On behalf of over 400 members of theNew Brunswick Police Association, Iam very pleased to be introducing theNew Brunswick Police Association’s30th Annual Crime Prevention Guide,focusing this year on PrescriptionDrug Abuse Awareness.
For 30 years, these annual guides havereached people across New Brunswickto educate our communities andprevent crime, and for 30 years wehave had the incredible support ofindividuals and businesses who makethis Annual Guide possible. Theircontributions help to ensure that thisguide not only gets published eachyear, but that it reaches thecommunities we work in every day.After 15 years as President, and now,as I move into retirement, I would liketo take this opportunity to give mysincerest of thanks to the communityand businesses who have supported usin getting our message of crimeprevention education out to overseven-hundred-thousand in NewBrunswick each year for the last 30years.
While this is my 15th and final year asPresident of the New Brunswick PoliceAssociation, it’s also my 30th year asan officer; and in my 30 years ofexperience, I have found education tobe key in helping prevent violence andcrime within our communities. Ibelieve, that together we can work toensure the safety of our loved ones andcommunities; and today, that can startwith educating ourselves onPrescription Drug Abuse and thedangers of abusing opiates.
Au nom de plus de 400 membres del'Association des policiers du Nouveau-Brunswick, je suis très heureux deprésenter le 30e Guide annuel sur laprévention du crime de l'Associationdes policiers du Nouveau-Brunswick,qui se concentre cette année sur lasensibilisation à l’abus de médicamentssur ordonnance.
Depuis 30 ans, ces guides annuels ontatteint les gens de partout au Nouveau-Brunswick afin d’aider à l'éducation denos collectivités et à la prévention ducrime. Depuis 30 ans, nous avons reçul’incroyable soutien des particuliers etdes entreprises qui rendent ce guideannuel possible. Leurs contributionsaident à faire en sorte que ce guide soitnon seulement publié chaque année,mais aussi qu'il touche lescommunautés dans lesquelles noustravaillons chaque jour. Après 15 ans àtitre de président, et maintenant que jeprendrai ma retraite, j’aimerais deprofiter de l'occasion pour remerciersincèrement la communauté et lesentreprises qui nous ont aidés à fairepasser notre message d'éducation enmatière de prévention du crime à plusde sept cent mille au Nouveau-Brunswick chaque année au cours des30 dernières années.
Bien que ce soit ma 15e et dernièreannée en tant que président del'Association des policiers du Nouveau-Brunswick, c'est aussi ma 30e année entant qu'officier; et au cours de mes 30années d'expérience, j'ai trouvé quel'éducation était essentielle pour aiderà prévenir la violence et le crime dansnos collectivités. FREDERICTON POLICE FORCE
FORCE POLICIÈREDE FREDERICTON
The Fredericton Police Force has made a significant developmental upgrade to their ExplosiveDisposal Unit by purchasing a new ICOR bomb robot, as well as a new bomb suit. Both itemsrepresent a significant investment (over $200,000) from the city, to upgrade the capabilities of thisunit. Officers train on a regular basis with their partners in the RCMP EDU team, to ensureinteroperability for both the city of Fredericton and surrounding communities. They are also lookingto expand their training to include explosive forced entry, to assist with Emergency Response Teamsin the near future.
La Force policière de Fredericton a apporté une amélioration importante au développement de songroupe d’enlèvement d’explosifs en achetant un nouveau robot à bombe ICOR, ainsi qu'une nouvellecombinaison anti bombe. Prescription drug abuse occurs whensomeone takes a medical drug that was notprescribed for them or they take it in adifferent manner than it was supposed to betaken. Most people who abuse a prescriptiondrug take more of the drug than a personwho takes it for medical purposes. Abuserswill build up tolerances to most drugs,meaning that it takes more and more of adrug to produce the same effect. After awhile, the abuser may be taking much largerdoses than normal. And a person abusing adrug may change its form, for example, theymay crush a pill or dissolve a drug so theycan inject it. • keep medication in a safe and secure place,such as a locked box or cabinet.

Using Medications SafelyPrescription and non-prescriptionmedications can help treat diseases andconditions, and make you feel better. Buttaking these medications correctly isimportant. • It came into widespread use in the mid-1990s with the introduction of thetransdermal patch that releases the druginto the patient's bloodstream over twoor three days. When the drug is processedin a clandestine lab with no qualitycontrols, it is difficult to get the dosageright, making it potentially much moredangerous.
• Chemical companies in China custom-design variants of pharmaceutical-gradefentanyl by tweaking a molecule ever soslightly. A few hundred micrograms – theweight of a single grain of salt – are enoughto trigger heroin-like bliss. But the linebetween euphoria and fatal overdose isfrighteningly thin: An amount the size oftwo grains of salt can kill a healthy adult.
• In British Columbia and Alberta, the twohardest-hit provinces, fatal overdoseslinked to fentanyl soared from 42 in 2012to 418 in 2015.
• Because illicit fentanyl is so potent, once itarrives here, the white crystalline powdergets diluted with powdered sugar, babypowder or antihistamines before it can besold on the street and consumed. It is alsomixed into other drugs, and sold as heroinor OxyContin.
• The arrival of illicit fentanyl in Canada is a"game changer," says Benedikt Fischer, asenior scientist at Toronto's Centre forAddiction and Mental Health. WhenCanada's opioid problem involved only theabuse of prescription drugs, he says,policymakers squandered an opportunityto address it. Now that many addicts areturning to a drug that is manufacturedwithout government oversight – and incountries beyond our borders, to boot –it is nearly impossible to get a grip on theproblem. "Even if we wanted to nowsuddenly take action, there's nothing wecan do," he says. "We have no way ofcontrolling thousands of illegal drug labs inChina."
• Unlike the massive infrastructure andcartels required to manufacture andtransport heroin or cocaine, just aboutanyone can buy and sell fentanyl. Becauseit is so powerful, a little goes a long way. Akilogram ordered over the Internet – anamount equal in weight to a medium-sizedcantaloupe – sells on the street in Calgaryfor $20-million, making it a drug dealer'sdream.

• Buyers are assured their packages won’tget seized at the Canadian border. Toavoid the risk of detection, says a supplierfrom China, he conceals the purchasealongside urine test strips. Not that thereis a need to worry: Canadian borderguards cannot open packages weighing lessthan 30 grams without the consent of therecipient.
• The supplier, who identified himself only asAlan, says he has two customers in Canada.He e-mails photos of fentanyl hiddeninside silica-desiccant packets – the typenormally used when shipping goods suchas electronics – and a screen shot of arecent order from Canada, including ashipping address for a clothing store inBritish Columbia's picturesque OkanaganValley.
• Demand for a replacement for OxyContinalso gave rise to another problem – acounterfeit version of the drug laced withillicit fentanyl smuggled into Canada andprocessed for street sale in labs. The labswould typically dye their pills green tomimic the 80-milligram OxyContin pills
favoured by opioid abusers, and sell themas "greenies" or "shady eighties."• In comparison to Europeans, NorthAmericans rely heavily on pharmacologicalsolutions to medical woes. In a culturewhose citizens pop over-the-counter pillsto treat every minor ailment, says Sgt.Darin Sheppard, of the RCMP's FederalSerious and Organized Crime SyntheticDrug Operations, many drug users viewthe "greenies" as deceptively harmless.
• Police across Canada have shut down 20fentanyl labs since that first major bust inApril 2013, mostly operated by organized-crime groups, according to a Globe review.
• The Canada Border Services Agency, thefirst line of defence in preventing illicitgoods from entering the country, isresponsible for clearing international mail.In 2015, the agency made just under11,000 illicit-drug seizures, half of whichcame through the postal system.
• As long as Chinese officials do not crackdown more aggressively on exporters,medical experts say, the responsibility forchange falls squarely on those in power atthe end of the supply chain.

Opioid Death Toll• 19 people in NB died from opioid overdoses during the first 9 months of 2017.
• 17 opioid overdose deaths betweenJanuary and June and two between Julyand September.
• 5 of the deaths are believed to be linked tofentanyl (a powerful opioid that has claimedhundreds of lives across Canada this year).

Accroître l’accessabilité dela Naloxone
• Selon le groupe opérationnel du N.-B. sur les opioïdes, la province a l'un des tauxles plus bas de fentanyl au pays, mais lesresponsables de la santé affirment qu'il estimportant d'être prêt si la situations'aggrave.
• Le gouvernement du N.-B. étudie desoptions pour rendre la naloxonedisponible sans frais pour les populationsvulnérables du N.-B.
• Le groupe opérationnel concentreégalement les domaines clés, y compris lasurveillance et le suivi, les ressources,l'éducation, les interventions de traitementet la sensibilisation et l'évaluation.
• Il étudie également les équipements deprotection individuelle dont les premiersintervenants ont besoin pour rester ensécurité s'ils sont exposés au fentanyl etaux communications publiques.
• De nombreux premiers intervenantsportent déjà des trousses de naloxone

If aperson decides to use the drug again, andmistakenly believes he or she can use thesame amount of the drug as before detox,there is a risk of overdose or even death.

Ce typede centre peut se retrouver dans diversmilieux, comme les hôpitaux, les centresavec hébergement et les centres detraitement à l’externe.
Soins continus : Prestation de soins post-traitement visant à favoriser lerétablissement et le maintien de saineshabitudes, pouvant comprendre laparticipation à des groupes de soutien parles pairs, la prise continue de médicamentset le logement supervisé adapté.
Thérapie et counseling enmatière de dépendance
La thérapie et le counseling sont lesméthodes les plus courantes pour traiter lesproblèmes de consommation d’alcool ou dedrogue. Ils peuvent être dispensés selon uneformule individuelle, conjugale, familiale oude groupe. Toutefois, les thérapiesindividuelles, consistant à rencontrer unthérapeute ou un conseiller seul à seul, et degroupe, consistant à rencontrercollectivement un thérapeute ou unconseiller ainsi que d’autres clients ayant lemême genre de problèmes, sont les plusrépandues.

Les programmes de traitement structuréssont intensifs et suivent un horaire detraitement et d’activités strict. Ils peuventcomprendre une thérapie individuelle ou degroupe, des séances d’information sur lessymptômes ou le développement d’habiletéssociales et des activités de planification dutraitement.
Médicaments utilisés pourtraiter les dépendances
Il existe plusieurs médicaments qui peuventaider les personnes aux prises avec unedépendance à l’alcool ou à la drogue. Lesplus utilisés visent à traiter la dépendance àl’alcool et aux opioïdes (p. ex. analgésiquessur ordonnance, héroïne). Les médicamentsdonnent des résultats optimaux lorsqu’ilssont combinés au counseling et à d’autresformes d’aide. Le médecin de famille peutévaluer s’il convient de recourir auxmédicaments pour traiter la dépendance.Soulignons que les médicaments décrits plusloin nécessitent une ordonnance et que laprise continue devrait faire l’objet d’unesupervision par un médecin ou unfournisseur de soins de santé autorisé ouagréé formé à cet effet (cette exigence peutvarier selon la province et le territoire).
Médicaments utilisés pourtraiter la dépendance auxopioïdes
La buprénorphine-naloxone combine labuprénorphine et la naloxone et se présentesous forme de comprimé. Elle vise à traiterla dépendance aux opioïdes et pose unrisque de surdose moindre. Labuprénorphine reproduit les effets physiquesde la substance causant la dépendance,tandis que la naloxone sert à prévenir lemésusage.
La méthadone atténue les symptômes dusevrage des opioïdes et l’état de manque, enplus de bloquer le sentiment d’euphoriecausé par les autres opioïdes, et peut êtreutilisée en pharmacothérapie. Le traitementd’entretien à la méthadone sertgénéralement à traiter les gens quiconsomment des opioïdes depuis longtempset qui n’ont pas réussi à arrêter avec lesautres types de traitement. Puissante, laméthadone peut mener à une surdose encas de mésusage; sa prise devrait donctoujours être supervisée par un médecin ouun autre fournisseur de soins de santéautorisé ou agréé qui est formé et autoriséà prescrire ce médicament (cette exigencepeut varier selon la province et le territoire).

Programmes de réductiondes méfaits
Les programmes de réduction des méfaitssont généralement utilisés par les gens quiont subi des méfaits dus à leurconsommation d’alcool ou de drogue, quin’ont pas tiré de bénéfice du traitement, quine veulent pas recevoir de traitement ou quipréfèrent continuer à consommer. Il enexiste de nombreux types.
Les programmes de distribution oud’échange de seringues consistent àremettre des seringues propres, à donnerdes seringues neuves en échange deseringues souillées et à fournir des seringueset d’autre matériel aux consommateurs dedrogues injectables, en plus de leur expliquercomment se débarrasser de leurs seringuesusagées de façon sécuritaire. Ainsi, cesprogrammes contribuent à réduire le risquede contracter des infections graves, commele VIH et l’hépatite C, associées à l’utilisationet à l’échange de seringues.
Le traitement de substitution auxopioïdes consiste à remplacer laconsommation illégale d’héroïne etd’analgésiques opioïdes sur ordonnance parla prise de méthadone, de buprénorphine-naloxone ou d’un autre substitut.
Les sites de consommation supervisée,qui constituent des milieux deconsommation sécuritaire, préviennent lessurdoses et autres méfaits.
Les services de prévention des surdoseset d’intervention donnent de la formationet des trousses de naloxone aux personnesà risque de faire une surdose d’opioïdes etaux personnes susceptibles d’être sur placepour intervenir en cas de surdose. Lanaloxone peut temporairement inverser leseffets d’une surdose d’opioïdes et ainsidonner le temps d’appeler le 9-1-1 pourobtenir une aide médicale d’urgence.
Prise en charge du sevrage(désintoxication)
Mieux connue sous le nom de « désintoxication », ou « désintox », la priseen charge du sevrage aide les personnesphysiquement dépendantes à l’alcool ou à ladrogue à se sevrer de façon sécuritaire. Lessymptômes de sevrage vont de légers (p. ex.anxiété, tremblements, problèmes desommeil) à graves et potentiellementmortels. En voici quelques exemples :

Différents types de services dedésintoxication sont offerts au Canada. désintoxication à domicile grâce auxquelsune personne reçoit de l’aide pour se sevrerchez elle. Un médecin ou un autrefournisseur de soins de santé pourra aiderle client à décider s’il a besoin d’unedésintoxication et quel type lui convient, lecas échéant.
Il est important d’entamer un traitementtout de suite après la désintoxication. Lespersonnes sevrées d’opioïdes commel’héroïne ou les analgésiques sur ordonnanceont perdu toute tolérance à la drogue. Sielles recommencent à consommer et croientà tort qu’elles peuvent prendre la mêmequantité qu’avant, elles risquent de faire unesurdose et même d’y trouver la mort.
