HEALTHCARE BENEFITSThe IDEX ChoiceComp Plan allows you to create your own benefits package. Log on to the Benefits Site at www.idexcorpbenefits.com to review the Benefit Highlights Guide and the options that are available to you. If you are a regular employee and work 30 or more hours per week, you are eligible to participate in the ChoiceComp Plan. Your benefits become effective on your date of hire. You may also enroll your eligible dependents in the Medical, Dental and Vision Plans. Verification of dependent eligibility will be required upon enrollment.

Eligible dependents include:

� Your legal spouse or domestic partner

� Your children, up to age 26

� Your children, of any age, who are physically or mentally disabled and incapable of supporting themselves, and can be claimed as a dependent on your U.S. federal income tax return

Medical & Prescription Drug Benefits You can choose from three medical Plan options, administered by BlueCross BlueShield of Illinois (BCBSIL). If you enroll in a ChoiceComp Medical Plan, your prescription drug benefits are administered by CVS Caremark.

� PPO Plan — A traditional PPO plan with a lower deductible and higher monthly premiums

� HSA Plan — A high deductible plan with a Health Savings Account (HSA)

� Bronze Plan — An Affordable Care Act Bronze PPO plan

Dental Benefits The Dental Plan is administered by Delta Dental. If you enroll in the plan, you can use any dentist and receive benefits, but you’ll pay less when you use in-network dentists. The Plan covers nearly all dental procedures, including orthodontics.

Health Savings Account (HSA) If you enroll in the HSA Plan, IDEX will include a Health Savings Account, which allows you to contribute pre-tax dollars to an account to pay for qualified healthcare expenses. Each year, IDEX will contribute to your HSA, even if you do not make any contributions. IDEX will contribute $500 to your account if you elect single coverage and $1,000 if you elect dependent coverage. When you are initially eligible, the IDEX contribution will be prorated, based on the time of year you enroll in the plan. The money in the account is yours to keep and funds roll over from year to year. For 2020, the IDEX contribution and your contributions may total up to $3,550 for single coverage per year and $7,100 for family coverage per year. When your HSA balance is over $1,000 you are able to start investing your funds. The monthly administration fees to invest your HSA funds are paid for by IDEX.

Flexible Spending Accounts (FSA) IDEX offers three Flexible Spending Accounts that can help you save for qualified healthcare and/or childcare expenses: the Healthcare FSA, the Limited Purpose FSA and the Dependent Care FSA. You are eligible to contribute to the Healthcare FSA if you are enrolled in the PPO or Bronze Plans. If you are enrolled in the HSA Plan, you are eligible for the Limited Purpose FSA and Dependent Care FSA. The Limited Purpose FSA can be used for dental and vision expenses only. Funds do not roll over from year to year and you must re-elect your contribution amount each year during the enrollment period. For 2020, you may contribute up to $2,750 per year to the Healthcare FSA, $2,750 per year for the Limited Purpose FSA and up to $5,000 per year to the Dependent Care FSA.

� Dependent Life InsuranceIDEX provides dependent life insurance in the amount of $10,000 for any eligible dependents.

Voluntary Benefit ProgramsIDEX offers additional benefits for you to purchase and enhance your coverage at a low cost. Voluntary programs include:

� Long-Term DisabilityLong-Term Disability benefits allow you to insure a portion of your salary at a low monthly rate, offering financial security and peace of mind. It provides ongoing benefits equal to 60% of your income, up to a maximum of $15,000 monthly, in the event that a disability prevents you from working for an extended period of time.

� Allstate Accident InsuranceProvides a lump sum benefit to help cover the costs associated with unexpected bills resulting from an accident.

� Allstate Critical Illness Insurance Provides a lump sum benefit to protect you and your family from a financial loss following a prolonged illness or injury.

� LifeLockIdentity theft protection.

� Personal Accident Provides an additional life insurance benefit in the event that you or your dependents (if elected) die from an accident.

� Supplemental Life InsuranceYou can supplement your IDEX company-paid life insurance by purchasing additional life insurance at your own expense.

Electing or Waiving CoverageYou have 31 days from the date of your hire to elect your benefits. If you DO NOT elect benefits during this period, you will automatically be enrolled in the PPO Plan (single coverage) and Long-Term Disability. Payroll deductions for both of these coverages will begin, and you will not be able to change this coverage until Open Enrollment, or if you experience a Qualifying Life Event.

Working Spouse/Domestic Partner SurchargeIf your covered spouse or domestic partner has access to medical coverage through his/her own employer, and you decide to cover them under the IDEX plan, you will be charged a $100 per-month surcharge. The surcharge will only apply if your spouse has access to coverage through his/her employer.

Opting Out of Coverage If you do not elect medical coverage through IDEX, you are eligible to choose the “Opt-Out” dollars.

If you are enrolled in the PPO or Bronze Plan and receive care from one of these providers, IDEX’s medical plan will cover the relevant charges at a higher percentage. IDEX Wellness ProgramIf you enroll in a medical plan through IDEX, you and your spouse/domestic partner are eligible to participate in the IDEX Wellness Program. If you successfully complete the program requirements within 60 days from your hire date, you will receive a monthly wellness credit. You and your spouse/domestic partner must receive a biometric screening and complete an online health assessment within 60 days after your date of hire to be eligible to receive the credit. Sign up for your biometric screening online at www.myinteractivehealth.com. Please use the IDEX sponsor code: K4BRA. If you complete the wellness screening, you will receive a $50 wellness credit per month. If your spouse participates, you will receive a $25 wellness credit per month. When you complete the wellness screening, you will receive your confidential results, along with a goal. In order to keep the wellness credit year to year, you will need to meet the goal set for you. If you are not able to meet the Wellness Program requirements due to medical reasons, you can contact Interactive Health to request a Reasonable Alternative Standard form.

The IDEX Wellness Program gives employees the opportunity to receive a biometric screening, while earning a wellness credit. New employees must receive a biometric screening and complete an online health assessment within 60 days of hire to be eligible for the full wellness credit. Current employees will receive a partial wellness credit for simply completing an online health assessment and a biometric screening. You will also have the opportunity to earn an additional wellness credit for meeting your Personal Health Goal. This applies to both you and your spouse/domestic partner.

IDEX Retirement Savings PlansIDEX offers two retirement benefit plans: The IDEX 401(k) Plan and the Defined Contribution Plan.

401(k) PlanThe 401(k) Plan allows you to save through pre-tax payroll contributions. IDEX matches the first 8% of your contributions at 50% of your contribution to a maximum of 4%. If you do not make an election within 30 days after your date of hire, you will be automatically enrolled in the 401(k) Plan with a 4% contribution. Each subsequent January, your contribution will be increased by 1% until your total contribution is equal to 8%. Once you make an election, the automatic enrollment process does not apply. Visit www.vanguard.com to set your contribution amount.

Defined Contribution PlanThe Defined Contribution Plan is fully funded by IDEX, with no employee contributions required. The Company contributes a match based on a percentage of your salary. The match percentage is calculated based on your age + years of service.

Employee Stock Purchase Plan (ESPP) The Employee Stock Purchase Plan (ESPP) allows you to purchase IDEX company stock through payroll deductions. Owning IDEX shares is one way to save for retirement or future expenses. You can contribute as little as $5 per paycheck up to a maximum of 10% of your eligible earnings. Additional IDEX Benefits Available to YouTuition Reimbursement ProgramIDEX wants to equip you for future development and growth. If you are a regular full-time employee in good standing with at least six months of service, you are eligible to participate in the Tuition Reimbursement Program. The program offers reimbursement of certain expenses for secondary educational institutions, up to $5,250 per year, the maximum amount allowed as exempt from tax withholdings.

Matching Gift ProgramWith the IDEX Matching Gift Program, you can support the educational institutions of your choice. If you are a full-time employee with at least six months of service, IDEX will match your gift of at least $50 to an accredited school located in the U.S. dollar-for-dollar to a maximum of $10,000 per person per year.

Short-Term DisabilityIf you are a full-time, U.S. employee who works 30 hours or more per week, you are eligible to participate in IDEX’s Short-Term Disability benefit. The IDEX Short-Term Disability policy provides disability pay if you are unable to work due to a serious medical condition. The policy benefit pays 100% of regularly scheduled hours of pay for days 1-90 after a five-day waiting period, and 60% of regularly scheduled hours of pay for days 91-180.

Parental LeaveRegular, full-time, non-union, U.S. employees are eligible for the IDEX Parental Leave benefits after completing at least 12 consecutive months of full-time employment. Parental Leave is provided so you can bond with your newborn or newly adopted child.

Eligible employees may take up to a maximum of six weeks of paid Parental Leave during the 12-month period immediately following the birth or new adoption of a child. Parental Leave taken under this policy is compensated at 100% of your regular, straight-time pay, and will be paid on your regular payroll cycle.

How to EnrollIf you have an IDEX e-mail address, be on the lookout for an e-mail inviting you to enroll in your benefits during your first couple of weeks of work. If you do not have an IDEX e-mail address, please call the Benefits Service Center at (877) 896-0683 for help with enrollment.

Healthcare BenefitsGo to www.idexcorpbenefits.com and click on the Start Here button to enroll. You may return at any time to change your elections until your enrollment period ends.

Retirement BenefitsA packet from Vanguard will be mailed to your home with enrollment instructions. If you do not make a 401(k) election, you will be automatically enrolled in the plan with a 4% contribution.
