot work or study because of an injury or illness.

Telephone Allowance

Telephone Allowance helps with the costs of maintaining a telephone and home internet service.

Youth Disability Supplement

Additional financial support to young people with a physical, intellectual or psychiatric disability who receive certain income support payments.

Householders and homesharers are matched according to individual needs, personality and preferences.

The Homeshare Program is free for householders over 65. For those under 65, there is a small monthly fee for setting up and monitoring a homeshare agreement.

Provides transport for eligible persons to “essential” hospital and/or doctor’s appointments. The service provides transport for up to a period of six weeks to allow sufficient time for clients to find alternative long term arrangements.

Eligible for people who:

• are unable to access suitable public transport, and/or live in an area that is remote from public transport

• do not have an escort or carer to accompany him or her on public transport to appointments, if accessible public transport exists

• live independently (consisting of caravan parks, rooming houses or boarding houses, hostels without paid support services attached to the accommodation)

• are unable to drive or to be driven by family/friends to appointments

• are not eligible for alternative transport schemes (eg. Ambulance, TAC, WorkCover, Department of Veteran Affairs Gold and other cards etc)

A request requires the submission of a Patient Transport Service Referral Form completed by a registered Medical Practitioner, Health Professional or Transport Coordinator from the referring agency or Hospital. Self referrals from clients are not accepted.
