Hand Receipt Procedures

DA FORM 2062IN ACCORDANCE WITH DA PAM 710-2, CHAPTER 5

Task: Complete a DA form 2062 in accordance with DA PAM 710-2 CH 5 and 3-71 CAV S-6 Standard Operating Procedures.

Conditions: Given a classroom environment, step-by-step instruction, a sample DA 2062, and blank DA 2062, and DA PAM 710-2 CH 5.

Standard: Using no more than three(3) blank DA 2062s, each soldier must properly sign for, and sign out one Laptop with power supply, IAW DA PAM 710-2 CH 5 and unit SOP.

Step 1: Type Cross out the words “Annex Number” from the space entitled “Hand Receipt/Annex Number”.

Doing So specifies whether the form is being used as a Hand Receipt or Shortage Annex.

Determination of this type determines how the form is read.

Step 2: From Fill in the second block of the first row, entitled “From:”

If you are signing for the equipment from an outside source, this is their name. If you are signing equipment out from the shop, this is your name, followed by “/ S – 6”

“Rank Last Name, First Name / Troop or S-Shop”

See examples below:

Ex: “SPC Snuffy, Joe / C Troop“ And “SPC Snuffy, Joe/ S-6” are both Correct.

“SPC Snuffy” or “C Troop” alone are not enough.

Step 3: ToTo the right of the block labeled “From:”, is the block to be filled in Step 3, labeled “To:” This should be filled out in exactly the same manner as the previous step, but in reverse:

If you are signing for the equipment from an outside source, this is your name, followed by “/ S – 6” . If you are signing equipment out from the shop, this is their name

Ex: “SPC Snuffy, Joe / C Troop“ And “SPC Snuffy, Joe/ S-6” are both Correct.

Step 4: Hand Receipt Number The block to the far top-left, entitled Hand Receipt Number is not normally used in temporary loan of equipment within the unit.

It primarily finds use by Primary Hand Receipt Holders (PHRHs), who use it to specify which hand receipt holds the items found on the primary hand receipt.

Step 5: End Item Stock NumberThe Second block of the second row, entitled “End Item Stock Number” should be filled in for all equipment if the item has a stock number.

For the purposes of our shop S.O.P., we do not include stock numbers on laptops that we receive for repairs/degauss.

Step 6: Box 3 of line 2 requires a description using a minimal number of words; use enough words to clarify what specific kind of item it is. If describing a laptop, for example, “Laptop” is not enough.

Correct Examples:

“Laptop, Dell e6510”

“Laptop, HP Elitebook 8530 q”

Incorrect Examples:

“Laptop”

“E6510”

Step 7: Publication Number & DateIAW DA PAM 720-1, the next two blocks, entitled “Publication Number” and “Publication Date” are to be filled out if there is a publication, regardless of it’s availability.

For the purpose of our shop S.O.P., these blocks will remain empty for laptops that are turned in for repairs/degauss.

Step 8: QuantityThis block, left empty in most cases, is used when there are multiples of the end-item in question.

Step 9: Blocks a – f / A - FCompletion instructions by block or column for DA Form 2062 prepared as an inventory list.

(a) Enter stock number of the item being described (if any).

(b) The item description, and Serial Number.

(c) Enter the SEC of the item (IAW shop S.O.P., leave Blank for laptops).

(d) Enter the unit of issue (“EA”,).

(A thru F)—

1. Enter the quantity on hand for each item listed. Name, Date, and Sign along the last column used for quantity- Typically column A.

Step 10: Page NumberLargely self explanatory, at the far bottom left is a “page _ of _ Pages” section.

Simply note which page you are on and how many pages the hand receipt spans. typically “ page 1 of 1 pages”, in our shop.

Final Step: Finishing TouchesAt the top of the page, note what must be done, especially if there are any extra steps required.

Practical Exercise:

Each soldier in the class will be provided with two(2) blank hand receipts. One will be filled out to sign out a laptop (provided) to the Instructor (SPC Snuffiluficus, Robin). For the purpose of this exercise, SPC Snuffiluficus works in C Troop. The other will be filled out to sign for a laptop (provided) as if from an outside source, Namely PVT Snuffy, Joe who works in S-3.
