    Screening Guidelines for Work Permit Determination

    The purpose of this checklist is to help in determining if a job requires a work permit due to ESS&H hazards,

    system complexities, or work coordination needs. If the job already has an established work procedure or has

    been reviewed and defined as Worker Planned work due to the training and qualifications of the crafts person

    or technician, then the checklist is not needed. Additional information for facility hazard determination can beobtained in the Facility Hazard AnalysisSubject Area. Check off the items that apply to the job:

    A. ESS&H or Other Permits Needed

    If two or more boxes are checked, the work is considered greater than a low hazard and a work permit isrequired unless there is an existing operating procedure that will be followed.

    B. Safety, Security and Facility ConcernsIf checked, then the following work is considered greater than a low hazard and a work permit is required to

    safely plan work involving the following safety and security issues unless the work is covered by a separate

    Standard Operating Procedure (SOP) or other work planning document and workers are properly trained.

    Involve your Safety and Health Services Division Rep in the work planning.

    Adding/Removing Walls or Roof - Work affects the integrity of a structure. Contact your ES&H subject matterexpert.

    Asbestos - Work disturbs or involves removing asbestos-containing material. See the Asbestos Subject Area.

    Beryllium - Work may produce airborne Beryllium dust in any manner. See the BerylliumSubject Area.

    Biohazard - Potential for exposure to biohazardous substances. See the Bloodborne Pathogens Subject Area.

    Chemical/Corrosives - Potential for or exposure to chemicals in excess of OHSA PELs. See the Chemical

    SafetySubject Area and contact the subject matter expert.

    Confined Space - Work may take place in a confined space. See the Confined SpacesSubject Area for

    classification and precautions.

    Critical lift - Work involves a critical lift or exposes expensive equipment to potential damage. See the Lifting

    Safety Subject Area and review training requirements.

    Cryogenic - Work involves extremely low temperature materials. See the Cryogenics Safety Subject Area.

    Energized Electrical Work - Contact your ES&H subject matter expert to determine if this should be checked.

    Elevated work - Work with a potential fall of six feet or use of scaffolding, harnesses, lanyards, orfall protection plan.

    Explosives - Must use work permit if no task specific procedure for explosives is utilized. See the Explosives

    Fumes/Mist/Dust - Work activity likely to result in an inhalation or dermal exposure to dust, fumes, or mist.

    Heat/Cold stress - Work requires exposure to extreme temperature conditions. See theNaturalHazards in the EnvironmentSubject Area.

    Hydraulic - Work requires the storage, handling, or use of hydraulic fluid.Transport of Haz/Rad Material - Transportation of hazardous or radioactive material in excess of

    Materials of Trade (MOT) quantities. See theMovement by Vehicle of Hazardous and Radiological

    Materials On-site and Transportation of Hazardous and Radiological Materials Off-siteSubject

    Areas.

    Lead - Work involves lead in any form. See the Lead Subject Area.

    Magnetic Fields - Work occurs within a magnetic field or creates one. See the Static Magnetic Fields

    Subject Area.

    Material handling - Moving heavy loads by personnel other than riggers or qualified materialhandlers. See the exhibit Guidelines for Industrial and Construction Workin the Ergonomics,Occupational Subject Area and Medical Clearance below, or contact the subject matter expert.

    Nanomaterials - Work involves materials consisting of, or containing structures of between 1- and100-nanometers (nm). See theNanoscale Particle ESH Subject Area.

    Noise - See theNoise and Hearing ConservationSubject Area and contact the subject matter expert.Lasers/Non-ionizing radiation - Work involves the use of non-ionizing radiation sources or devices other

    than welding equipment (i.e., microwaves, RF fields, etc.). See theNon-ionizing Radiation Safety

    Subject Area. Operates a Class 3 or 4 lasers on the job. See the Laser SafetySubject Area or contact

    the subject matter expert.

    Operational Safety Envelope - work could impact the operational safety envelope as defined by the Facility Use

    Agreement. See theTable of Facility Use Agreements.

    Oxygen deficiency hazard - Work is in a potential Oxygen Deficiency Hazard (ODH) area or may create

    one. See the Oxygen Deficiency Hazards (ODH), System Classification and Controls Subject Area.

    Removing/penetrating walls or doors - Firewalls or fire- smoke-doors may be impacted by work. Contact

    the Authority having Jurisdiction (AHJ) Joe Levesque at extension 4259.

    Pressurized Systems - See the Pressure SafetySubject Area. Railroad Work - Work involving railroad cars within the boundaries of BNL and on the trackbed up

    to the derailer. Work near railroad cars and tracks, maintenance in the area of railroad cars and

    tracks, accessing railroad cars, or using railroad cars to transport hazardous and non-hazardous

    materials. See Traffic Safety Subject Area.

    Rigging - Work Lifts that involve the use of basic lifting, hoisting and rigging equipment. See the LiftingSafetySubject Area and review training requirements.

    Security Concerns - Work conducted in a designated security area (PPA area, CAA, Limited Area, VTR) or

    where enhanced security systems are deployed (card readers, CCTV, duress alarms, etc.). Contact LP Security

    Suspect/Counterfeit Items - Work may involve a component or assembly of questionable manufacture and/or

    origin (e.g., a gear, screw, cam, transistor, circuit breakers, integrated circuit, fasteners, bolts, studs, fittings,

    valves, flanges, and couplings. See Suspect/Counterfeit ItemsSubject Area.

    Vacuum Systems - Systems greater than 100,000 joules of stored energy. Contact Safety and Health Services

    Division.

    Intermittent Energy Release - Potential release of energy that may create a startle hazard.

    C. Environmental/Waste Concerns:If checked, then the following work is considered greater than a low hazard and a work permit is required to

    properly plan work involving the following environmental issues unless work is covered by a separate Standard

    Operating Procedure (SOP) or other work planning document and workers are properly trained. Involve your

    Environmental Compliance Representative (ECR) in the work planning.

    Work Impacts Environmental Permit - Work impacts an area covered by an existing environmental permit, suchas an emission unit covered under BNLs Title V Air Permit, or a chemical storage area registered with Suffolk

    County (Article 12) or registered and/or permitted by New York State. Registered/permitted areas are labeled

    or posted as such.

    Atmospheric Discharges - Work creates a new air emission source or changes an existing one. See theNon-Radioactive Airborne Emissions orRadioactive Airborne EmissionsSubject Areas.

    Note: This would include the removal or installation of hoods, as well as any other exhaust from

    processes vented outside of a building.

    Chemical or Rad Material Storage or Use -Work involves bulk storage of chemicals (drums or tanks) and may

    increase storage of chemicals beyond limits of Suffolk County Article 12. See the Storage and Transfer of

    Hazardous and Nonhazardous MaterialsSubject Area.

    Cesspools (UIC) - Work involves an Underground Injection Control (UIC) device such as a cesspool or

    component parts. See the Underground Injection Control Subject Area.

    High Water or Power Consumption - Check withECRfor specific criteria.

    Cultural Historic Resources - Work may impact features of historic resources identified in the BNL

    Cultural Resource Management Plan including: BGRR, HFBR, Building 30, Building 120, Berkner

    Hall - Building 488, Chemistry Building 555, Gamma Forest, Camp Upton foundations and trenches;

    historic house sites. Contact M. Davis at extension 2165 with any questions.

    Threatened or Endangered Species/Wetland Areas - Work may impact habitat of protected species or

    wetlands. Permits may be required. Contact T. Green at extension 3091.

    The Long Term Response Actions Manager must approve Land Use Institutional Controls - any work done in or

    near a CERCLA clean up area. Contact theLong Term Response Actions POC.

    Liquid Discharges - Any of the chemicals not listed on the sink releasable list, or any discharge of radioactivity

    to the sanitary or storm water system. See the Liquid Effluents Subject Area. Also see the exhibit BNL State

    Pollutant Discharge Elimination System (SPDES) Permit Chemicalsin theEnvironmental Aspects and Impacts

    Subject Area.

    PCB Management - Any work that involves PCBs. See the PCB Management Subject Area.

    Spill Potential - Work involves the potential for a hazardous material or radioactive release to the environment.

    See theSpill ResponseSubject Area.

    Note: A release can include uncontrolled releases to floor and sink drains, to soil, and to air.

    Soil Activation/Contamination - Work involves activated or contaminated soil. Contact yourFacility

    Support Representative.

    Waste - Work generates regulated waste. See the Hazardous Waste Management, Industrial Waste

    Management, Mixed Waste Management,Radioactive Waste Management, orRegulated Medical

    Waste ManagementSubject Areas. Work that only generates clean (non-regulated waste), and that

    has no other ESH, complexity or coordination concerns does not require a work permit.

    Underground Ducts/Piping - Work that involves pipes or duct work suspected of containing

    chemicals/radioactive materials in any quantity. Contact yourECR.

    Other - Any other environmentally related concerns. Example:

    Change/Addition/Removal of a backflow device in a potable water system. See the DrinkingWaterSubject Area.

    Pollution prevention (P2)/ Waste Minimization Opportunity - Check this box when the job is planned in

    such a way as to reduce/eliminate a waste stream (haz/rad waste, air emissions, liquid discharge,

    etc.). See the Pollution Prevention and Waste Minimization Subject Area.

    D. Radiation IssuesIf checked, then the following work is considered greater than a low hazard and a work permit is required to

    properly plan work involving the following radiation issues unless work is covered by a Radiation Work Permit

    (RWP), separate Standard Operating Procedure (SOP) or other work planning document and workers areproperly trained.

    Rad. Materials may exceed area posted limits. Contact yourFacility Support Representative.

    Radiation/Contamination - Work involves radiation or contamination not covered by an RWP. Rad Material transfers in or out of facility - Work involves adding radiological material to facility invento

    Check for impact on facility hazard categorization inventory limits.

    E. Complexity/Uncertainty Issues

    If checked, then the following work is considered greater than a low hazard and a work permit is required toproperly plan work involving the following complexity issues unless the work is covered by a separate Standard

    Operating Procedure (SOP) or other work planning document and workers are properly trained.

    Shutdown of several systems is required to properly isolate the work area. There are five or more major tasks/activities involved in the job. Work requires notification of any regulatory agency. Work involves a large number of steps that require action or sequences of steps that may not be

    obvious.

    Human Factors review should be performed to identify error likely situations. Work is prone to rapidly changing conditions (any shift change or resumption of work will require a

    review of changes).

    Work is infrequently done or it is a first time activity. Equipment will be used outside of its normal operating range.
