It is best to go through the entire booklet and highlight the scholarships that you are eligible to apply for. The scholarships have been divided into three sections:

Section 1: Scholarships that use the Middleborough High School Application (page 3)

Section 2: Scholarships that use the Middleborough Dollars for Scholars, Inc. Application. (page 21)

Section 3: Scholarships that use their own application. (page 27)

Please make sure you read the description for each scholarship to verify qualification and what other information may be needed. Please staple that information to your application. Do not use paper clips. Some scholarships require meetings and/or portfolios. It is imperative that you follow all specific instructions. Many (but not all) scholarships require a transcript accompany the application. You may obtain official transcripts from Mrs. Amaral in the Guidance Office. Please note that you will only receive one copy of your unofficial transcript. You must make copies and attach them with any other information that is requested in the various scholarships. Please do not wait until the last minute to put your transcript request in to Mrs. Amaral. Please Note: It is the student’s responsibility to assure that they are submitting a completed application packet. To the right of each scholarship it will indicate if the scholarship is open to Alumni, the Class of 2021 or both. If no special application is needed, you must fill out the Middleborough High School application (Sample located on pages 19 and 20 of this booklet). The information on the Middleborough High School application remains the same for each scholarship, so you should follow the following steps:

1. Download the Middleborough High School Scholarship Application from the MHS website. 2. Complete Part 2 of the Scholarship Application. 3. Complete Part 3 Activities Sheet. 4. Proofread! 5. Go back to Part 1, click on the Drop Box and select the scholarship that you would like to apply for. 6. Print (double-sided) a copy of the Scholarship Application and Activities Sheet. 7. Go back to the Drop Box and select the next scholarship that you would like to apply for. 8. Print and repeat for each scholarship you are applying for.

Please note: If a scholarship does not appear in the drop box, it does not use the Section I application. All scholarships are listed alphabetically by the first name/word of the scholarship. Unless otherwise noted, completed scholarship applications must be submitted directly to the sponsor, by no later than 4:00pm on Friday, March 12, 2021. Late or incomplete applications will not be accepted.

If your application says to submit to the MHS School Counseling Office, do not slide it under the door or leave it on a desk. Please be sure to hand it directly to a member of the School Counseling Department Staff. For easy access, all Special Applications in Section 3 can be downloaded from the Middleborough High School website. Unless otherwise noted, payment of scholarships will be made upon evidence of successful completion of your first semester with an official transcript from the college/university. If you have any questions, please see your School Counselor. We have made every effort to display scholarship information accurately and completely. However, sponsoring organizations may have to change policies, awards, and/or requirements after this booklet is published due to fluctuating interest rates or other variables which could affect the amounts.

The scholarships listed in Section 1 of the scholarship booklet all subscribe to the Middleborough High School Scholarship Application. Do not use the sample copy in this booklet. Please click on the Middleborough High School Scholarship link. Then click on Section 1. Next, click on the MHS Scholarship Application, download and complete your applications. All applications in this section need to be typed. Print the application and the Activities Sheet back to back and staple any requested materials to the back. Submit the application and all requested materials according to the instructions in each individual scholarship description, to the specific designated address. ALBERT A. AND RUTH W. THOMAS SCHOLARSHIP FUND Alumni

2021

Available to one or more seniors or alumnus of Middleborough High School, who seek education beyond grade twelve. Recipients may receive an award for more than one year. Numbers of recipients, amounts of scholarships, and the repetition of awards are at the discretion of the selection committee. College graduates may also apply for this scholarship. Students planning to attend institutions other than traditional colleges or universities may apply. Attorney Thomas A. Maddigan will make the selection.

ARLEEN AND ARTHUR H. DUNHAM EDUCATIONAL TRUST SCHOLARSHIP

Alumni 2021

The annual income derived from this trust fund is awarded to a student who is graduating from Middleborough High School, or an alumnus, and who is seeking higher education in a college. The award is given as an aid and incentive to students of promise with limited means. Character, service and leadership will also be considered in the selection of the recipient. The amount of the award varies and is payable to the recipient in varying amounts according to need at any given time while in college. The Superintendent of Schools and Attorney Thomas A. Maddigan will make selection. You must attach a copy of your completed FAFSA (SAR).

DANA KELBLE SCHOLARSHIP 2021

Dana John Kelble was a lifelong Middleborough resident. He graduated from Middleborough High School in 2013, later serving in the Army National Guard and then entering the workforce as a field mechanic and CDL driver. Dana had a passion for anything with a motor and enjoyed doing any type of outdoor activities with his friends and family. This scholarship is being offered by RYCO Excavating in Dana’s memory for like-minded students that plan to pursue a similar career path.

Open to the Class of 2021 with preference given to:

1. Any student joining the armed forces after graduation (armed forces reserves and attending college full time or armed forces fulltime and attending an online college)

2. Any student that attends a technical college with aspirations to become an automotive, diesel technician or any other trade.

3. Any student wishing to attend a CDL driving school after graduation such as Parker, Netts, etc. with aspirations of entering the workforce as a professional driver after high school.

4. Any student attending college whose family gross income is under $45,000.00. 5. Student must submit a transcript and have a minimum 2.50 GPA.

In 300 words or less, please describe what your plans are for after high school. Include how this scholarship will help you meet those goals.

Deane Award has been established for the purpose of providing a scholarship for a worthy and needy graduate of Middleborough High School who desires to, and is qualified to, obtain an education above the high school level. Said scholarship shall be given each year to a candidate selected by the Principal of the Middleborough High School and the appointed representative of the First Unitarian Universalist Society of Middleborough. Only one recipient shall be selected each year for the Deane Award.

Debbie Blais Real Estate will award a scholarship to a graduating senior living in the Town of Middleborough. The applicant must be attending a college or trade school during the 2021-2022 school year. Eligibility will be based on scholastic achievement, school and community activities, financial need and an essay explaining financial need.

DOROTHY J. GAMACHE MEMORIAL ART SCHOLARSHIP

This scholarship is provided from the Will of Dorothy J. Gamache, a life-long resident of Middleborough, and a former art student. One $1,000 scholarship will be awarded to a graduating senior or alumni or two $500 awards to students who plan to attend a college of art. Aptitude and financial need will be considered in the selection. This scholarship will be payable after successful completion of the first semester. DR. RICHARD GROSS MEMORIAL SCHOLARSHIP 2021

This scholarship is offered to a graduating Middleborough High School senior who will be attending a 4 year college with the intention of pursuing a career as a health care professional. This $1,000 scholarship is given to honor the memory of Dr. Richard Gross, a primary care physician who had a medical practice in the town of Middleborough for 34 years. He was well known by his patients to be a compassionate, dedicated and knowledgeable practitioner. He was well respected by his colleagues and awarded the Mass Medical Clinician of the Year award in 2013 by both the Bristol and Plymouth County Medical Societies. He died in 2014 of glioblastoma.

The student should exhibit high academic standing with a GPA of at least 3.0 and submit a transcript. The student should demonstrate strong moral character and citizenship evidenced through involvement in school and community activities and service groups. Supporting documentation should include a transcript, resume, recommendation, and essay. The 200 word essay should explain the student’s desire to become a health care professional and how they will contribute to the well-being of others. Financial need will be considered.

The family of Dr. Richard Gross will present this award at a separate award ceremony in Spring, 2021, where family, friends, former patients and colleagues will come together to celebrate the life of Dr. Gross and the student who represents his ideals. Student attendance at this event is mandatory. Family and friends are welcome. You may submit your resume in lieu of page two of the MHS scholarship application.

Harlow for the awarding of a scholarship to a person or persons who is graduating, or has graduated from, the Middleborough High School, and who intends to pursue a career in the engineering field. FAYE H. DEANE SCHOLARSHIP FUND

This award is to be given to one or more students, in the current graduating class or who have graduated from the Middleborough school system, and who are seeking further education beyond the twelfth grade. The income shall be awarded annually, and the same recipient may receive an award for more than one year. The Trustees shall have sole discretion to distribute an award in installments over more than one year and the amounts to any one student do not have to be equal in amount. These scholarships awarded by a committee, which will meet at the school in early May.

FRIENDS OF CRANBERRY SPECIALTY HOSPITAL OF PLYMOUTH COUNTY

James was a life-long resident of Middleboro and a 2001 graduate of Middleborough High School. JANET FICKERT TROWBRIDGE MEMORIAL SCHOLARSHIP 2021

Janet (Fickert) Trowbridge was a lifelong resident of Middleborough. She attended the University of new Hampshire to study English, her favorite subject. She thoroughly enjoyed UNH and graduated with her BS in English and began teaching. She taught in various school systems on the South Shore and her longest and final teaching assignment was at Milton High School. Janet continued to love English, literature and writing throughout her career. She inspired many students to pursue their academic dreams throughout her lifetime. Ms. Trowbridge loved the Town of Middleborough. She established a scholarship fund in her will for a Middleborough High School scholarship fund. She donated money to assist other Middleborough students to achieve their academic dreams. Two scholarships will be awarded to two students who demonstrate academic performance, outside activities (extracurricular activities, volunteer activities, work) and financial need.

The Kiwanis Club of Middleborough will award scholarship to any MHS Key Club Alumni residing in Middleborough enrolled in a college or university program. The Kiwanis Club will consider any applicants that have completed their freshman year of college, are in a graduate program or an evening school program. Financial need, academic achievement and community service are the main criteria for this award. Students applying for this scholarship must write a letter to the club stating why they feel they deserve this scholarship. Applicants need to submit copies of their transcript, financial aid form and a list of community service events. Each of the above requirements is important and must be met.

LAURA C. VANNETT MEMORIAL SCHOLARSHIP 2021

This scholarship will be given in memory of Laura C. Vannett, a graduate of the Middleborough High School Class of 2010, to a graduating senior from the Middleborough High School who has been accepted to any two or four-year College or University. Recipient must be, and remain in, good academic standing with at least a 2.5 GPA and be interested in education or a career which helps others. A brief essay explaining, “What you believe is your greatest gift or talent and how you intend to use it to help or serve others”, and a transcript must

accompany scholarship request.

LOIS W. BUCK MEMORIAL SCHOLARSHIP 2021

Two $1,000 scholarships will be awarded to graduating seniors of Middleborough High School who best exhibit those characteristics that were important to Mrs. Buck, a Mathematics teacher in the Middleborough Public School System for over 40 years. Her goals in teaching, as best remembered by her family, were not to create the best student, but to bring out the best in every student. Lois W. Buck was a strong advocate of education. She wanted to make students aware of the wealth of knowledge that exists and to realize the value of education. Keep these thoughts in mind and write an essay of not less than 200 words and not more than 500 words with respect to the following: Why are you choosing to further your education? What are your goals and/or ambitions in education and life? These scholarships will be paid to those students who excel in upper level math courses and who exhibit a desire to further their education in a mathematically related field of study. Students must have completed four years of high school math and be attending a four-year college or university to qualify. Include a transcript from the guidance department, this this year’s classes, test scores and your essay with the MHS application.

The Louisa Burt Wood Pratt Scholarship consists of scholarships of varying amounts which are given to students who are graduating this year, have graduated from the Middleborough school system, and are seeking education beyond the 12th grade for the study of music related areas. These scholarships can be for a graduating senior and/or those already in college. Attorney Thomas A. Maddigan is solely responsible for awarding this scholarship.

Lt. Leon “Butch” Desrosiers took great pride in aiding and assisting young people of Middleborough during his years as a police officer. The Middleborough Police Officers take great pride in honoring Lt. Leon “Butch”. Desrosiers’ memory in awarding this scholarship in his name.

The Teacher's Memorial Scholarship will be awarded to seniors of Middleborough High School who are to be enrolled in an accredited institution. These scholarships, given in memory of all the teachers who have served the children of Middleborough, will be given annually to students who are children of teachers, children of support personnel, or students entering the field of education. Please attach a brief essay explaining your future goals and why you believe the committee should select you as a recipient. Also include the occupation and job location of your parent/guardian if you are applying as a child of a teacher or support personnel. Final selection will be based on financial need. The Oak Point Homeowners Scholarship Fund (Hart Family Scholarship) is donated by Jaqueline Trepanier and Sal Litchman, in memory of members of the Hart Family of Middleborough. The Scholarship will be awarded to a member of the Class of 2021 who will attend a four-year college, a two-year college or an accredited trade program. OAK POINT HOMEOWNERS SCHOLARSHIP FUND (Helen S. Fisher Memorial Scholarship) 2021

The Oak Point Homeowners Scholarship Fund (Helen S. Fisher Memorial Scholarship), donated by Charlie Fisher, will be awarded to a member of the Class of 2021. Helen S. Fisher graduated from Memorial High School Class of 1937 (now the Middleborough High School). She was a resident of Middleborough and worked at Raytheon Company. In addition, she was a shop steward for the IBEW, a Past President of the Whitman Women’s Club and served on various scholarship committees throughout her lifetime. Mrs. Fisher met her husband, Charles Fisher, at Fort Devens after World War II. They had one child, Charles, who currently resides at Oak Point. Mrs. Fisher was a strong advocate of education and had a passion for reading. As such, this scholarship will be awarded to a member of the Class of 2021 who will be pursuing a degree in education. The recipient must maintain at least a 2.5 GPA and submit an official certified transcript at the completion of their first semester. Failure to submit the official transcript by March 1, 2022 will result in the forfeiture of the scholarship award.

Roderick MacLean Berry, Jr. (“Mr. Berry”) was a dedicated coach, educator and an Assistant Principal at Middleboro High School for over thirty years. Mr. Berry was a football coach for several years and he was responsible for forming and creating the first wrestling program and wrestling team for MHS. Mr. Berry remained the head coach until he retired the coaching role in 1989. This scholarship, given in his memory by his family, is to be awarded to a hardworking Middleborough High School graduating senior who will be attending an accredited college or recognized trade or technical school. ROSE M. WESTON MEMORIAL SCHOLARSHIP 2021

Her four children, and their families, started the Rose M. Weston Scholarship Fund in May of the year 2000, the year that both she and her husband, Thomas Weston, died. Rose M. Weston was a two-time Past President of the Middleborough Cabot Club and longtime resident of Middleborough. Rose M. Weston worked for the Middleborough School System and her four children graduated from the Middleborough and Memorial High Schools. As a child growing up during the depression in Elmira, New York, Rose longed to attend college. Her parents both passed away when she was a young teen. Through her own hard work and perseverance, she attended several colleges including Iona and Syracuse. While she never graduated from college, departing to marry and start a family of her own, she always valued education. Throughout her life, she supported activities that encouraged Middleborough youth. Her work with the Pilgrim Fellowship youth group at Central Congregational Church, the Girl Scouts of America, and the Middleborough Cabot Club Scholarship Program demonstrate this commitment. The Rose M. Weston Scholarship is awarded to a female in the Class of 2021 at Middleborough High School who has lost either one or both parents prior to the age of 18. The recipient is one who attends an institute of higher education and is judged by the MHS Scholarship Committee or Weston Family as being eligible to receive the award. The intention is to give an award each year. If there are no qualified applicants, funds for that year will be used for further capitalization of the fund. The Weston Family will invest the funds and notify the Middleborough High School of the amount of the annual award and provide information about the requirements as developed. The family will select recipient.

In honor of Tim Harnett, who coached and mentored many of Middleborough’s young people during his life. Tim believed that participation in youth or high school sports taught valuable life lessons. Miss Anna C. Erickson was a Mathematics teacher in Middleborough for over thirty years and is still fondly remembered by former students. The Anna C. Erickson Scholarship is given in memory of Miss Erickson by her family and friends to a deserving high school senior who meets the qualifications established by the committee from time to time. Emphasis is placed upon a high standard of academic achievement.

The recipient chosen must maintain at least a 3.0 GPA and submit an official transcript upon completion of their first semester of college and a copy of the next semester’s bill. Mrs. Eleanor Osborne is a retired English teacher and the founder of Speech & Theater Workshop (STW), the co-curricular theatre program at Middleborough High School. STW was established in 1978 to provide opportunities for students to participate in the production and performance of plays from a variety of genres, time periods and cultures. Members are encouraged to take ownership of the program in all areas of management, design, production, performance, publicity and outreach. It is through Mrs. Osborne’s dedication to STW that has given this program its continued success. Elizabeth T. Reynolds (better known as “Betty”), along with her husband, Charles, had nine children & two granddaughters who went through the Middleborough Public Schools. Betty’s many interests included making bird houses, planting flowers & growing vegetables. Her love of the outdoors showed in her bountiful gardens. This scholarship is given in memory of Betty, by her husband, and will be open to MHS alumni continuing their college education relating to the preservation of nature/environmental studies. Consideration will be based primarily on high moral character, leadership and financial need (preference for participating athletes). The John R. Hilsabeck, Jr. Television Production Scholarship is given in honor of Mr. Hilsabeck, a retired Middleborough High School History and Television Production teacher. John Hilsabeck devoted 33 years to Middleborough Public Schools, 16 years as the Television Production teacher and AV Coordinator. Mr. Hilsabeck was the creator and visionary behind the television production program at Middleborough Public Schools and was able to create a studio at the John T. Nichols, Jr. Middle School to support the award winning program for students in Middleborough. This scholarship is available to any graduating senior or graduate in their 1st, 2nd, or 3rd year of their undergraduate program who completed all four years of the television production program at Middleborough High School. Mr. Scott Nelson was a beloved science teacher, department head, and coach at Middleborough High School. Nelly was unique in that he grew up in Middleborough, graduated from Middleborough High School and, following college, worked his entire professional career at Middleborough High School mentoring many students, faculty, staff, coaches, and players. In addition to teaching and coaching at MHS, Nelly worked summers at the Park Department. Nelly was committed to his students and was always there to mentor, give advice and maintained a calming influence through difficult times. His focus and commitment to MHS and the town were unwavering. To those who truly knew him, he is missed for his guidance and support. HOWARD W. MAXIM FOUNDATION SCHOLARSHIP Alumni

Scholarships are available to students who have successfully completed their first year of an accredited college. Applicants must be residents of Middleborough, Lakeville, Rochester or Carver. Scholarships will be limited to one (1) per family per year. Emphasis will be assisting a student(s) who is studying in the science, technical or engineering field and how the student will use his or her education to benefit and/or assist the Middleborough, Lakeville, Rochester or Carver, MA communities and its residents. Second, third or fourth year college students are eligible to apply.

To apply for the Howard Maxim Foundation scholarship a completed application, a copy of the student’s current college grades and an essay on their major goals and how they will use their education to help others in the before mentioned communities must be submitted.

The purpose of the Middleborough Sportsmen’s Club is to promote the use of all firearms and other sportsmen’s tools, with regard to development in proper safety standards and good sportsmanship. It is also our purpose to foster respect for the rights and property of all landowners.

To that end, a scholarship will be awarded by the Middleborough Sportsmen’s Club to any Middleborough High School graduating senior, with at least a 2.5 GPA, and who has been accepted to any accredited two or four-year college, university, or technical school. An official transcript must accompany the scholarship request. The purpose of this scholarship is to provide financial assistance to an individual of future generations who will be protecting our rights and natural resources. This scholarship is available to any senior girl who is a resident of Middleborough, has participated in MYSL softball and is planning on attending an accredited two or four year college or university. Preference is given to applicants who played multiple seasons of MYSL softball and have donated time back to MYSL in the form of volunteerism. Additional preference is given to players who played high school softball, MYSL summer softball, or are scheduled to play MYSL until they “age-out.”

Scholarships are awarded annually to students who are the children or grandchildren of current Mitchell Memorial Club members (in good standing). These scholarships are competitive and determinations are made based on class rank, teacher recommendation, and student involvement in co-curricular activities such as active school citizenship, athletics, performing arts and clubs.

The M.M.C Scholarship Committee is solely responsible for selecting the recipients and the number of scholarships awarded for 2021 will be determined by the pool of eligible and qualified applicants. Students who do receive an award may apply again and those students who receive awards should note that these are “once only”.

Applications need to be picked up at the Mitchell Memorial Club by a club member who is the parent or grandparent of the applicant and are available beginning March 1, 2021. This application needs to be fully completed and returned to the M.M.C. prior to 7:00 P.M. on April 20, 2021. When submitted, application materials should be clearly marked to the “Attention: M.M.C. Scholarship Committee”. Please also note that incomplete applications will not be considered.

ROCKLAND TRUST SCHOLARSHIP 2021

Applicant must live within Rockland Trust’s service area. Rockland Trust employees and their relatives are not eligible. Applicant must be entering college for the first time, enrolled as a full time student (as defined by the accredited educational institution at which the applicant has enrolled), in an undergraduate course of study which offers courses beyond the high school level. Enrollment only at accredited colleges, universities and community colleges will be considered. The scholarship is available only in the first semester of study. Applicant must fall in the top 50% of their graduating class. It is the responsibility of the applicant to notify Rockland Trust of other financial aid that has been awarded to the applicant. The Rockland Trust scholarship award plus any financial aid received by the student must not exceed the total of tuition, books, fees, room and board. Should the recipient withdraw from the educational institution, any refund due should be returned to Rockland Trust. Official transcripts of grades from the high school the applicant attends must be included with the guidance counselor’s portion of the application and must include the two most recent semesters’ grade point average. A copy of the applicant’s highest ACT or SAT scores must be included in the guidance counselor’s portion of the application. If you are chosen as one of the 11 award recipients of the 2021 Rockland Trust scholarship, you will be required to supply a letter from the college to which you have been accepted for the fall semester of 2021 to verify enrollment. If you are chosen and do not supply this letter, you will forfeit your scholarship award.

SOUTH SHORE BAY BAND MUSIC ACHIEVEMENT SCHOLARSHIP AWARD 2021

South Shore Bay Band is a non-profit organization dedicated to giving musicians the opportunity to play and perform concert band music for the South Shore communities. Our band members range in age from high school to senior adults. We perform summer outdoor concerts in the South Shore area. Money raised from these concerts goes to charitable causes, such as the Music Achievement Scholarship Awards. This award is offered to any graduating high school music student planning to attend a college or university in the next academic year. Applicants will be judged on information provided on the completed application form.

TRAVIS ALBERT – CIRCLE OF FRIENDS SCHOLARSHIP 2021

In memory of our son/brother, Travis Albert, this scholarship will be awarded to one or more graduating senior(s) from the Class of 2021. Travis was a student and friend to many in the Middleborough & Lakeville communities. He passed away in 2006 from a very rare and debilitating illness known as Batten Disease. Travis left a lifetime of memories during his 13 years with us, had it not been for the love and support of his friends and family this would not have been possible. Herein lies the reason for this Scholarship Trust; to carry on his memory and to give back some of what was given so generously to Travis and his family. This scholarship was presented for the first time to the “Class of 2011”, which would have been Travis’ graduating class. It is our dream to continue this tradition every year hereafter. Travis taught us all to Hope and Believe and that living a “Life of Quality” is one of the most important lessons to learn.

A scholarship is being offered to a graduating senior of Middleborough High School. Preference will be given to a student entering a field dealing with Wildlife Management, Forestry, or other environmental applications.

A complete application will include the following for the committee’s consideration: 1. Completed Application Form. 2. Two letters of recommendation from non-family members. 3. A typed essay detailing your future plans (250 words or less). 4. A copy of your high school transcript.
